RESEARCH SUBJECT INFORMED CONSENT

To the Participants:

Please read the consent form and ask questions and clarifications before
you decide to participate in this study or not. You are free to ask questions
during your participation in this research.

Research Title: COPING UP for PANDEMIC 2020 - 2021

1. Purpose of the Research study

The entirety of this study is to determine the profile of the head of

household of Grdade 11 – Narra students and the perception of the head of
household to the blended system of education this pandemic 2020 - 2021.

1. What is the profile of the head of household, in terms of:

1.1 age;
1.2 sex
1.3 civil status;
1.4 educational attainment;
1.5 household size
2. What are the issues/constraints encountered by the head of
household from his child/students during this blended learning system
of education.
3. What are the solutions/measures made to address the
issues/constraints encountered by the head of household from his
child/students during this blended learning system of education.
4. What is the perception of the head of household in the implementation
of the blended learning system of education during pandemic 2020 -
2021.
2. Procedure/s

You will be one of the fifty (50) head of households (respondents) to

answer a structured, pre-validated, English based survey questionnaires for the
data gathering instrument.

The survey questionnaire was divided into four parts:

Part I determines the profile of the head of household (respondents)

which includes , sex, civil status, educational attainment, and household size.

Part II covers the issues/constraints encountered by the head of

household from his child/students during this blended learning system of
education.

Part III covers the solutions/measures made to address the

issues/constraints encountered by the head of household from his child/students
during this blended learning system of education.

Part IV covers the head of household’s (respondents) perception

assessment towards the implementation of blended learning system of education
this pandemic 2020 - 2021.

3. Possible Risk/s or Discomfort/s

No possible risks can pose danger to you in any form during the conduct
of this study. The implementation of the method may last for three (3) months
and the interview may last for one to two hours.

4. Possible Benefits

The result of the study will benefit the teachers and the academe in
intensifying their teaching practices to effectively deal with the changes of the
educational system this pandemic 2020 – 2021.

For the Future Researcher, the result of the study may serve as a point of
reference for similar studies.

For the head of household (respondents) and child/students, the result of

the study will serve as information for them to be more involved in participation
to the activities in the implementation of blended learning system this pandemic
2020 - 2021.

5. Financial Considerations

There will be no compensation for participation in this study and no cost will
be required from you during your participation in this study.
 6. Confidentiality

Your identity in this study will be treated with utmost confidentiality. The
results will be published for academic purposes only.

7. Termination of Research Study

You are free to choose whether or not to participate on this study. There
will be no penalty if you choose not to participate. You may withdraw from your
participation at any time without penalty.

8. Available Sources of Information

Any further questions you have about this study will be answered by the
researcher.

9. Authorization

I have read and understood this consent form, and I volunteer to

participate in this research study. I understand that I will receive a copy of this
from. I voluntarily choose to participate, but I understand that my consent does
not take away any legal rights in the case of negligence or other legal fault by
anyone who is involved in this study. I further understand that nothing in this
consent form is intended to replace any applicable laws.
