COMPLAINT AFFIDAVIT
I, KATHLEEN KAYE P. EUGENIO , of legal age, single, Filipino and resident of
No. 8-G Manhattan Parkway. Gen. Malvar St., Bgy. Socorro, Quezon City, after having
been sworn to in accordance with law depose that:

1. I am filing formal complaints/criminal charges against Grace Kelly Tansi  (herein

referred also as the respondent) a resident of No. 4256 Lotus St., Sunvalley Subd.
Paranaque City, Philippines or 24-E Quirino St., TS Cruz Subd. Bgy. San Agustin,
Novaliches, Quezon City and 83 3rd St. Fortune 5 Village Paseo de Blas, Valenzuela
City;

RECOMMENDED CRIME FOR PROSECUTION

2. Violation of BP 22 and other appropriate charges under the circumstances, which

were committed as follows:

VENUE AND JURISDICTION

3. The crime was committed in Quezon City.

ULTIMATE FACTS COMMON TO ALL THE CHARGES

1. I first met the respondent in an online sales transaction when the

respondent manifested her intention of buying two (2) Louis Vuitton bags from me, by
chatting “MINE” the said bags I am selling online.

2. We have come to an agreement that the respondent will buy the two (2) bags totally
amounting to Sixty Eight Thousand Pesos (Php 68,000.00).
The manner of payment we agreed upon is that the respondent will pay Sixty Eight
Thousand Pesos (Php 68,000.00) through a dated check.
  
3. The respondent issued a BDO check with Number 0000864204 dated July 9, 2021
in the amount of Sixty Eight Thousand Pesos (Php 68,000.00). However upon
presentment, the said check was dishonoured and returned for the reason “Account
Closed”. A photocopy of the said check is hereto attached as Annex “A” and made an
integral part hereof;

4. Through a lawyer, I have sent the respondent a Demand letter through a private
courier three (3) times at her three (3) known address written above  all dated
September 4, 2021 , however, said letters were returned for reasons “RTO” (returned to
origin) for reasons that addressee is not known on the given addresses, copies of the
said transaction receipts is hereto attached as Annex “B” , “C” and “D” and made
integral part hereof.

5. The very copy of the demand letter with marked RTO are likewise attached hereto as
Annex “E”, “F” and “G” all made integral part hereof;
 
 
6. I have also made other demands through online chatting but to no avail and her
account was already non-existent;

7. Because of the foregoing, I have no choice but to proceed and file this case against
the respondent.

8. This statement is based on our present personal profile and from actual occurrence
of the events, specifically true and correct to the best of my knowledge and ability.

9. I say no more.
