Background:
Airbus received a report on an in-service event on A319 aircraft about the loss of the right hand windshield in flight, during
the cruise phase (OIT 999.0039/18).
The windshield was the original from delivery, manufactured by the supplier Saint-Gobain Sully (SGS).
The investigation was led by the CAAC which has issued the final investigation report reference SWCAAC-SIR-2018-1.
Note: no English version of this report has been issued by the CAAC.
Description:
The investigation of the A319 event determined that both structural plies of the RH windshield were damaged, due to a
continuous arcing in the windshield heating system (at the level of the power supply wires). Such electrical arcing requires a
combination of at least the following contributing factors: mechanical damage to the insulation sleeve of the power supply
wires and water ingress resulting in the wires corrosion. Several tens of seconds after the failure of the 2 structural plies, the
last ply failed (outer ply, non-structural) and the windshield was ejected due to the pressurisation loads.
Solution / Recommendation:
MITIGATION:
Following in-flight windshield loss event, the FCOM & QRH have been updated in order to improve procedure in case of
cockpit window crack during flight. This procedure now includes shoulder harness on and if inner ply is found crack, oxygen
mask on.
The procedure “COCKPIT WINDSHIELD / WINDOW CRACKED,” has been updated in December 2018 (refer to FOT
999.0100/18).
During investigation, a sampling inspection was performed on A320 and A330/A340 families in-service SGS windshields in
order to collect in-service data and electrical measurements on the windshield system.
These data, in addition to SGS collected information from returned in-service windshields, have been used in order to build
the requirements for a fleet-wide in-service inspection.
Inspection Service Bulletins (ISBs) and associated SGS Vendor Service Bulletins (VSB) will be dispatched to inspect
SGS windshields on A300/A310, A320 and A330/A340 aircraft families.
*A300/A310 and A330/A340 families SB dispatch dates will be updated when known. References of those SBs are not
yet allocated.
Potential AD will be related to those SBs.

The compliance time will depend on windshield P/N, windshield temperature sensor fault history, windshield age/utilization
(cycles/hours) and will be detailed in the ISBs.
The electrical measurements will have to be reported on a chart that will define a coloured area.
The repetitive inspection interval will depend on this electrical measurements area.

The SGS VSBs will provide instructions to open / close the terminal block, to prepare the electrical connection on the “Z bar”
external part of the windshield, and restore the weather seal once the test is performed.
The VSBs will also include the procedure to inspect the windshield in maintenance shop, if the windshield has been removed
from A/C and stored as spare in operators / MROs facilities.
In case of NIL finding, the windshield can continue to be operated with repetitive inspection until replacement by a
windshield with P/N that is not affected by the inspection.
In case of finding during inspection, and depending on the finding nature (during visual inspection, electrical tests…), the
windshield may continue to be operated with repetitive inspection, or will have to be replaced before next flight, or the
aircraft will have to be dispatched under MMEL.
All the finding cases will be detailed in the ISBs.
The average finding rate expected is 15%.
Dispatching the aircraft under MMEL will imply the windshield anti-ice system deactivation and flying with no icing
condition.
On A320 family, an additional MMEL item (30-42-03B) has been included for the purpose of SB 56-1022 and 56-1023. It
will allow aircraft dispatch in case of finding with the active windshield de-icing system and the maximum cruise level
FL230. This new item was approved by EASA and is under approval by FAA, which has received the MMEL project. Similar
MMEL projects are not forecasted for the A300/A310 and A330/A340 families.

The estimated elapsed time for the inspection is 1.5H per windshield.
MODIFICATION:
SGS has developed a new windshield design. The design was improved in the moisture ingress protection and the damage
protection to the electrical sleeves/braids.
All the GKN and PPG windshields P/N are also not part of the inspection.
The estimated elapsed time for windshield replacement is 5H per windshield (10MH).
The following linefit modifications will introduce the new SGS windshield P/N:
As windshield is a SFE (Seller Furnished Equipment), concerned operators by the above linefit modifications will receive
dedicated RFC (Request For Change) updates.
Although SB/VSB advance copies cannot be dispatched prior to the SB/VSB issuance dates, operators can from now on
follow the below recommendations to better anticipate the preparation of the inspection:
For A300/A310 and A330/A340 operators, webinars will be scheduled at a later stage.
