The passenger ship Doña Paz collided with the oil tanker Vector in 1987, causing a fire that killed an estimated 4,000 people in the world's greatest peacetime maritime accident. Both vessels failed to maintain a proper lookout, and the overcrowded Doña Paz lacked sufficient life jackets.
A fire at the NCCC Mall in Davao City in 2017 killed 38 people after starting from a short circuit near the ceiling in an area selling fabric and furnishings. The building's sprinkler system on the third floor where the fire started was not working and had valves manually switched off.

The passenger ship Doña Paz was sailing from Tacloban, Leyte, to Manila. MT Vector, an oil tanker, was transporting 1,050,000 gallons of gasoline and other petroleum products from Bataan to Masbate at the same time. The two yachts crashed shortly before midnight on December 20, 1987, only five days before Christmas, in the Tablas Strait near Marinduque. This sparked a fire that swiftly spread throughout the ship. Survivors would subsequently describe that, in addition to the ship, the water looked to be on fire as a consequence of the ship's gas and petroleum leaks. In what is considered the world's greatest peacetime maritime accident, an estimated 4,000 people died. The absence of life jackets and the passenger vessel's overcapacity—at least two thousand people were not recorded on the manifest—added to the disaster. The collision of the two yachts could have been prevented if both yacht keeps a constant lookout. Avoid sailing or motoring in ship channels, especially if visibility is poor because of fog, rain or darkness. It’ll be very hard to put out a fire from something full of combustible materials.
NCCC Mall (December 23, 2017)
A fire consumed the 14-year-old NCCC Mall in Davao two days before Christmas, a season when most Pinoys go shopping at malls. A short circuit near the ceiling of the fabric and furnishings areas caused the fire. The fire burned for 32 hours, killing 38 people, the majority of whom were contact center personnel working on the fourth level. Authorities eventually discovered that the building's third story, where the fire originated, lacked a working sprinkler system. Sprinkler system control valves were also manually switched off at the time of the incident. A mandatory maintenance checking for sprinkler system should always be conducted time to time. A fire drill and a nearby fire exit will help the people inside especially those who are on the higher floors of the building
Kentex Slipper Manufacturing Fire (2015)
A business that made flipflops and rubber shoes caught fire on May 13, 2015, when welding sparks ignited chemicals that were incorrectly kept near the firm's entrance. The fire took five hours to entirely extinguish, and 74 people had died by that time. The majority of the bodies were discovered on the second level and were substantially burned, reduced to charred heads and bones. Survivors of the fire criticized the factory's terrible working conditions. Rex Gatchalian, the mayor of Valenzuela City, was charged with corruption and reckless imprudence, culminating in several homicide and physical harm accusations two years later. The indictment against Gatchalian, however, was dismissed by the Antigraft Court's Second Division. Safety precautions should be implemented, the cause of this fire is human negligence of work safety. Welding on a place with combustible chemicals and for sure chemical containers without lid that sets the fire up. Informing the workers regarding what’s happening during their work and if there are things that might cause problems to them. Like informing them that the place has combustible materials that can spark a fire the the tiniest form of fire.
