I, CRISTINA ESCONDE CALLO, of legal age, married, a Filipino citizen, and resident of Mercedez Benz
Street, Batal, Santiago City after having duly sworn according to law, do hereby depose and state that: 

1. My maiden name is CRISTINA ESCONDE CALLO;

2. However,I got married to CHRISTIAN MARK L. ALBERTO on May 10, 2012 at Bantug Science
City of Muños, Nueva Ecija;

3. Thus, my married name is now CRISTINA C. ALBERTO;

4. I state that CRISTINA ESCONDE CALLO and the CRISTINA C. ALBERTO is the name of one
and the same person and that is myself;

5. Wherefore, I am executing this affidavit to attest to the truth of the foregoing facts and for
whatever legal intents and purpose it may serve.
