RELEASE WAIVER AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

I, GARABILEZ, EMIL . of legal age, Filipino and with postal address at ______________PATPATA
1st,__________________________________________ after having been sworn in accordance with law,
do hereby depose and state that:

I received from ASIAPRO MULTIPURPOSE COOPERATIVE the total amount of Fourteen Thousand Six &
75/100 (Php14,006.75) Philippine Currency, as complete settlement of any and all my claims due from
ASIAPRO MULTIPURPOSE COOPERATIVE.

In consideration of the above:

1. I release, remise and forever discharge ASIAPRO MULTIPURPOSE COOPERATIVE its associates,
successors-in-interest, stockholders, directors or officers from any action, sum of money, damages, ever
had, now have, or which I, my heirs, successors, and assigns hereinafter may have upon or by reason of
any matter, cause or thing whatsoever, up to the time of these presents, the intention hereof being
completely and absolutely to the release of ASIAPRO MULTIPURPOSE COOPERATIVE, its associates,
successors-in-interest, stockholders, directors or officers from all liabilities arising wholly, partially or
directly from my membership/employment and/or assignment therewith.

2. I further warrant that I will not institute any action nor make any claim against ASIAPRO
MULTIPURPOSE COOPERATIVE, by reason of my membership/employment with ASIAPRO
MULTIPURPOSE COOPERATIVE. I also manifest that this document shall not be taken by me, my heirs or
assigns as a confession and/or admission of liability on the part of ASIAPRO MULTIPURPOSE
COOPERATIVE its successors-in-interest, officers, directors and owner-members for any matter, cause,
demand or damage that I may have against any or all of them.

3. I finally declare that I have read and understood this document and the release, waiver and
quitclaim hereby given are made willingly and voluntarily and with full knowledge of my rights under the
law.
