CERTIFICATION

This is to certify that Fatima National High School, Fatima, Cortes, Bohol is ready to conduct
limited face-to-face to the students for the following reasons, to wit:

 The school is not utilized as Quarantine Facility;

 The Municipality is identified as low-risk area specifically the barangay where the school
is located considering that it has zero (0) positive case;
 The school has complied the minimum health requirements and protocols by procuring
the health kits and constructing health facilities as required by IATF;
 The teachers are capacitated to implement the limited face-to-face following the
minimum standard of health protocols required by DOH and IATF;
 The parents and community are supportive in the conduct of limited face-to-face; and
 The school selected Senior High School (SHS) students considering that there are
competencies that really need to be done in face-to-face and that the enrollment is
manageable with only fifty-five (55) Grade 12 and seventy-three (73) Grade 11 students.

This certification and justification is issued to support the request in conducting limited face-to-
face.
