Autobiography Assignment
6th Graders, Include at least eight of the following elements in your autobiography:

Each element is worth 3 points based on completeness of answer. An element that is merely mentioned will
receive only one point. One that adds additional information, but still leaves desired information out, will
receive two points. An element that is complete and responds to all of the mentioned aspects will receive
maximum credit of three points.

MUST DO: Name and Birth Date – Include the month, day, and year you were born, as well as the location
If known; also include how you got your name/who named you
If known; you may also include any interesting information regarding your birth
Example: My name is Michael Andrew Smith. I was born March 8, 1992, at Strong Memorial Hospital in
Rochester, New York. My mother named me after Michael Rodriquez, a doctor who saved my older sister,

Parents/Guardians – Their names/ any important or interesting information about them

 This may include where they work, interesting things they have done in their lives, their interests, etc.

Brothers/Sisters – Include their names, ages, interests, memorable experiences you have shared with them,
and some information about the relationship you have with them (for instance, do you get along well?) --

History of where you have lived – Use the Internet to research the history of the places you have lived.

Achievements/Awards – Anything you have done that is special. This may include, but is not limited to,
awards you have received. (Example: In 1998, when I was seven, I vividly remember being very proud of myself
for finally learning to swim. I remember going to a lake with my best friend’s family. We rowed in a small boat
