Autobiography Assignment
6th Graders, Include at least eight of the following elements in your autobiography:

Each element is worth 3 points based on completeness of answer. An element that is merely mentioned will
receive only one point. One that adds additional information, but still leaves desired information out, will
receive two points. An element that is complete and responds to all of the mentioned aspects will receive
maximum credit of three points.

MUST DO: Name and Birth Date – Include the month, day, and year you were born, as well as the location
If known; also include how you got your name/who named you
If known; you may also include any interesting information regarding your birth
Example: My name is Michael Andrew Smith. I was born March 8, 1992, at Strong Memorial Hospital in
Rochester, New York. My mother named me after Michael Rodriquez, a doctor who saved my older sister,
Samantha. She had cancer and he helped her…

Parents/Guardians – Their names/ any important or interesting information about them


 This may include where they work, interesting things they have done in their lives, their interests, etc.
(Example: I live with my mother, Pamela Smith. She is a personnel manager at Kroger Grocers. My dad, Doug
Jacobs, lives with his new wife, Janine, in Albany, New York. They both like to go rock climbing…)

Brothers/Sisters – Include their names, ages, interests, memorable experiences you have shared with them,
and some information about the relationship you have with them (for instance, do you get along well?) --
(Example: My sixteen-year-old brother, Joe, and I do not get along. He always tries to get me in trouble. One
time he ate an entire pan of brownies that my mom baked for church. Then he took the dirty plate he used to
eat them and planted it in my room. My mom caught him, though. She knew I had been at volleyball practice
since school got out and Joe was the only one who could have possibly eaten the brownies…)

Extended Family – (grandparents, cousins, aunts, uncles, nieces, nephews, in-laws) Include, names, ages, and
relationships to you. (Example: The newest member of my family is my niece. Her name is Stephanie and she is
three months old. She is the daughter of my brother and his wife, Kara. I was at the hospital when she was
born and I held her on her first day in the world…)

Pets – Include what kind of animal they are, their age (if known), their name (along with who named and them
and why they gave them their particular name) when you got them, and some experiences you have had with
them. (Example: I’ve had my boa constrictor, Jake, since I was four. My dad named him after the wrestler, Jake
the Snake. My dad and I love him, but my sister is afraid of Jake and of the mice we feed him…)

Hobbies/Interests/Talents – Virtually anything such as playing video games, collecting comic books, riding
horses, performing magic tricks etc. (Example: One of my favorite things to do is paint. My older sister is an art
student in college and she started helping me learn about painting when I was a fourth grader. Ever since…)

Favorites – Discuss your favorite music, movies, T.V. shows, sports, holidays, foods, etc. Explain why your
favorites are your favorites. (Example: My favorite T.V. show is Everybody Loves Raymond. I think it is really

foolish in front of Deborah, his wife. No matter how hard he tries, he always messes things up…)
Clubs/Activities – Describe any organizations, in or out of school, to which you have belonged such as
volleyball, Student Council, 4-H, Church (Example: I have been in 4-H for six years. Each year I enter cattle. Last
year, I won reserve grand champion. I also won ribbons for scrapbooking the past two years.)

Friends – Tell about your best friends.


How you met, how long you’ve know each other, what you like to do together, why you are such good
friends, etc. (Example: I met my best friend, Dustin, when we were just third graders. We both had Mrs.
Johnson. She made us partners one day. We found out we lived by each other and started hanging out that
day. Ever since, we’ve been inseparable. We like to…)

History of where you have lived – Use the Internet to research the history of the places you have lived.
Mention a few interesting things that have happened. (Example: I live on Lake Manitou, not far from where
the Colonial Hotel once stood many years ago.)

Travel – Describe any interesting places you have lived or visited.


If you have never been anywhere, you may discuss where you would like to go (Example: Although I’ve
never left Indiana, I really want to go to Colorado to see the Rocky Mountains. I’ve been interested in them
ever since…)

Life changing or unforgettable experiences – this could be virtually any experience that has changed the way
you look at the world (Example: My life changed forever on May 10, 1994 when my little sister, Katie, was born
at Woodlawn Hospital. I was no longer the baby of the family. Now my parents expected me to…)

Achievements/Awards – Anything you have done that is special. This may include, but is not limited to,
awards you have received. (Example: In 1998, when I was seven, I vividly remember being very proud of myself
for finally learning to swim. I remember going to a lake with my best friend’s family. We rowed in a small boat
to the middle of the lake and his dad threw me over…)

School Experiences – Describe the best, worst, or most memorable school experiences (Example: One of my
favorite memories of school is that every other Friday in fourth grade, we played dodge ball for thirty minutes
in the gym.)

Role Models – Describe who you look up to and why you look up to them (Example: My role model is my
grandma. She is very wise. She seems to know about everything. She is also the kindest person I know. This one
time she….)

Goals – Please list both short-term (Example: This year, I want to be the starting forward for the basketball
team.) and long-term goals (Example: I really want to go to Purdue to be a veterinarian. I love animals. I also
know that this career pays well.)
