1. PURPOSE
The purpose of this procedure is to define the steps that are followed in accident/ incident
investigation.
2. SCOPE
This procedure applies to any personnel involved in or responsible for the safety of workers.
3. RESPONSIBILITY
The SHEQ Officer and Supervisors shall be responsible for training of persons involved and
ensure that they understand and implement this procedure.

4. DEFINITIONS
Serious injury is any injury at work resulting in:
I. Any temporary disability.
II. Any permanent disability.
III. Loss of consciousness.
IV. Any eye injury resulting in an imbedded foreign body.
V. Any injury resulting in more than one day’s absence from work excluding day of injury.
VI. Any injury requiring a doctor’s attention
VII. Any injury resulting in a fatality.
MINOR INJURY
Any work related accident with no loss of time but resulting in any cut, strain, burn, bruise,
inhalation irritation or abrasion treated by a nurse or first aider
A NEAR MISS
An incident is anything that has a potential to cause harm or injury or damage to property.
A PROPERTY DAMAGE INCIDENT
An incident which involves no injury but in damage to company property

 All injuries no matter how minor must be reported to the immediate Supervisor on the
shift of its occurrence.
 Failure to do so may result in repeated commission of the unsafe condition, loss of
compensation, or disciplinary action.
 It is the duty of the front line Supervisor to report to the head of section and the SHEQ
Officer the circumstances of the injury before the end of the shift.
 It is the responsibility of the First Aider to attend to the injured employee immediately
after the injury.
 Following the injury the Supervisor shall:
 Complete a clinic note, specify WWC
 Collect from the injured employee, if possible and other eye witnesses, information
regarding the occurrence of the injury.
 Make arrangements in consultation with the First Aider to transport the injured to the
clinic
 The clinic Nurse on duty will fill a Certificate of Injury if the injured person is a referral
case to the hospital.
 The Supervisor will then complete preliminary investigation report form, “The Internal
Incident/ Accident Form! Pass it over to the Foreman, Section Manager, Head of
Department and finally to the SHEQ Officer. Each one filling in comments on their
sections on the report form.
 In case where injury results in loss of time or the injured has been sent home, Supervisor/
Foreman shall inform the most senior person of the department concerned.
 The investigation team shall conduct investigation and publish the preliminary report
within 24 hours from the time of reporting the accident or incident.
 In all cases, implementation of recommendation shall be short term to immediate long
term with clearly set dates and names of people accountable.
 Where appropriate review on a monthly basis shall be done to monitor progress.

The respective Head(s) of departments shall ensure that all recommendations in the accident
investigation report are implemented with the agreed time frame as specified in the
investigations schedule.
NEAR MISS
All near misses must be reported to the Supervisor and the SHEQ Officer and recorded with full
detail of cause and probable consequence.
All near misses, deemed as dangerous must be investigated as required by this procedure.
PROPERTY DAMAGE.
 The respective H.O.D shall ensure that Property Damage accidents or incidents are fully
investigated to determine
 The true facts and correct sequence of events.
 The main cause of the damage to property.
 Any underlying cause not immediately apparent
 Other contributory factors.
 The cost damage based on repair or replacement
 Disciplinary Action
FATALITIES
 All fatal accidents shall be reported to the Manager responsible for Safety who shall
immediately without delay inform the (NSSA) before any paper work is done.
 Immediately thereafter, all operations in the affected area must be stopped until
investigations are completed, and a written/verbal permission by NSSA have been
granted.
 The Human Resources Department shall inform the next of kin, and make necessary
Funeral arrangement and then:
 Counsel the affected parties.

 THE Divisional Responsible person/ SHEQ Officer shall report minor, disabling and fatal
accidents to NSSA.
 Minor accidents within 14 days, disabling injuries within 24 hours and fatal immediately
