1.0 INTRODUCTION
This procedure describes the requirements for Entry into a Confined Space to allow
work to be performed in a safe manner (under the Precautions and Mitigations of a
Permit to Work).
The purpose of this Procedure is to assist in identifying the hazards which may
require risk assessment when planning work activities associated with entry into
confined spaces; and to specify the precautions required to achieve adequate control
of the activities to achieve safe and successful confined space entry.
No person shall enter or carry out any work in a Confined Space other than in
accordance with a Permit to Work which provides for safe working conditions by
controlling risks for all personnel entering the Confined Space.

4.0 DEFINITIONS
Acceptable entry The conditions that must exist in a confined space to allow entry and to
conditions ensure that personnel involved with a confined space entry can safely
enter into and work within the space.
Attendant An individual stationed outside a confined space who monitors the
authorized entrants and who performs all attendant's duties assigned in
this procedure. The attendant must be trained and certified as
competent to perform these assigned duties
Authorized A UEP or contractor employee who is authorized by the Area Authority
entrant to enter a confined space under a PTW system.
Authorized Gas A person, who has undergone an accredited training, completed an On-
Tester (AGT) the-Job (OJT) on AGT, holds a valid AGT card, and is certified.
(normally AGTs are recorded in TNA books and not issued cards or
certificates)
Authorized Gas A person who has been suitably briefed on the use of and response to a
Monitor (AGM) continuous gas monitor.
Confined Space Confined Space is a space of any volume which:
is not intended as a regular workplace;
has restricted means for entry and exit;
which can be bodily entered.
may have inadequate ventilation and / or atmosphere which is
either contaminated or oxygen deficient or enriched; and
is at atmospheric pressure during entry. A space which is not at
atmospheric pressure shall not be entered under this Procedure

Deviations Departure from the Design Intent, Standard specification or SOP
Engulfment The surrounding and effective capture of a person by a liquid or finely
divided (flowable) solid substance that can be aspirated to cause death
by filling or plugging the respiratory system or that can exert enough
force on the body to cause death by strangulation, constriction, or
crushing.
Entry The entering of any Confined Space or working with any part of the body
projecting into a manhole or opening of a tank or compartment of a tank
or pit.
Entry Supervisor A person responsible for determining if acceptable entry conditions are
present at a confined space for authorizing entry, for overseeing entry
operations, and for terminating entry, as required by this program. The
entry supervisor will be named in writing on the entry permit. The entry
supervisor may be any properly trained UEP representative/employee/
contractor. He may be the same person as the PA/Performing Authority.

Isolating / De-isolating Authority (IA)
The IA is responsible for isolating specific sections of plant or items of equipment to the
highest quality and for the security of isolation. The IA is also responsible for proving
the integrity of the isolation to the PI and PA and for monitoring the integrity of
isolations whilst they are in force.
There shall be separate Isolation Authorities for (1) Electrical circuitry and (2)
Mechanical/Process systems.
PI may be the IA for mechanical side and for electrical side where only breaker LOTO
are required.
IA shall:
Examine the work site, with the PR and PI, to identify the types of isolation
required and to ensure that all hazards associated with the isolating work activity
have been identified in the TRA and the controls are specified within the Isolating
Certificate (IC)
Confirm on the Isolation Certificate (IC) that any stored and residual energy has
been effectively discharged / released
Demonstrate the effectiveness of isolation to the performing Authority

6.0 PLANNING
6.1 Introduction
Prior to entry into a confined space, the equipment shall be isolated, emptied,
cleaned and gas freed as necessary.

Entry into confined spaces shall be strictly controlled to prevent persons being
exposed to risks associated with toxic and/or flammable fumes and vapors or
NORM in the atmosphere, and with atmospheres which may be deficient (or
enriched) in oxygen OR having inert gas
Gas tests required for hot work shall be carried out separately (the tests done for
entry are not valid for the Hot Work Permit)
Except in circumstances set out in Section-9 (Deviations), no-one may enter or
remain in a confined space unless the atmosphere is within the criteria in
Annexure-2, Permitted Limits for Confined Space Entry & Work
Note: Prior to any entry into a confined space, the atmosphere shall be tested to
determine the necessary precautions required, and an entry certificate issued. An
entry certificate is not a work permit and a relevant Work Permit is required for all
kinds of work to be performed inside a confined space.
Gas or vapor remaining from the process performed in the confined space
Gas or vapor entering from adjoining plant which has not been effectively
isolated. All isolations for confined space entry shall be in accordance with
Isolations procedure
Gas or vapor emitted from scale or sludge, particularly resulting from
mechanical disturbance during access or cleaning or due to the heat from
welding operations
Gas, vapor or fumes produced by operations being carried out in the confined
space such as welding and cutting, brush and spray painting and the use of
adhesives and solvents
Exhaust gases drawn into the confined space from prime movers or heating
equipment
Biological Hazards inside CS during extended time of repair or inspection
(Like tanks repair jobs may continue for weeks and CS is open for ventilation

The ingress of steam, hot water or large quantities of liquids which may cause
scalding or drowning
Communication difficulties
Poor access and egress restricting movement for normal work and escape

Note: Consideration shall be given to the possibility of an unconscious
casualty requiring to be evacuated in an emergency
The requirement for a winch, harness and lifeline are mandatory
Wearing a harness and attached lifeline shall only be considered
impracticable if the line would impede an unassisted exit. The line should still
be deployed even if not attached
Slippery surfaces
Sources of ionizing radiation (e.g. level gauges, sludge and Low Specific
Activity (LSA) scale or Naturally Occurring Radioactive Material (NORM)).

Enrichment of only a few per cent will make materials that will normally only burn
slowly or with difficulty, burn fiercely with catastrophic results for the occupants of the
confined space.
Oxygen cylinders shall be kept outside the confined space where practicable

Hoses supplying oxygen shall be removed from the space during work breaks
Oxygen shall never be used to ‘sweeten’ the atmosphere of a confined space

a. Gas testing for entry shall only be carried out by approved persons who have
been fully trained and certified in gas testing for confined space entry
purposes, i.e. Authorized Gas Tester (AGT) and gas testing equipment shall
be valid for its calibration and in good health.
b. The AGT shall request an Entry Certificate from the Issuing Authority to
confirm that the confined space is isolated and to authorize AGT to proceed
with gas testing and inspection of the confined space, subject to the
conditions below in section (d).
Any special operational conditions within the space, (Nitrogen purged, check
for sludge etc.) are recorded on respective permit.
c. Where practicable, the gas test shall be carried out from outside the confined
space (eg using extension probes).
He shall not enter if the conditions at the entrance are outside the criteria
for ENTRY WITH BA (see Annexure-2)
An Entry Attendant shall be stationed at the entrance so that he is able to
keep the AGT under effective surveillance (see Section 7.1 for definition)
and equipped to raise the alarm if the AGT should get into trouble and
also to warn the AGT of an external emergency
The AGT shall wear BA
He shall only enter the confined space without BA if he is updating a valid
entry certificate which has already certified the space FIT FOR ENTRY
WITHOUT BA
The AGT shall wear a harness with a lifeline and a recovery winch (or
any suitable means for safe & quick rescue in case of emergency like full
body harness with an attached safety line, a mechanical winch and tripod
set up over the access point, use of David armetc.)
This shall only be considered impracticable if the nature of the confined
space is such the lifeline would impede the AGT in making an unassisted
emergency exit

In such a case, the harness will still be worn, and although not attached,
the lifeline and winch will be deployed

6.4 Ventilation
Note: The provision of ventilation does not supersede the requirements to wear
breathing apparatus
The space shall only be classed as fit for ENTRY WITHOUT BA if the oxygen,
NORM and flammable gas levels are the same as measured outside in fresh air,
toxic contaminants do not exceed current occupational exposure limits (see
Annexure-2). Also, the AGT shall confirm that there are no materials left inside which
in his opinion may give off such quantities of vapor and fumes if disturbed that the
atmosphere would cease to be classed as fit for ENTRY WITHOUT BA.
a. When the gas test has been completed, the Permit Issuer shall consider the
results. He shall then issue the appropriate Work Permit, cross-referenced to
the CSE certificate (Annexure-5) on which is specified the period in which he
considers it safe to enter the space, taking into account the nature of the space,
possible contaminants, type of isolations and the type of work proposed.
An Entry Certificate on its own does not authorize entry for any purposes other
than testing by AGT.

b. Entry to the CS shall commence and continue at any time during the period for
which the certificate is valid. If the space is vacated during the period of validity,
a gas test shall be undertaken before re-entry. If continuous gas monitoring
equipment has been in use throughout the vacated period, entry may proceed
without the gas test up to the next scheduled gas test.
c. Entry to the CS shall only be made via a nominated, controlled access point. All
other possible points of access shall have No Entry signs. Until confined space
entry is permitted, access at all possible points of access must be positively
prevented.
d. A sign board (CSE job in progress in VESSEL # xxxxx) should be posted closer
to CS during the job.

space as per Annexure-6

Ensure and log rest periods out in the open air for entrants according to the type
of work and the risk
Keep Entrants under effective surveillance by one or more of the following
methods:

NOTE: One of the Entrants must be, at least, an Authorized Gas Tester and be
equipped with a personal or portable Gas Monitor. Should conditions worsen to
the point where the monitor alarms, the space must be vacated, ventilated and
re-tested until conditions have improved to within the limits set out on the Entry
Certificate.
Even though a gas test may indicate a confined space is fit for ENTRY WITHOUT
BA, if work is likely to generate toxic or flammable vapors (e.g. application of paint
coatings) to such an extent the atmosphere cannot be kept free of contaminants by
forced or natural ventilation, the space shall be treated as NOT fit for ENTRY
WITHOUT BA and the additional precautions listed in Section 6 below shall be
followed.

The Entry Attendant shall perform the duties listed in Section 7.1.
Where the space contains residual fluids (eg water) into which a person might fall so
that his face and BA is submerged, a rescue person who shall be stationed at the
access point, equipped to make an entry wearing BA so as to give immediate help if
he considers it safe to do so.
They shall wear positive pressure BA of a type approved by UEP and shall be
trained in its use
Note 1: Canister Respirators shall not be used under any circumstances. Air-
supplied apparatus / BA cylinder fitted sets shall be used.
9.0 DEVIATIONS
Special circumstances may require a deviation of the foregoing procedure for entry
into confined spaces. Such a deviation shall only be implemented after a rigorous
TRA2 has been undertaken and a written procedure has been approved and
authorized by the appropriate approving authorities as given below:

A Rescue or ER Plan shall be prepared and approved for all Confined Space Work. It
shall detail:
The main objective of the Rescue Plan is to ensure rescue of person in case of an
emergency inside CS. Size / capability of Rescue Team shall commensurate with the
risk level of the job in progress. CSE work MUST not progress in case of non-
availability of rescue team and equipment as identified per ERP.

Rescue equipment must be in good working order, sited as near to the worksite as
possible and personnel trained in its use must be readily available and aware of the
entry.
(b) Fire Safety – consider the risk of fire during rescue operations that could occur as
result of the presence flammable materials, the potential for spark generation and the
integrity of electrical lighting.
(c) First Aid – the plan should identify that the appropriate first aid facilities are
available to deal with emergency situations and that those involved in rescue have
the appropriate level of training to be able to administer that first aid.
Note: A format of Rescue Plan for Confined Space Entry is added as Annexure-3
This Confined Space Entry procedure is replacing the current Middle Indus & Mehar
(MI&M) Confined Space Entry for OMV E&P assets (3.2-HQ-STD-005). The
procedure contains fundamental changes brought in MI&M PTW Structure including
CSE related activities, roles & responsibilities. Due to these changes, it is envisaged
that MI&M sites may need to adopt significant changes. To this end, it is considered
reasonable to allow a specific timeframe for the following to take place at MI&M
Sites.

Appendix-8 Guidelines for Confined Space Entry in Drilling, WO & Rigless Operation
A. Introduction:
The guidelines aim to define minimum requirements for safe execution of the entry and work
in Confined Spaces.
Confined Spaces within the D&C work location includes but not limited to:
Cellar,
Mud tanks,
Cuttings tanks,
Water tanks.
B. Scope
Drilling Contractors PTW System shall be applicable at all D&C sites, therefore all CSE jobs
shall be performed under Drilling Contractor CSE Procedure provided:

Pre-job safety meeting shall be carried out. The work be discussed by the Permit
Issuer/Permit Approver with the person(s) involved in the work.
D.2 Gas Testing:
Gas testing (oxygen, LEL, CH4 & H2S) shall be carried out by trained & authorized
person prior to start of work. Continuous gas monitoring shall be carried to ensure
conditions are safe to continue work. Requirements for frequency of gas testing shall
be agreed and documented in risk assessment and same to be specified in the
permit.
Gas tests required for hot work shall be carried out separately (the tests done for
entry are not valid for the Hot Work Permit).
Written procedure for gas testing shall be available and know to Permit Authorities &
AGT. Procedure shall contain permissible limits for required parameters.
The person carried out gas testing (Authorized Gas Tester - AGT) shall authorized
based on written test or training certificate/attendance form. List of AGT shall be
maintained by Drilling Contractor and shall be approved by WSL or his designee.
D.3 Isolation:
The equipment shall be isolated, emptied, cleaned and gas freed as necessary.
Isolation shall be carried out as per Drilling Contractor Energy Isolation Procedure,
however for CSE positive isolation shall be mandatory. In case of deviation from
positive isolation, isolation risk assessment shall be carried out to demonstrate
activity to ALARP level.
D.4 BA
Entry without BA is only permissible if the oxygen, NORM and flammable gas levels
are the same as measured outside in fresh air, toxic contaminants do not exceed
current occupational exposure limits (see Table-1).
