1.0 INTRODUCTION

This procedure describes the requirements for Entry into a Confined Space to allow
work to be performed in a safe manner (under the Precautions and Mitigations of a
Permit to Work).

2.0 PURPOSE AND SCOPE

The purpose of this Procedure is to assist in identifying the hazards which may
require risk assessment when planning work activities associated with entry into
confined spaces; and to specify the precautions required to achieve adequate control
of the activities to achieve safe and successful confined space entry.

No person shall enter or carry out any work in a Confined Space other than in
accordance with a Permit to Work which provides for safe working conditions by
controlling risks for all personnel entering the Confined Space.

NOTE: Confined spaces include but are not limited to:

 storage tanks, process vessels, boilers, silos and other tank-like compartments
usually having only a manhole for entry
 open topped spaces such as pits, or submersible pump pits which are not
subject to good natural ventilation
 pipes, sewers, tunnels, silos, shafts and ducts, and similar structures
 Excavation as per UEP Ground Disturbance procedure
 Well cellars; Sump pits

The hazards that this document addresses arise through the confined nature of the
place of work in combination with one or more of the following:

 The possible presence of flammable or toxic substances


 An oxygen-deficient atmosphere
 An oxygen-enriched atmosphere
 Ingress of solids or liquids
 The presence of excessive heat

4.0 DEFINITIONS

The following definitions shall assist in the understanding of this procedure:

Acceptable entry The conditions that must exist in a confined space to allow entry and to
conditions ensure that personnel involved with a confined space entry can safely
enter into and work within the space.
Attendant An individual stationed outside a confined space who monitors the
authorized entrants and who performs all attendant's duties assigned in
this procedure. The attendant must be trained and certified as
competent to perform these assigned duties
Authorized A UEP or contractor employee who is authorized by the Area Authority
entrant to enter a confined space under a PTW system.
Authorized Gas A person, who has undergone an accredited training, completed an On-
Tester (AGT) the-Job (OJT) on AGT, holds a valid AGT card, and is certified.

(normally AGTs are recorded in TNA books and not issued cards or
certificates)
Authorized Gas A person who has been suitably briefed on the use of and response to a
Monitor (AGM) continuous gas monitor.
Confined Space Confined Space is a space of any volume which:
 is not intended as a regular workplace;
 has restricted means for entry and exit;
 which can be bodily entered.
 may have inadequate ventilation and / or atmosphere which is
either contaminated or oxygen deficient or enriched; and
 is at atmospheric pressure during entry. A space which is not at
atmospheric pressure shall not be entered under this Procedure
Confined Space CSE certificate is document which authorizes the controlled entry in to a
Entry Certificate CS in connection with a work permit, It provides information regarding
the CS atmosphere, testing frequency and a set of guidelines & work
criteria to mitigate risks.
A separate suitable PTW shall be issued to perform work as alone CSE
Certificate is not a permit
Deviations Departure from the Design Intent, Standard specification or SOP
Engulfment The surrounding and effective capture of a person by a liquid or finely
divided (flowable) solid substance that can be aspirated to cause death
by filling or plugging the respiratory system or that can exert enough
force on the body to cause death by strangulation, constriction, or
crushing.
Entry The entering of any Confined Space or working with any part of the body
projecting into a manhole or opening of a tank or compartment of a tank
or pit.
Entry Supervisor A person responsible for determining if acceptable entry conditions are
present at a confined space for authorizing entry, for overseeing entry
operations, and for terminating entry, as required by this program. The
entry supervisor will be named in writing on the entry permit. The entry
supervisor may be any properly trained UEP representative/employee/
contractor. He may be the same person as the PA/Performing Authority.

Gas Free A non-explosive atmosphere, but not necessarily a completely hazard


free atmosphere.
Hazardous An atmosphere that may expose personnel to the risk of death,
Atmosphere incapacitation, impairment of ability to self-rescue (i.e., escape unaided
from a permit space), injury, or acute illness from one or more of the
following causes:

(1) Flammable gas, vapor, or mist in excess of 10% of its lower


explosivity limit (LEL);
(2) Airborne combustible dust at a concentration that meets or exceeds
its LEL;

NOTE: This concentration may be approximated as a condition in which


the dust obscures vision at a distance of 5 feet (1.52 m) or less.

(3) Atmospheric oxygen concentration below 19.5% or above 23.5%;


(4) Atmospheric concentration of any substance which could result in
personnel exposure in excess of its occupational exposure limit
(OEL) or permissible exposure limit (PEL);

NOTE: An atmospheric concentration of any substance that is not


capable of causing death, incapacitation and impairment of ability to
self-rescue, injury, or acute illness due to its health effects is not covered
by this provision.

(5) NORM (naturally occurring radioactive material)

- Any closed equipment reading of 25 micro roentgens per hour


(25 uR/hr) including background.
- Any open equipment reading 50 (50 uR/hr) including background.

(6) Any other atmospheric condition that is immediately dangerous to


life or health.

Immediately Any condition that poses an immediate or delayed threat to life or that
dangerous to life would cause irreversible adverse health effects or that would interfere
or health (IDLH) For example, H2S at 100 ppm and CO at 1200 ppm are considered
IDLH, NIOSH has established a complete list of IDLH Values of various
gases
Isolation Isolation or Energy Isolation is the process of ensuring:

(1) That all means of energy such as electrical, mechanical, hydraulic,


pneumatic, chemical, and thermal etc. have been disconnected and
unintended re-energizing is impossible.

(2) That entire system under consideration has been purged, flushed,
drained and depressurized in a way that all hydrocarbon fumes,
vapours, mists, sludge, chemicals and remnants of toxic, flammable and
hazardous; do not exist.

Oxygen deficient An atmosphere containing less than 19.5 percent oxygen by volume.

Oxygen enriched An atmosphere containing more than 23.5 percent oxygen by volume.

Retrieval system The equipment (including a retrieval line, chest or full-body harness,
wristlets, if appropriate, and a lifting device or anchor) used for non-entry
rescue of persons from permit spaces.
Testing The process by which the hazards that may confront entrants of a permit
space are identified and evaluated. Testing includes specifying the tests
that are to be performed in the permit space and at a pre-set frequency.

NOTE: Testing enables employers both to devise and implement


adequate control measures for the protection of authorized entrants and
to determine if acceptable entry conditions are present immediately prior
to, and during, entry.

5.2 Brief summary of all above R&Rs are tabulated below:

Operations shall be responsible for:

a) Operating in accordance with this procedure and self-regulation of compliance


b) Justifying any deviations, using appropriate risk assessment techniques, in all cases
where existing facilities do not permit the defined method of confined space entry, or
where other deviations are proposed. Such deviations shall be formally approved as
per risk based DOA provided in Section 9.0 Deviations and recorded together with
the risk assessment and necessary additional precautions
c) Maintaining a recorded system of training for confined space entrants, entry
attendants
d) on-site rescue teams
e) Appointing Competent Persons to take responsibility regarding this procedure
Engineering Services shall be responsible for:

a) Provision of authoritative advice on confined space entry methods


b) Evaluating relevant technological developments and identifying the potential for their
application
HSE shall be responsible for:

a) Custody of this procedure, in liaison with Engineering Services and Operations. SPA
for custody and revision of this procedure shall be the HSE Manager. HSE
Manager shall provide assurance to VP Technical that correct procedure has
been provided and is being complied with
b) Providing assistance on provision of a formal risk assessment and advisory service,
to Field Operations on the adequacy of proposed confined space entry methods in
cases where hazards or risks are severe
Permit Issuer (PI) shall ensure (in view of CSE related activities):

 Issues confined space permit, and have it approved by AA


 Verify that all tests have been conducted and all procedures and equipment are
in place before signing the permit
 Personnel involved are aware of specific hazards and their controls, ensures
validity of training cards of personnel involved before issuing the PTW and
documentation of tools and material i.e. checklists
 Ensure that all activity is being done as per PTW scope and gas test are being
done as per specified frequency
 The 1st entry into a confined space is authorized and completed in presence of
Area Authority
 Adequately trained, competent and certified Authorized Gas Testers are
appointed
 All the appropriate control measures as indicated in the TRA and PTW have been
put in place prior to allowing a Permit to be executed
 Confirm that the Permit Receivers fully understand the scope of the task and that
other members of the work party have been fully briefed via a documented safety

 Identify possible effects or links to other work permits or simultaneous operations


in the area and inform all personnel that can be affected
 Exercises “Pause & re-think” for the activity
 The entry process continues if and only if the safe conditions specified in the
permit(s) and certificate(s) prevail
 Agreement on suitable energy isolation methods as per UEP Energy Isolation
Procedure
 Associate Certificates and appropriate level of risk assessment are completed
and appropriately reviewed and endorsed by appropriate authorities
 All requirements of the PTW Procedure are applied
 Appropriate permits (Hot, Spark Potential or Cold Work) and associated
certificates (Isolation etc.) are issued for the job
 There is adequate evidence regarding medical fitness of the CSE personnel,
especially they don’t have claustrophobia
 Terminate entry and cancel the permit if necessary
 Records & reports the violations or shortcomings
 Verify that rescue services are available and the means for summoning them are
operable
 Endorse changes of CS related crew
Area Authority (AA)

Only the Area Authority shall approve a Permit to Work for initial or first entry into a
Confined Space. Subsequent entries into the confined space for the same job may be
authorized by the Authorized Permit Issuer of the area.

Description of Area Authority is as follows:


 Prior to signing the Certificate, the AA should physically verify the conditions and
enter the associated Work Permit number on the Certificate
 The AA shall approve the relevant TRA2 after it has been reviewed by the
Performing Authority Team Leader
 The AA shall highlight any special conditions or instructions to the PR/PA that
may be required to perform the work activity safely
 The AA shall review and sign on the entry certificate after ensuring that all
requirements of the risk assessment and certificates have been fulfilled
 Immediately before first entry to the Confined Space, the Area Authority shall
witness a test being carried out within the confined space
 The AA shall have a prioritized plan for having OJTs prepared. Once the OJTs
are prepared, the AA shall conduct the OJTs of the team and tag them
‘competent’ as per TNA section. Note: This responsibility can be delegated to
Production Supervisor. In case of computer-based training (CBT) on OJTs, he
shall ensure the integrity of related CBT and tag them ‘competent’ as per TNA
section
Note: An updated Table of Area Authorities shall always be maintained with the Head
of Operations. (Refer to PTW Procedure for the list of approved Area Authorities).
Permit Requester / Receiver shall ensure:

 Prepare the appropriate type of the permit properly


 In conjunction with the PI complete the Confined Space Entry Certificate and any

other Certificates
 Produce an appropriate Task Risk Assessment reviewed by Performing Authority
Team Leader
 Prepare a Toolbox Talk identifying work and location hazards, and all necessary
controls. The work site shall be examined when preparing the Toolbox Talk
 Ensure a Toolbox Talk is conducted
 Personnel involved are adequately trained and competent in the use of
appropriate safeguards and test equipment for controlling those hazards
 Ensure that the PI has authorized the Permit before it is given to the Performing
Authority
 Ensure that there is a recognized Performing Authority in-charge of the work for
all periods when work is being done under the Permit
 Ensure adequate attendants i.e. SW(s) are assigned for the CS
 Ensure that the Performing Authority is briefed on the work, hazards and controls
 Ensure a set of emergency procedures (ERP / Rescue Plan etc.) relevant to the
confined space entry task has been developed, and that all the work crew are
familiar with those procedures. Rescue services are available and that the means
for summoning them are operable
Isolating / De-isolating Authority (IA)

The IA is responsible for isolating specific sections of plant or items of equipment to the
highest quality and for the security of isolation. The IA is also responsible for proving
the integrity of the isolation to the PI and PA and for monitoring the integrity of
isolations whilst they are in force.

There shall be separate Isolation Authorities for (1) Electrical circuitry and (2)
Mechanical/Process systems.

PI may be the IA for mechanical side and for electrical side where only breaker LOTO
are required.

IA shall:
 Examine the work site, with the PR and PI, to identify the types of isolation
required and to ensure that all hazards associated with the isolating work activity
have been identified in the TRA and the controls are specified within the Isolating
Certificate (IC)
 Confirm on the Isolation Certificate (IC) that any stored and residual energy has
been effectively discharged / released
 Demonstrate the effectiveness of isolation to the performing Authority

Performing Authority (PA)

Performing Authority can be the Entry Supervisor. PA is responsible for ensuring


that:
 A Toolbox Talk is conducted to inform the work party of the work procedures,
TRA’s and the requirements of the Permit, ensuring that all members of the work
party understand what is required
 A confined space is properly isolated before entry is permitted
 The Permit and all associated certificates are displayed at the worksite whenever
work is underway
 Provide consent on Isolation certificate if agrees the isolation after witnessing the
isolation effectiveness
 The atmosphere in the confined space has been fully tested for oxygen, toxic,
e.g. H2S, NORM* and flammable materials, by an authorized gas tester and that
all procedures and equipment specified by the permit are in place before
endorsing the permit and allowing entry to begin
 Ensure acceptable entry conditions are maintained as per PTW / Certificate
requirements during the entire period of entry
 Stay at or in close visual contact with the worksite at all times, to supervise the
work party or obtain the necessary shift change endorsement
 Ensure adequate ERP / Rescue Plan relevant to the confined space entry task
has been developed, and that all the work crew are familiar with those
procedures. Rescue services are available and that the means for summoning
them are operable

Note: *NORM survey to be carried out in close proximity of confined space activity area.
All sites shall have NORM meters and be calibrated as per other portable gas detectors PM
plan.
Confined Space Attendants

 Maintain continuous count and identification of authorized entrants


 Remain outside the space until relieved
 Communicate with entrants as necessary to monitor entrant status
 Monitor activities inside and outside the permit space
 Order exit if required
 Summon rescuers if necessary
 Prevent unauthorized entry into the confined space
 Perform non-entry rescues if required
 Not perform other duties that interfere with the duty to monitor and protect the
safety of authorized entrants
 Barricade the manway or entry point when job is stopped

Authorized Entrants

 Be able to recognize signs or symptoms of exposure


 Understand the consequences of exposure to hazards
 Know how to use any needed equipment

 Communicate with attendants as necessary


 Alert attendants to the warning signs or the existence of a hazardous condition
 Exit as quickly as possible whenever ordered to do so

Note: Please see Section 7.2 for further details.


Authorized Gas Testers

AGTs ensures that:


 The equipment they use is suitable for the atmosphere to be tested, properly
calibrated and maintained, and within its validation period
 They are sufficiently knowledgeable to interpret the results correctly
 They are aware of the likely contaminants and the appropriate testing regime
 Record subsequent gas test results as per predefined frequency mentioned on
PTW. (and, at beginning of each shift, after work breaks)
 When a test result indicates that it is NOT safe to work, the AGT shall:
o Tell PA to immediately stop any current work
o Tell the Permit Receiver and PI that it is not safe to work, and why, so
that the problem can be dealt with. The PI will immediately withdraw
the Permit from the PR until such time as the area is proven to be safe
o Stop the work immediately and ask CSE attendant to initiate
evacuation if required and shall inform PA and PI about stoppage with
a reason

During CSE permit execution phase:


 Proceed to the Confined Space and under the conditions in CSE procedure and
the TRA Level-2; performs gas tests and inspects the space
 Determine by gas testing and inspection, the requirements for entry by the
workforce
 Record the initial gas test results
 Tick to indicate the overall requirement for Breathing Apparatus (BA)
Note: once ticked the BA requirement cannot be altered unless the Entry
Certificate is cancelled, and a new certificate issued
 Inspect the confined space externally i.e. using hand torch or as per Level II TRA
guidelines and Provides feedback to the PI any observations regarding the
confined space. (Residual sludge, awkward access etc.)

6.0 PLANNING

6.1 Introduction

Prior to entry into a confined space, the equipment shall be isolated, emptied,
cleaned and gas freed as necessary.

 Planning for confined space entry shall include the following:

o Confined Space Hazard Identification and assessment of risks


o Gas Testing and inspection
o Issuance of Entry Certificate along with validity
o Procedures for Control of Work

Note: A summary of the overall process is shown in Annexure-1.

 Entry into confined spaces shall be strictly controlled to prevent persons being
exposed to risks associated with toxic and/or flammable fumes and vapors or
NORM in the atmosphere, and with atmospheres which may be deficient (or
enriched) in oxygen OR having inert gas
 Gas tests required for hot work shall be carried out separately (the tests done for
entry are not valid for the Hot Work Permit)
 Except in circumstances set out in Section-9 (Deviations), no-one may enter or
remain in a confined space unless the atmosphere is within the criteria in
Annexure-2, Permitted Limits for Confined Space Entry & Work

Note: Prior to any entry into a confined space, the atmosphere shall be tested to
determine the necessary precautions required, and an entry certificate issued. An
entry certificate is not a work permit and a relevant Work Permit is required for all
kinds of work to be performed inside a confined space.

6.2 Hazard Identification and Assessment of Risk

A detailed hazard identification and Assessment of Risk shall be made to establish


whether the proposed confined space entry can be performed safely in accordance
with this Practice (see Annexure 1, 3, 4 and Section-9). Some of the dangers which
should be considered are as follows:

a. Dangerous concentrations of gases and vapors can arise from sources


inside or outside a confined space. These include:

 Gas or vapor remaining from the process performed in the confined space
 Gas or vapor entering from adjoining plant which has not been effectively
isolated. All isolations for confined space entry shall be in accordance with
Isolations procedure
 Gas or vapor emitted from scale or sludge, particularly resulting from
mechanical disturbance during access or cleaning or due to the heat from
welding operations
 Gas, vapor or fumes produced by operations being carried out in the confined
space such as welding and cutting, brush and spray painting and the use of
adhesives and solvents
 Exhaust gases drawn into the confined space from prime movers or heating
equipment
 Biological Hazards inside CS during extended time of repair or inspection
(Like tanks repair jobs may continue for weeks and CS is open for ventilation
purpose

b. Other hazards arise due to:

 Mechanical equipment in the space

 The ingress of steam, hot water or large quantities of liquids which may cause
scalding or drowning
 Communication difficulties
 Poor access and egress restricting movement for normal work and escape
 Poor access and egress for rescue
 Note: Consideration shall be given to the possibility of an unconscious
casualty requiring to be evacuated in an emergency
 The requirement for a winch, harness and lifeline are mandatory
 Wearing a harness and attached lifeline shall only be considered
impracticable if the line would impede an unassisted exit. The line should still
be deployed even if not attached
 Slippery surfaces
 Sources of ionizing radiation (e.g. level gauges, sludge and Low Specific
Activity (LSA) scale or Naturally Occurring Radioactive Material (NORM)).

 Excessive temperature in the confined space (causing personnel heat stress).
 Possible ingress of fumes from drain systems, etc.
 Possible collapse of excavations. Adequate shoring to be in place, prior to
entry

c. Hazards from oxygen-enrichment

The special dangers of working in oxygen-enriched atmospheres cannot be over-


emphasized. Oxygen enrichment may occur by leakage of oxygen supplies or by
build-up of oxygen during oxygen-rich flame cutting processes.

Enrichment of only a few per cent will make materials that will normally only burn
slowly or with difficulty, burn fiercely with catastrophic results for the occupants of the
confined space.

Every precaution shall be taken to avoid oxygen enrichment. In particular:

 Oxygen cylinders shall be kept outside the confined space where practicable
 Oxygen supplies shall be isolated outside the space during work breaks
 Hoses supplying oxygen shall be removed from the space during work breaks
 Oxygen shall never be used to ‘sweeten’ the atmosphere of a confined space
 The space shall be adequately ventilated at all times

d. Hazards from oxygen deficiency and inert gas accumulations.

There is also a danger of oxygen deficiency due to combustion processes,


purging with inert gas, inert gas welding (MIG/TIG) or oxidation processes (eg
rusting) occurring in steel vessels left completely closed for some time
Particular care needs to be exercised while using Nitrogen

6.3 Gas Testing

a. Gas testing for entry shall only be carried out by approved persons who have
been fully trained and certified in gas testing for confined space entry
purposes, i.e. Authorized Gas Tester (AGT) and gas testing equipment shall
be valid for its calibration and in good health.

b. The AGT shall request an Entry Certificate from the Issuing Authority to
confirm that the confined space is isolated and to authorize AGT to proceed
with gas testing and inspection of the confined space, subject to the
conditions below in section (d).
Any special operational conditions within the space, (Nitrogen purged, check
for sludge etc.) are recorded on respective permit.

c. Where practicable, the gas test shall be carried out from outside the confined
space (eg using extension probes).

d. In some circumstances it may not be possible to sample a representative


portion of the space from the entrance, taking into account ventilation
arrangements and the possibility of heavier than air gas remaining in low
areas. In this event, the AGT may have to enter the space in order to
complete his tests. If this is necessary, the first three sections of the Entry
Certificate constitute permission for the AGT to proceed with his tests,
entering if necessary.

The conditions for the entry of the AGT shall include:

 He shall not enter if the conditions at the entrance are outside the criteria
for ENTRY WITH BA (see Annexure-2)
 An Entry Attendant shall be stationed at the entrance so that he is able to
keep the AGT under effective surveillance (see Section 7.1 for definition)
and equipped to raise the alarm if the AGT should get into trouble and
also to warn the AGT of an external emergency
 The AGT shall wear BA
 He shall only enter the confined space without BA if he is updating a valid
entry certificate which has already certified the space FIT FOR ENTRY
WITHOUT BA
 The AGT shall wear a harness with a lifeline and a recovery winch (or
any suitable means for safe & quick rescue in case of emergency like full
body harness with an attached safety line, a mechanical winch and tripod
set up over the access point, use of David armetc.)
 This shall only be considered impracticable if the nature of the confined
space is such the lifeline would impede the AGT in making an unassisted
emergency exit

 In such a case, the harness will still be worn, and although not attached,
the lifeline and winch will be deployed
 Reviving equipment ready for immediate use shall be kept close at hand

6.4 Ventilation

Natural ventilation within a CS may be inadequate and forced ventilation may be


considered to gas-free CS before entry. Points to consider when selecting method of
ventilation during the TRA are as follows:

 Type of ventilation to be used e.g. forced (usually a blower fan, trunking or


eductors) or extract (usually exhaust fan and trunking)
 Forced is usually the preferred method, however for welding extract ventilation is
the preferred method as it draws the fumes away (ensures that fumes are drawn
away from the welder’s airways)
 Layout of space, position of openings
 Properties of pollutants for effective circulation of air
 Fresh air to be drawn from an area, which is not contaminated
 Ventilation exhaust routed away from possible sources of re-entry
 Measurement of pollutants and their limits
 Time required for ventilation equipment to reach adequate air circulation
 Earthing of ventilation equipment
 The airline must be introduced at or extended to the bottom of the vessel This
ensures removal of heavy gases or vapours and effective circulation of air
 The build-up of static electricity is more prevalent during cool dry conditions,
typically below 50% relative humidity. Depending on the work environment, anti-
static clothing and special static removal devices may be necessary to prevent
ignition from static electrical discharge

Note: The provision of ventilation does not supersede the requirements to wear
breathing apparatus

6.5 Criteria for Confined Space Entry

The space shall only be classed as fit for ENTRY WITHOUT BA if the oxygen,
NORM and flammable gas levels are the same as measured outside in fresh air,
toxic contaminants do not exceed current occupational exposure limits (see
Annexure-2). Also, the AGT shall confirm that there are no materials left inside which
in his opinion may give off such quantities of vapor and fumes if disturbed that the
atmosphere would cease to be classed as fit for ENTRY WITHOUT BA.

6.6 Entry Certificate Issue and Validity

a. When the gas test has been completed, the Permit Issuer shall consider the
results. He shall then issue the appropriate Work Permit, cross-referenced to
the CSE certificate (Annexure-5) on which is specified the period in which he
considers it safe to enter the space, taking into account the nature of the space,
possible contaminants, type of isolations and the type of work proposed.
An Entry Certificate on its own does not authorize entry for any purposes other
than testing by AGT.

b. Entry to the CS shall commence and continue at any time during the period for
which the certificate is valid. If the space is vacated during the period of validity,
a gas test shall be undertaken before re-entry. If continuous gas monitoring
equipment has been in use throughout the vacated period, entry may proceed
without the gas test up to the next scheduled gas test.

c. Entry to the CS shall only be made via a nominated, controlled access point. All
other possible points of access shall have No Entry signs. Until confined space
entry is permitted, access at all possible points of access must be positively
prevented.

d. A sign board (CSE job in progress in VESSEL # xxxxx) should be posted closer
to CS during the job.

e. Pre-Specified / pre-determined no. of entrants at a time will enter the CS.

7.0 PROCEDURE FOR ENTRY INTO CONFINED SPACES WITHOUT BA

(See Section 6.5 and Annexure-2).

7.1 Entry Attendant

A suitably trained Entry Attendant shall be stationed at the designated entrance to


the confined space and shall:

 Maintain a record of personnel, equipment and tools entering and leaving the
space as per Annexure-6
 Maintain access control and allow only Specified / pre-determined no. of entrants
at a time to enter the CS
 Ensure and log rest periods out in the open air for entrants according to the type
of work and the risk
 Keep Entrants under effective surveillance by one or more of the following
methods:

- line of sight (not always possible);


- voice contact (allowing for distance and ambient noise);
- radio with agreed periodicity of contact;
- pre-arranged signals on air-klaxons, whistles, etc.;
- pre-arranged lifeline signals;
- distress signal unit.

 Be equipped with a device (eg radio, whistle, etc.) to summon assistance rapidly
if entrants get into trouble.

Note 1: Such assistance shall consist of readily available rescue and


resuscitation equipment and personnel trained in its use.
Note 2: As part of Rescue Plan for the CSE, a rescue team from available
resources shall be nominated and informed which must attend the
pre-job briefing to ensure familiarity with the proposed work and

location. Resources shall be managed according to situation


requirement.

 Ensure an adequate supply of breathable air is maintained whilst Entrants are


inside the confined space
NOTE: The Entry Attendant has a vital function to perform. The Permit Issuer must
confirm that the Entry Attendant understands his duties and is competent to perform
them. He should have gone through formal 3rd party certification and have valid
certification card.

Under no circumstances may the Entry Attendant:

 enter the Confined Space &


 be assigned other jobs
 leave the confined space unattended while entry is in progress.

7.2 Authorized Entrants

Entrants to the confined space shall:

 Wear suitable Personal Protective Equipment (PPE), as required


 Wear a lifeline and harness as specified on Work Permit
 According to the type of work and the risk, take adequate rest periods out in the
open air
 Occasionally monitor for oxygen (O2), Hydrocarbons and H2S
 Be able to recognize signs or symptoms of exposure
 Be able to understand and act on CSE attendant signals and agreed steps
 Understand the consequences of exposure to hazards
 Know how to use any needed equipment
 Communicate with attendants as necessary
 Alert attendants to the warning signs or the existence of a hazardous condition
 Exit as quickly as possible whenever ordered to do so

NOTE: One of the Entrants must be, at least, an Authorized Gas Tester and be
equipped with a personal or portable Gas Monitor. Should conditions worsen to
the point where the monitor alarms, the space must be vacated, ventilated and
re-tested until conditions have improved to within the limits set out on the Entry
Certificate.

7.3 Work Activity Affecting ENTRY WITHOUT BA Status

Even though a gas test may indicate a confined space is fit for ENTRY WITHOUT
BA, if work is likely to generate toxic or flammable vapors (e.g. application of paint
coatings) to such an extent the atmosphere cannot be kept free of contaminants by
forced or natural ventilation, the space shall be treated as NOT fit for ENTRY
WITHOUT BA and the additional precautions listed in Section 6 below shall be
followed.

8.0 ADDITIONAL PROCEDURE FOR ENTRY INTO CONFINED SPACES WITH BA

See Annexure-2.

8.1 Entry Attendant

The Entry Attendant shall perform the duties listed in Section 7.1.

Where the space contains residual fluids (eg water) into which a person might fall so
that his face and BA is submerged, a rescue person who shall be stationed at the
access point, equipped to make an entry wearing BA so as to give immediate help if
he considers it safe to do so.

8.2 Authorized Entrants

Entrants shall perform the duties listed in Section 7.2, plus:

 They shall wear positive pressure BA of a type approved by UEP and shall be
trained in its use

Note 1: Canister Respirators shall not be used under any circumstances. Air-
supplied apparatus / BA cylinder fitted sets shall be used.

9.0 DEVIATIONS

Special circumstances may require a deviation of the foregoing procedure for entry
into confined spaces. Such a deviation shall only be implemented after a rigorous
TRA2 has been undertaken and a written procedure has been approved and
authorized by the appropriate approving authorities as given below:

Any such deviation to this Procedure shall only apply to a specific task.

10.0 CLOSURE OF PERMIT AND RESTORATION OF CONFINED SPACE

 When all work is complete and the vessel or CS is ready to be returned to


service, a supervisor shall conduct a post entry review. A site-specific checklist
should be used for proper restoration. In addition to items listed on the permit,
questions to consider include:
o Are all personnel out of the confined space?
o Are all blinds removed using the blind list?
o Are all tools, equipment, spares and materials removed?
o Ventilation blower and temporary lighting removed from the confined
space
o All temporary electrical connections, cables, fittings, etc. removed from
the confined space
o Scaffoldings dismantled and materials removed from inside the confined
space?
o Proper housekeeping inside and around the confined space ensured?
o Are all manways and flanges closed and sealed?
o Have start-up procedures been reviewed?

11.0 RESCUE / ER PLAN

A Rescue or ER Plan shall be prepared and approved for all Confined Space Work. It
shall detail:

 Exact location of Confined Space


 Those involved
 Means of communication
 Work description
 Sketches of confined space internals (where appropriate)
 Entry Checklist
 Actions to be taken in the event of an emergency

The main objective of the Rescue Plan is to ensure rescue of person in case of an
emergency inside CS. Size / capability of Rescue Team shall commensurate with the
risk level of the job in progress. CSE work MUST not progress in case of non-
availability of rescue team and equipment as identified per ERP.

The plan shall include consideration of the following issues:

(a) Rescue Equipment – the following may be considered:


 Safety harnesses with an adequate length of rope; in some situations,
attached to a tripod and winch
 Hand torches or cap lamps (intrinsically safe according to situation)
 First aid equipment
 Firefighting equipment
 Audible alarms for summoning help
 Means of Primary & Secondary communication

Rescue equipment must be in good working order, sited as near to the worksite as
possible and personnel trained in its use must be readily available and aware of the
entry.

(b) Fire Safety – consider the risk of fire during rescue operations that could occur as
result of the presence flammable materials, the potential for spark generation and the
integrity of electrical lighting.

(c) First Aid – the plan should identify that the appropriate first aid facilities are
available to deal with emergency situations and that those involved in rescue have
the appropriate level of training to be able to administer that first aid.

Note: A format of Rescue Plan for Confined Space Entry is added as Annexure-3

12.0 TRANSITORY PROVISION

This Confined Space Entry procedure is replacing the current Middle Indus & Mehar
(MI&M) Confined Space Entry for OMV E&P assets (3.2-HQ-STD-005). The
procedure contains fundamental changes brought in MI&M PTW Structure including
CSE related activities, roles & responsibilities. Due to these changes, it is envisaged
that MI&M sites may need to adopt significant changes. To this end, it is considered
reasonable to allow a specific timeframe for the following to take place at MI&M
Sites.

 Training & Certification required for CS Entrants, Rescue Team Members, CSEA,
AGT
 OJT Assessments
 Any specific responsibility defined in GSR Procedures.
 Any specific attire, apparel, badge or sign that will distinguish any specific
authority.

The timeframe provided in this Transitory Provision is for completion of all activities
defined above by the 31-Dec-2020.

13.0 TRAINING NEED ANALYSIS (TNA)

All personnel likely to be involved in CSE, either as PI, PA, AA, Entrants, Attendants,
Rescue team members or AGTs shall undergo suitable training to ensure that they
possess the understanding, knowledge and skills necessary to safely perform their
assigned duties.

Appendix-8 Guidelines for Confined Space Entry in Drilling, WO & Rigless Operation

A. Introduction:
The guidelines aim to define minimum requirements for safe execution of the entry and work
in Confined Spaces.
Confined Spaces within the D&C work location includes but not limited to:
 Cellar,
 Mud tanks,
 Cuttings tanks,
 Water tanks.
B. Scope
Drilling Contractors PTW System shall be applicable at all D&C sites, therefore all CSE jobs
shall be performed under Drilling Contractor CSE Procedure provided:
 The Contractor shall demonstrate that the Procedure is principally complaint with the
guidelines. Where any gaps are found contractor will be required to submit:
o Action Plan to bridge gap with the guidelines, or
o Develop standing instruction for its workforce to meet the guidelines requirements
For UEP Rigless location, all Service Companies shall be work under UEP PTW System and
CSE Procedure
C. Roles & Responsibility
R&R with respect to Control-of-Work (CoW) Framework are defined in UEP Procedure for
CSE (HSE-SFT-000-IN-PRO-00002-00) & CoW (HSE-SFT-000-IN-PRO-00003-01),
however R&R with respect to the guidelines are & site specific roles and responsibilities with
entry and work in Confine Space is defined:

 Coordinate development of contractor action plan to bridge guidelines
requirements with drilling contractor & HSE.
 Effective Implementation of the guidelines in D&C Operations

C.5 Performing Authority / Permit Requestor / Permit Recipient


 Performs and directs the work of work party, shall notify start and completion of
the works and communicates with the Permit Issuer & Approver and Isolation
Authorities on the rig.
 Appointed Confine Space Entry Attendant/Safety Watch who is obliged to
observe and communicate with working inside of the confined space.
C.6 Confine Space Entry Attendant / Safety Watch
 Shall maintain adequate communication with the person(s) inside the confined
space and with the backup personnel.
 Shall immediately communicate any change in conditions to the workers in the
confined space.
 Shall monitor any life support systems that are used during the confined space
entry.
 Shall not engage in any other activity that may distract attention away from the
workers in the confined space.
 The register/log of persons entering and leaving the confined space along with
their tools
 After Job completion, safety watch / confined space entry attendant must ensure
that all entrants along with their tools has outside the confined space, as per

 He has an obligation to STOP WORK IF CONDITIONS CHANGE TO

C.7 Electrical / Process / Mechanical/Instrument Isolation Authority
 Isolation/off electric power supply, compressed air or pressurized fluid or any
other kind of energy in accordance with the Lockout/Tagout.
 Communicate the execution of the above-mentioned Activities to affected
person(s).
 After completion of the work inform the Permit Issuer & Recipient of Work, of the
intention to power up and make sure that it has been well understood.
C.8 Service Companies / Sub-contractors
 All persons on the rig, including employees of subcontractors (services) are
required for the proper application of this guidelines, the PTW is needed to
perform any work inside the confined space.
D Minimum requirements Confine Space Entry
D.1 Job Planning
 Written procedure for confine space entry shall be available and all work party and
permit authorities are trained in the content of the procedure.
 Written risk assessment (TRA/JSA) is carried out by performing authority and review
and approved by Permit Issuer & Area Authority.
 CSE job must be carried out under Drilling Contractor Permit to Work System,
therefore the job permit along with CSE certificate shall be issued prior to perform
work.
 Permit & associated CSE Certificate is valid for one shift and/or 12-hrs therefore
fresh permit and certificate will be issued.

 Pre-job safety meeting shall be carried out. The work be discussed by the Permit
Issuer/Permit Approver with the person(s) involved in the work.
D.2 Gas Testing:
 Gas testing (oxygen, LEL, CH4 & H2S) shall be carried out by trained & authorized
person prior to start of work. Continuous gas monitoring shall be carried to ensure
conditions are safe to continue work. Requirements for frequency of gas testing shall
be agreed and documented in risk assessment and same to be specified in the
permit.
 Gas tests required for hot work shall be carried out separately (the tests done for
entry are not valid for the Hot Work Permit).
 Written procedure for gas testing shall be available and know to Permit Authorities &
AGT. Procedure shall contain permissible limits for required parameters.
 The person carried out gas testing (Authorized Gas Tester - AGT) shall authorized
based on written test or training certificate/attendance form. List of AGT shall be
maintained by Drilling Contractor and shall be approved by WSL or his designee.
D.3 Isolation:
 The equipment shall be isolated, emptied, cleaned and gas freed as necessary.
 Isolation shall be carried out as per Drilling Contractor Energy Isolation Procedure,
however for CSE positive isolation shall be mandatory. In case of deviation from
positive isolation, isolation risk assessment shall be carried out to demonstrate
activity to ALARP level.
D.4 BA
 Entry without BA is only permissible if the oxygen, NORM and flammable gas levels
are the same as measured outside in fresh air, toxic contaminants do not exceed
current occupational exposure limits (see Table-1).

D.5 Confine Space Rescue Plan


 Confine Space Rescue Plan shall be prepared and approved for all Confined Space
Work.
 The main objective of the Rescue Plan is to ensure rescue of person in case of an
emergency inside CS. Size / capability of Rescue Team shall commensurate with the
risk level of the job in progress. CSE work MUST not progress in case of non-
availability of rescue team and equipment.
D.6 Communication:
 Clear communication shall be maintained by displaying Permit & certificate & where
necessary use of signboards. The copy of permit & certificate shall also be displayed
on permit board and permit office.
D.7 Ventilation
 Proper ventilation shall be ensure, where the natural ventilation is not effective forced
ventilation blowers or fans to be used.
D.8 Miscellaneous
 At least one of the persons involved in the operation should be located outside of the
confined space, at the place which allows him to monitor the situation and
communicate and assure workers inside.
 Persons carrying out the work must be equipped with required equipment according
to the nature of their work.
 Avoid long-term continuous operation and use a rest break system.
