1. Reference: Regional Order Number 493-2018 dated May 23 ,2018.

2. In view of the above reference, the undersigned would like to request that the
Medical Team in the aforementioned Regional Order be removed as committee in
the Conduct of agility test because our role is prior to the conduct of the said activity
and Medical standby during the activity.

3. Furthermore, we should be included in the Regional order Number 494-2018 which

includes the composition of Standby Medical Services Team during the Conduct
of Physical Agility test.
