1.0 PURPOSE
This Health, Safety & Environment Plan has been prepared to provide a framework for CIPL
personnel to plan and develop a program, to achieve the objective contained in the project Health,
Safety and Environment Policy.
Our HSE Program sets our requirements for the protection of life and health. It also provides
guidelines to eliminate, where possible damage of and to equipment while carrying out specific
work task.
We have over the years established ourselves as a HSE oriented EPC Contractor by
implementing the high standards during the course of any work and have ensured that only
qualified and experienced personnel are employed to successfully execute HSE program of the
project.
2.0 SCOPE
The Health, Safety and Environmental Plan reflect the high priority that CIP Senior
Management upon the HSE for this project “Mehsana Bathinda Pipeline Project Phase II, (Section VI)”
It also demonstrates their commitment to ensure that all reasonably practicable measures are
taken during execution of this project:
 Protect the Health & Safety of all persons engaged at work site.
 Comply with the relevant statutory and Contractual Health and Safety requirements.
 Ensure the Health; Safety & Environment is not affected by the work.
 Provide and maintain plant, places and systems of work that are safe and without risk to
health and the environment.
 Provide all personnel with adequate information, instruction, training and supervision.
 Effective control, Co-ordinate and monitor the activities of all personnel on the Project,
including contractors, in respect of Health, Safety & Environment.
 Ensure that all construction – planning takes into account the Health, Safety& Environment of
all persons that may be affected by the work.
 Inform all relevant persons with the detail of HSE expectation and Risk assessments that may
affect their Health and Safety.

4.0 POLICIES
CIPL has developed IMS -Integrated Management System Policy (ISO 9001:2008, ISO14001:2004
&OHSAS18001:2007) . These shall beimplemented by site management throughout the
construction activity.
The CIPL, CEO signed IMS policies shall be prominently displayed on site.

5.0 TERMS AND DEFINITIONS
5.1 DEFINITIONS
ACCIDENT
An unplanned, undesired event or condition that, results in harm to people or damage to plant /
equipment.
INCIDENT
Any undesired event or condition that results or could have resulted in harm to people damage
to property or the environment or loss.
AUDIT
A systematic and Independent examination of the Health and Safety management systems and
their outcomes to determine the effectiveness of the Health & Safety management system.
COMMUNICATION
It is the process of conveying information to other party or parties, so it can be understood
correctly and acted upon.
NEAR MISS
An unexpected, unwanted event not causing loss, injury or illness but which under slightly altered
conditions can lead to an accident.
FATAL
Death resulting from an accident.
MANHOURS WORKED
The total no. of employee hours worked by all employees working in the premises. It includes
managerial, supervisory, professional, technical, clerical and other workers including contractor
labors.
FIRST AID CASES
First aid cases are where the injured person is given medical treatment and discharge
immediately for reporting on duty, without counting any lost time.
LOST TIME INJURY (LTI)
Any work injury which renders the injured person unable to perform his regular job or an
alternative restricted work assignment on the next scheduled work day after the day on which
the injury occurred.
MEDICAL TREATMENT CASE (MTC)
A Medical Treatment Case is any work-related injury that involves neither Lost Workdays nor
Restricted Workdays but which requires treatment by, or under the specific order of, a physician
or could be considered as being in the province of a physician.
RESTRICTED DUTY CASES
Days of restricted work activity are those workdays on which, because of occupational
Injury or Occupational illness the employee was assigned to another jobs on a temporary basis,
worked at a permanent job at less than full time or worked as a permanently assigned job could
not perform all duties normally connected with it. The no. of lost work days should not include
the day of injury or onset of illness or any day on which the employee would not have worked

even through able to work. This is a recordable case of incident / accident where lost time is not
completed.
HEALTH & SAFETY REVIEW
A formal recorded management evaluation of the effectiveness of the Construction Health &
Safety management systems and identification of actions for continual improvement.
HEALTH & SAFETY TOUR
A highly visible HSE site visit / walkabout by senior management to gain feedback from the
workforce and demonstrate commitment to Health, Safety & Environment.
SUB-STANDARD ACT, PRACTICE OR CONDITIONS
Deviation from standard act, practice or condition.
5.2 TYPE OF INCIDENT / ACCIDENT & THEIR REPORTING
All body Injury, damage to property or other loss that may arise out of or in consequence of the
execution and completion of works shall be reported to client and relevant govt. agencies.
The two categories of accidents, which are as follows: -
Non Reportable Cases
An accident, where the injured person are disabled for less than 24 hours.
Reportable Cases
In this case the injured person is disabled for 24 hours or more and is not able to perform his
duty.
Lost Work Days – DAYS AWAY FROM WORK
The total number of calendar days on which the injured person was temporarily unable to work
as a result of lost workday case or permanent partial disability.
Frequency Rate
Frequency rate is the number of Lost Time Injury (LTI) per Million Man hours worked.
Mathematically, the formula read as: -

7.0 ORGANISATION AND RESPONSIBILITIES
It is everyone’s responsibility to ensure that Contractor’s HSE policies and those of Company are
adhered to but the ultimate responsibility for the implementation of Safety policies, objectives
and the achievement of HSE targets, as described in this document, rests with the Project
Manager.
This part of the HSE plan describes the Company’s organizational structure for Health,
Safety & Environment on the Project and defines the responsibilities of a number of keyperson
having significant contributions to make in the successful implementation of this HSE Plan.
In order to have a structured order of responsibility, brief description of HSE responsibilities for
key individual is as follows:
Project Manager- HSE responsibilities shall include, but will not be limited to:
The Project Manager is responsible for the establishment of the Project HSE implementation
Plan. This plan shall be according to CIPL policies and Client HSE procedures with the assistance of
other Managers and the HSE Manager. Accident prevention procedures will be included in all

activities by the application of sound HSE planning. The Project Manager shall set the pace for the
project HSE program with the site HSE Manager.
Planning Manager- HSE responsibilities shall include, but will not be limited to:
 Shall report to Project Manager.
 In absence of Project Manager shall assume full responsibility for HSE activities on the Project
Overall management and execution of the Work.
 Shall ensure that construction personnel have the relevant competency and experience to
execute the work safety.
 Shall ensure that rules and regulation have complied by subcontractor and terms and
conditions related to HSE are incorporated in work order.
 Shall ensure work procedures contain HSE requirements and hazards are identified, assessed,
properly managed and are understood by supervisors / workers.
 Shall visit the work site on routine basis to ensure a high level HSE awareness among the
workers is maintained.
 Shall attend weekly HSE meeting and ensures HSE concerns are addressed.
 Shall encourage and support his supervisors to execute safe working practices.
 Shall manage all subcontractors to ensure that overall HSE compliance is achieved.
 Shall ensure compliance with all statutory requirements applicable for the project.
HSE Officer / Engineer responsibilities shall include, but will not be limited to:
 Shall report to Project Manager.
 Shall act as a focal point with Client’s representative, local authorities and Sub-contractors on
HSE matters.
 Shall inspect emergency serviceand give report to concerned department / Project manager
for compliance of deficiency and for provision of trained and qualified personnel to maintain
the emergency services.
 Shall co-ordinate Client HSE inspection / walkabout Sessions.
 Shall co-ordinate with all departments, on HSE matters.
 Shall organize HSE committee meeting.
 Shall conduct initial HSE induction for all workers.
 Shall carry out daily tour of construction sites with supervisors to coordinate and maintain a
good housekeeping and safe practices.
 Shall train and assist supervisors to conduct toolbox meetings.
 Shall ensure that the specified precaution in work permits is complied with.
 Shall conduct HSE meeting on site with attendance by relevant supervisors and work leaders.
 Shall conduct and carry out incident / accident investigation with work supervisors and
submit the relevant reports.
 Shall co-ordinate with HRD for implementation of firefighting system and accident handling
system.
 Shall prepare the monthly HSE statistics and submit it to the Management & Client.

 Shall investigate all accidents and recommend the appropriate corrective measure.
 Shall inform all construction subcontractors about the project HSE requirements and
ascertain that subcontractor complies with the HSE requirement.
 Shall review risk assessment and emergency plan made by the execution team.
Construction Supervisor HSE responsibilities shall include, but will not be limited to:
Construction Supervisors are directly responsible for the control and activities of craft employees
on construction projects. They play key role in the execution and maintenance of an effective
jobsite HSE program.
The Site Manager and Planning Manager shall establish the standard for a good HSE program.
The actual performance of the HSE program is the prime responsibility of Construction Supervisor
who has direct contact with the execution team.
Construction Supervisors are responsible for wide range of activities. They must plan their HSE
activities with the same care and effort as they do other portions of their work program.
Sub-Contractor’s responsibilities shall include, but will not be limited to:
 Shall attend both client and company HSE induction briefings.
 Shall be fully equipped with required Personal Protective Equipment (PPE), while executing
work at site.
 Shall ensure that workers must be supervised and attended by a full time competent
supervisor approved by the construction manager.
 Shall obtain an approval for permit to work before carrying out work in the client’s premises.
 Shall take all necessary precautions to ensure that their activities would not affect other
fellow worker or, any third party at the work Site.
 Subcontractor shall take full responsibility for health, safety and environment of his employee
during all site operations.
 Subcontractor shall at his own expense arrange for accident prevention tools & assurances.
 For activities i.e. heavy earth moving equipment operations, LMV & HMV driving, electrical
work on high and low tension lines Sub Contractor shall employee skilled and competent
persons.
 Subcontractors shall arrange for medical checkup of his entire employee before employment
and maintain record.
 Subcontractor shall inform his employee about the hidden & visible hazards associated with
their jobs.
 Subcontractor shall nominate HSE coordinator out of the working staff & also nominate
members for HSE committees.
 Subcontractor shall ensure that PPE are available all time and shall be used by his employee
with no exception.
 Subcontractor shall arrange for necessary first aid equipment at site.
 Subcontractor shall ensure that lifting machine, tool and tackle wire ropes, slings, chains,
shackles, cranes, pulley blocks shall be of good condition and tested by competent person.
 Subcontractor shall be responsible to ensure implementation of all legal requirements for
workmen under the Local Laws and relevant International Standards.

Associates HSE responsibilities shall include, but will not be limited to:
 Shall report to Line Supervisor.
 Shall carry out their duties in a safe manner and take care for his Safety and that of other
fellow workers who may be affected by his acts.
 Shall work according to the instructions of his supervisors and always comply with the rules
and regulation of health, safety and environmental manual.
 Shall maintain tools and equipment issued to him in a safe operating condition and report
defect to his immediate supervisor without delay.
 Shall obtain necessary work permits from his supervisors and abide by his instruction.

9.0 HSE PROCEDURES FOR CONSTRUCTION ACTIVITIES
9.1 NEW EMPLOYEE INDUCTION
Each employee, upon his arrival on the project, shall be instructed on Company’s HSE
procedures and requirements. HSE orientation shall be conducted by site HSE
Manager/Supervisor or trained HR representative. Employees shall be required to attend this
HSE orientation before commencing any activities.
Basic HSE training will be provided to all personnel during the induction-training program. Job
specific HSE training will be provided to personnel where a risk assessment identifies hazards
that exist specifically to a task or operation. No person will be permitted to work unless they
have been informed of the possible hazards associated with their work and given instruction on
how to control such risks effectively.
On completing attending contractor’s in-house HSE induction, the person will sign an attendance
form to declare that he had understood the content and shall abide to follow and comply with
good safe work practices. They may only then be qualified to be issued with a personal ID card
for access to the work site.

Refresher Induction Course

9.2 RISK ASSESSMENT, METHOD STATEMENTS & SAFE SYSTEMS OF WORK.
HAZARD
Risk assessment is the systematic identification of the hazards associated with construction work
and the evaluation of the risks associated with those hazards. A hazard - risk assessment must
take account of all the work activities and consideration must also be given to anyone else who
could be affected by the work activities.
HAZARD Observation Areas
All Health, Safety & Environmental hazards that may be present during the Contractor’s
construction activities include:

Hazard identification shall be conducted prior to commencement of construction for the
reduction of potential major hazards by implementation of identified controls. The objective
being to ensure the risk associated with construction is as low as reasonably practicable.
Risk Assessment
Risk assessment process will identify certain activities for which identified controls are
documented with information of who, when, and how to ensure that the activities are properly
controlled and executed safely and without risk to health, safety and the environment.
All such written controls for execution of work will clearly identify the objective, the sequence of
operations, foreseeable hazards, precautionary and protective measures required and will be
easily understood by the personnel who are to supervise and carry out the work.
Hazard Control
For each identified hazard work supervisor must stipulate the systems to prevent the hazard and
minimize the consequences. These shall include:
 Maintenance of equipment
 Inspection of equipment
 Personnel protective equipment
 Fire and gas detection and firefighting equipment

Method Statement/Operation Control Procedure/Quality Control Procedure
For critical activities having high ranking of risks, method statement / QCP/OCP Procedure shall
be developed. For all other activities, risk assessment shall be performed, documented and shall
be made available at place of work.
Where sub-contractors are employed on the project, the contractor will ensure that such
companies provide adequate Environmental, Health and Safety information and training to their
employees. This will include an induction, information contained in risk assessments / method
statements and any specific training identified in such assessments / method statements.
Alternatively contractor shall organize the same.
Contractors will provide similar assessments covering their respective operations and all risk
assessments will be reviewed and revised as necessary should circumstances change.
A suitable and adequate risk assessment shall be carried out for all work and these assessments
should be regularly reviewed. Records should be kept and be available when requested.
Pre Task Instructions
Before start of the job work supervisor shall organize pre work briefing to all working crew and
shall inform hazards & inherent risk involved in work and precautionary measures related to it.
Prior to commencing each day’s work, supervisors/foreman shall hold a short pre work task
instruction meeting with their respective crews. This task instruction meeting shall highlight
important HSE requirements that are required to carry out their assigned tasks.
Tool Box Meeting
Each work supervisor shall conduct daily toolbox meetings to discuss health and safety issues
before commencement of any activity. Work Supervisors shall lead these meetings. This meeting
shall be no longer than 5-10 minutes of duration and items covered at these meetings shall
include:
 Hazards and its control applicable to the particular activity.
 Safety measures / controls to be adopted during executing the job
Any important information / happening related to particular activity.
Job Specific Toolbox Meeting
Before starting new activities or activities which are more hazardous or dangerous and require
special safety precautions, Job specific Toolbox meeting shall be organized by work supervisor
along with the assistance of area HSE officer to discuss safe methods of work. The subject
matters to be discussed with working crew must highlight specifically the hazards anticipated
during execution of the job.

9.3 ACCIDENT NOTIFICATION AND INVESTIGATION
All incidents and accidents, near misses and dangerous occurrence shall be notified
immediately through telephone and notification shall be submitted to client within 24hrs. CIPL
shall undertake detailed investigations into all accidents and incidents including near misses to
establish causes and prevent recurrence. A member of the site HSE team shall carry out such
investigations that shall identify the immediate and underlying causes. An investigation report
shall be prepared detailing remedial actions with close out dates and same shall be distributed
to the Client within 72hrs.
All the events of personal injury to any person or in the event of any loss of or, damage to the
works, materials supplied or Equipment supplied by any company accidents shall be recorded
in the Accident / Incident Report form. All incidents shall be reported to HSE department /
Project Manager immediately by telephone / radio / fax / in person. All incident and accidents
shall be recorded and reported in accordance with Company’s norms. Incidents and Accidents
shall include any occurrence, which caused injury (even if no lost time involved), harm to the
environment, and damage to equipment or loss of production. All incident and accident shall
he notified to company immediately (within 24 hours). The investigation team shall be formed
and the full report of the incident/accident shall be forwarded to Company.
All accidents/incidents shall be thoroughly investigated to establish primary and contributory
causes as specified accident format. Any evidence must remain in place and be left undisturbed
until company gives approval. All relevant attachment including sketch shall be made by
supervisor of concerned department and attached with the incident report. An incident register
shall be maintained and forwarded to company.
All incidents shall be investigated by HSE department / jointly by involving line management to
establish causes and prevent recurrence.
a) The objective of Accident or Incident investigation report is to prevent recurrence of accident
in future.
b) Notwithstanding the above mentioned, the following action are to be carried out by the
company :-
 In case of any major or fatal accident notification shall be forwarded immediately to
HSE department / Project Manager/Medical services/HRD.
 The work site shall be secured in the undisturbed state till it is made safe.
 Any evidence of witness shall be secured and preliminary investigation report of
accident shall be initiated and forwarded as soon as possible to head of HSE,
Project Manager and Medical services within 24 hours.
The investigation shall aim at:
 The events that lead to the incident.
 The fact of the incident itself.
 The relevant fact of what occurred immediately after the incident.
 Identifying the required action to be taken in order to prevent similar recurrence.
The causes of the accident either unsafe act or unsafe conditions or could also due to direct or
contributory factors along with first aid or medical treatment, disability etc. must be specified in
report.

9.4 MONTHLY HSE REPORT AND STATISTICS
The HSE statistics shall be tabulated and forwarded to company and to the HO on monthly
basis. Details of the following classification of statistics related to injuries and near miss
incidents shall be tabulated. These statistics shall also be brought to the notice of the Project
Manager for his review and assessment against the HSE targets
Standard format, consisting of the following classified headings shall be tabulated:
1. Man-hours exposure
2. No. of Lost Time Injuries (LTI )
3. No. of Man days lost
4. LTI Frequency
5. LTI Severity
6. No. first aid cases
7. No of vehicle incidents
8. No. of fire incidents
9. No of near miss incidents

9.5 PERMIT TO WORK SYSTEM (PTW)
Certain work connected with Construction shall be carried out under a Work Permit System
where the risk ranking is high.
 The most formal method of ensuring safe working practices - safe systems of work - is to have a
permit - to - work procedure. A permit - to - work system shall ensure that a trained authorized
person shall pre-assess the hazardous circumstances involved and then prescribe the conditions
and limits for the work to take place.
 The permit certificate requires acknowledgement by the performing authority (team leader)
who is responsible for doing the work to indicate that working conditions and limits are
thoroughly understood.
Work Permit Requirement
Work permit systems are the most formal method of ensuring safe systems of working. Their use
shall be reserved for work where there are potential hazards and that the precautions necessary for
that work need positive enforcement.
Definitions
Issuing Authority
Is the person who signs a Work Permit and authorizes the work to start, provided that all the
prescribed special conditions have been or shall be complied. Normally the Section in-charge /
Area in-charge having the accountability issues the work permit. He shall ensure that all
supporting documentation has been obtained and is properly completed before the Work Permit
is signed by him.
Performing Authority
Is the person who 'receives' the Work Permit? Normally a supervisor of the operatives carries out
the work. He shall ensure that he, and all the operatives involved, understands the conditions,

limitations, and precautions necessary as stipulated in the Work Permit, and that these are complied
with.
Description and Limitations of Permits
There are different Work Permit Forms to cover Construction Works. These are:
 The Hot Work at Height Permit
 The Height Work Permit
 Excavation Permit
 The Electrical Isolation Work Permit
 Radiography work
 Working in confined space
 Heavy Lifting Operation
Procedures for Work Permit Issue
a) Requirement
Work Permits are required for all work carried out within the Restricted Area or as per above
condition having relatively high ranking risks.
b) Preparation and Issue
The Issuing Authority shall prepare Work Permits. All sections of the Work Permit Form must be
completed.
When the Issuing Authority is satisfied that all conditions and precautions have been complied with,
he may sign and issue the permit original to the applicant. The Recipient shall retain / or display the
Permit at work location.
c) Posting, Suspension, Completion and Cancellation
All Permits are automatically suspended upon emergency alarms sounding. Before work
re-commences after an alarm stoppage, permission must be obtained from the Issuing Authority.
When all work is finished, the person in charge of the work, normally the recipient of the Work
Permit shall sign the 'Work completed' section of the Form and return it. When the Issuing Authority
has received the completed Permit it shall automatically be cancelled
d) Validity
A Work Permit is only valid until the time / date shown. Normally a Work Permit shall be issued
for a period not exceeding 12 hours. Each Permit may be extended on a 12-hour basis. The
permits shall be presented for re-validation each day before work commences. A Work Permit
ceases to be valid upon the sounding of an emergency alarm.
e) Verification
Contractors HSE representative shall verify the conditions of permit are being complied sincerely.
In case of any observations, it shall be bought to the notice of section in-charge for correction and
proceed for work.
9.6 HSE INSPECTIONS & AUDITS
HSE INSPECTIONS
Regular planned inspections shall be undertaken. Supervisors shall continuously monitor Safety on
the site as an integral part of their line responsibility. Any substandard practices and conditions
noted during the inspection shall be recorded on the inspection report appended to the plan.

As part of the project’s environmental, health and safety culture planned environmental, health
and safety tours shall be undertaken by senior management on a routine basis. These Health and
Safety Tours are designed to be a highly visible walkabout by site management with the aim of
demonstrating management commitment to its environmental, health and safety policy.
During these tours, senior management shall encourage feedback from the workforce and
identify the actions necessary to improve the project’s health and safety performance. Any
substandard practices and conditions noted during the tour shall be reported on the "HSE walk
about report”.

HSE AUDITS
CIPL shall submit an Audit Plan to EIL/Owner indicating the type of audits and covering following
as minimum:
- Internal HSE audits regularly at least on quarterly basis by engaging internal qualified and
experienced auditors
- External HSE audits shall be regularly once in a year by engaging qualified external and
experienced
9.7 TRAINING AND COMPETENCE
Training Matrix & Plan
To achieve our HSE objectives and Targets, HSE training shall be planned and regularly organized
for all personnel working on the Project as per the training matrix. All managerial, supervisory
staff and workmen shall attend HSE Training Courses, organized time to time.
Training programs listed below shall be organized to make the people aware of the hazards &
dangers at work site but not limited to the followings:
 Working At Height
 Demolition
 Safe storage of fuels, oils and lubricant.
 Prevention of oil spillage. Provision of dip trays below machinery to collect oil and grease.
Disposal of used oil and lubricants.
 Waste Management – Collection, storage & disposal

 Use of PPE’s
 Housekeeping, Hygiene and sanitation.

Training registers will be maintained for all training courses delivered.
Specific HSE Training
Separate training efforts shall be held throughout the construction period. This training shall be
based on the result of HSE inspections, audits, hazardous working conditions, daily HSE
observations, etc.
The site HSE Manager and HR Manager shall be responsible for organizing these training sessions.
Specific training shall also be conducted for specific activities such as:
 Fall Protection
 Hazardous materials Communication
 Electrical safety
 Power Actuated tools
 Firefighting equipment
 Confined space Entry (if required )
 Work permit procedure
Such training shall be conducted by the site HSE In-charge and shall be carried out prior to the use
or implementation of an activity which requires same.
First Aid Training
Training on first aid shall be organized for supervisor and workers by qualified doctor /
competent authority. The names of the trained first aiders shall be displayed at all prominent
work locations.
EMERGENCY RESPONSE TRAINING
Drills and exercises
Since the work is in city area, emergency drill shall be conducted only if practicable. Emergency
drills and exercises shall be carried out to test preparedness of emergency arrangements.
Emergency evacuation drill shall be organized on likely incidental scenario. Emergency evacuation
drill register shall be maintained containing information for type of drill, location of drill,
participants and observations.
 Shifting of injured personnel from work location to First Aid Center / hospital.
 Fire drills
 Traffic Jam
 Other emergency situations i.e. Road Accident etc.
Training shall be organized time to time internally and by the aid of external agencies. For
external agency a request shall be made to HR department of IECC Limited for organizing the
training.

9.8 HSE MEETINGS
HSE Committee Meetings
The Monthly HSE Committee Meeting shall be chaired by the Project Manager. Sub- contractor
HSE representatives and Construction supervisor shall be scheduled to attend the meetings.
Invitations to sit on the committee shall be extended to Client representatives.
The responsibilities of this Committee shall include the following:
 Overall objective analysis of the site HSE activities.
 Good Safety practices being adopted/ to be adopted.
 Deficiencies found during safety inspection / audits and measures to be takento improve
the same.
 Incidents/accidents or near misses that have occurred and review of Investigation and
recommendation.
 Hazard observed and corrective action.
 Identification of Unsafe act.
The HSE meeting shall be recorded and copy of minutes of meeting shall be forwarded to all
concerned for action and implementation. In this meeting the suggestions of the attending
members shall be reviewed on their past experience and shall be recorded for action and
implementation.
The HSE committee meeting members shall comprise of the following personnel.
 Project Manager - Chairman
 HSE Manager (In-charge) - Secretary
 Sr. Managers / Engineers heading different areas / sections - Committee member
 Work supervisor (one from each discipline) - Committee member
 Site HSE Engineers/ Officers - Committee member
 In-charge Plant & Equipment - Committee member
 In-charge Site Electrics - Committee member
 In-charge Stores - Committee member
 Work crew representatives - Committee member
 Representative of Subcontractor - Committee member
 Labor contractors representative - Committee member
 Employer’s representative - Committee member
 (Clients HSE In-Charge)
 Meeting shall be held on a fixed day of each month, with Client’s participation preferably.
 Minutes of meeting shall be produced by the committee secretary and action parties
identified. Minutes of meeting shall be forwarded to client, PM & contractors HOD’s at site
 Agenda for meeting should include a summary of the month’s HSE activities and
performance, complied according to Client’s and Contractors requirement.
 Other agenda should include but not be limited to the following:
i. Review of project HSE objectives and targets
ii. Any particular concerns on HSE matters that employee may want to highlight.

viii. Stimulate interest in safety by organizing safety week, safety competition, talks and
film-shows on safety, preparing posters or taking similar other measures as and when
required or as necessary.
ix. Perform site visit with a view to check unsafe practices and detect unsafe conditions
and to recommend remedial measures for their rectifications including first-aid medical
and welfare facilities.
x. Result of site observations, inspection and audits
xi. Notify the hazards associated with use, handling and maintenance of the equipment
used during the course of building and other construction work
xii. Suggesting measures for improving welfare amenities in the construction site and other
miscellaneous aspect of safety, health and welfare in building or other construction
work.
xiii. Review health hazards associated with handling different types of chemicals and other
construction materials and to suggest remedial measures including personal protective
equipment.
xiv. Review the last safety committee meeting minutes and action taken report for non-
compliance if any.
xv. Any particular concerns on HSE matters that employee may want to highlight.
Supervisory Staff HSE Meeting
Supervisory staff meeting shall be held on a weekly basis. This meeting shall be conducted by the
site HSE Manager and chaired by Project Manager. The purpose of this meeting is to discuss the
past week’s activities and next week’s activities related to HSE.
9.9 EMERGENCY PREPAREDNESS AND RESPONSE
Emergency Planning and Preparedness
Procedures for Emergency Preparedness & Response to clearly define the action to be taken in
the event of an emergency orpotential emergency shall be drawn up. The emergency procedures
shall be regularly reviewed and updated with reference to IMS Procedure CDP-MR-09. All visitors
arriving on site shall be instructed on the emergency arrangements prior to being allowed on site.
Practice drills for identified emergency situations, including rescue operations shall be
undertaken. Foreseeable emergencies would include, but not necessarily be limited to:
 Major Accidents
 Major Fire
 Electrical Cable rupture
 Earth Collapse
 Natural calamities
 Explosion

 Security alerts
In order to ensure an adequate response to emergency situations it shall be ensured that an
adequate number of suitably trained personnel are appointed who are competent in the use of
firefighting equipment and provision of First Aid. Each site office shall display emergency
response plan as per the attached annexure VII.
Emergency Evacuation
 In case of an emergency area supervisor shall notify the responsible person over telephone.
 The responsible person shall decide whether an evacuation is necessary and shall direct the
activation of alarm signal accordingly.
 Area in-charge shall direct all employees to assemble in the nearest and designated assembly
point in an orderly manner without making noise.
 All plant and equipment shall be switched OFF. Lighting shall remain in ON but capable of
being switched off shall also be switched OFF.
 Where a work group does not have a dedicated assembly point then area in-charge shall
decide the place where to assemble during time of emergency.
 Roll call of the employees shall be taken and status to be reported to the area in-charge.
Steps shall be taken to ensure that every person on site knows the steps and that emergency
drills are tested periodically.
Written information giving details of nearest hospital, fire station, police station, shall be
displayed in prominent locations.
 Instructions for raising the alarm. (At site continuous vehicle horn for 2 minutes shall be alarm
to collect all person at identified assembly point, discreet vehicle horn for 1 minute shall be
considered as situation under control)
 Instructions to report to the nearest assembly point.
 Information about the assembly point.
 An indication of the locations of fire escape routes / safe place.
 Positions of fire extinguishers.
 Instructions about using fire extinguishers.
In case of serious injury special arrangements shall be made available based upon site conditions.
For any severe injury Project Manager / HR Manager / HSE Manager shall be authorized to take
necessary arrangements for lifting / transporting the injured to the correct location.
9.10 PERSONAL PROTECTIVE EQUIPMENT
The site HSE in-charge is ensuring that standard PPEs are available at site as per the defined PPE
matrix. As a minimum dress code all Personnel shall be required to wear hard hat, safety
footwear and Fluorescent jacket at construction sites. This shall be emphasized during the site
induction training.
Personnel not having appropriate PPE will not be permitted to work on site. Where there is need
for additional personal protective equipment, safety spectacles, hearing protection etc, based on
hazard - risk assessments, the user shall get it issued from store before moving to site.
All Supervisors shall be responsible for ensuring that suitable PPE is available, in serviceable
condition and that it is used at the required times.

Head Protection:
All personnel shall be issued with safety helmets, which shall be worn at all times when in
construction environment. Safety helmets shall be checked periodically for signs of wear and in
particular for cracks in the shell and damage to the hammock. Any evidence of such shall result in
the helmet being discarded and replacement issued.
Hand Protection:
Workers employed on bar bending, mixing asphalt materials, cement, lime mortar and concrete
shall be provided with water protective footwear (Gum boots) and protective gloves. Appropriate
gloves shall be worn at all times by all personnel involved in works where hands are exposed to
harmful substances, cuts, abrasions, punctures, chemical burns or harmful temperature
extremes.
Foot Protection:
Suitable hard toe foot wear shall be worn at all times by all personnel considering the nature of
works and danger of foot injuries due to falling objects, rolling objects, object piercing the sole or
when employees foot are exposed to electrical hazards.

Ear Protection:
Ear protection shall be worn by personnel involved in areas of high noise level or when working
with equipments that generate high noise. Whenever practicable, equipment generating high
noise shall be located at the maximum possible distance from any work being performed. Sound
reducing boxes shall also be fitted to the equipment wherever practicable.
Eye Protection:
Safety glasses with rigid side shields shall be worn at all in areas where eye hazards exist due to
flying particle, high speed flying particles, dust ingress, chemical splash, radiation glare, hot
sparks or metal spatter or harmful vapors. Face shields shall be worn in conjunction with safety
glasses when grinding, chipping, jack hammering.
Respiratory Protection:
Respiratory equipment, weather stand bottled-type or self- contained breathing apparatus shall
only be worn by trained and qualified personnel. Those engaged in mixing or stocking cement
bags or any materials that are injurious to eyes and lung shall be provided with protective goggles
and masks.
Fall Protection:
All personnel shall comply with the usage of full body safety harness (double lanyard), which
states that anytime employees working from an unprotected elevation of 2m or more, fall
protection must be used. Additional safety measures like providing fall arrestor life line (PP rope
of 8mm) and safety net shall be provided depending upon the site conditions and job
requirements
Face Protection:
Face shield is required when there is a possibility of materials penetrating the skin area of the
face, specifically in grinding operation and abrasive blast cleaning operations.

Some of devices / instruments shall be required and maintained for monitoring of various EHS
aspects i.e Portable multi gas detector, Sound level meter, Digital lux meter, ELCB &Earthing
resistance Meter, Fire blanket and fire extinguish system during execution of project.
9.11 WELDING AND FLAME CUTTING
Flame Cutting
In flame cutting operation, the metal plates are cut with the flame of burning fuel gas and other
gases and sometime include the use of pressure. In oxygen cutting, the metal is heated by
burning fuel gases and then severed or removed by the chemical reaction of metal and oxygen.
The handling and storage of gas cylinders needs particular attention.
 Welding and cutting gases commonly used, include oxygen, acetylene and argon. Reference
should be made to the relevant material safety data sheets.
 Protect cylinders from cuts or abrasions.
 Do not lift compressed gas cylinders with an electromagnet or slings. Where cylinders must
be handled by a crane, carry them in a cradle or suitable platform.
 Do not tamper safety devices in valves on cylinders.
 Load cylinders to be transported, allowing less movement between cylinders. Secure them to
prevent violent contact or upsetting.
 Cylinders should be stored in a safe, dry, well ventilated place prepared and reserved for that
purpose.
 Cylinders should not be stored near elevators, gangways, stairwells, or other places where
they can be knocked down or damaged
 Cylinders of oxygen stored indoors should not be within 6 meters of cylinders containing
flammable gases or highly combustible materials.
 Acetylene and liquefied fuel gas cylinders should be stored with the valve end up.
 Defective pressure gauges shall not be permitted on regulators.
 Gas cylinders during use shall be kept vertical, secured by chain on mobile trolleys having
rubber tyres.
 The oxygen regulator should be equipped with a safety relief valve or be so designed that,
should the diaphragm rupture, than broken parts will not fly.
 Never allow oxygen and oil to mix as it may cause an explosion.
 Long hoses should not be used, as they are difficult to purge. Leaking hoses must be repaired
immediately.
 Protect hoses from sparks, slag, grease, oil and hot objects.
 Check valves (flashback arrestors) should be installed at the torch end of each hose to prevent
back-flow of gases.
 Before changing torches, shut off the gas at the pressure reducing regulators and not by
crimping the hose.
 Never put down a torch until the gases have been completely shut off.
 Do not hang torches from a regulator or other equipment that allows them to come in
contact with the sides of gas cylinders.

 When extinguishing the flame, close the acetylene and oxygen valves in the order
recommended by the torch manufacturer.
 NEVER keep gas cylinders inside a confined space.
 When gas welding is complete it is essential that the shut off valve on the cylinder is closed so
there is no danger of the gas building up in a confined area such as an excavation, vessel or
tank.
ARC Welding
In arc welding, metals are joined by heating with application of pressure or use of filler metal. The
process includes shielded welding which uses gas or a solid flux to blanket the weld. Welding
cables may be subjected to severe abuse if they are dragged over work under construction and
across sharp corners, or run over by vehicles. Special attention needs to be given to prevent
electric shock.
 Replace or repair defective cable immediately.
 In confined areas, cover or arrange cables to prevent contact with falling sparks.
 Never change electrodes with bare hands, wet gloves, or when standing on wet floors.
 Earth or ground the frames of both portable and stationary welding units.
 Keep welding cables dry and free of grease and oil to prevent breakdown of insulation.
 Take special care to keep welding cables away from power supply cables or high-tension
wires.
 If a cable (either a work lead or electrode lead) exposes bare conductors, then cover the
exposed portion with rubber, plastic or friction tape equivalent in insulation to the cable
covering.
 If cables have to run some considerable distance from the welding unit then consider placing
them onto overhead supports.
 Protect cables that must be laid on the floor or ground so they will not interfere with safe
passage or become damaged or entangled.
 If it is necessary to weld or cut near combustible materials, the combustible materials should
be moved to a safe distance away or protected by non-combustible curtains or sheet metal.
Floors of wood or other combustible materials should be swept clean and, if possible,
protected with a flame resistant covering. Alternatively combustible material is required to
be isolated.
 A fire watch and a fire extinguisher should be maintained throughout the welding or cutting
operation and continued for at least 30 minutes after job completion
 Gases (the oxides of nitrogen, carbon monoxide, ozone) and metallic dusts and fumes may
pose respiratory hazards during welding work. If gases, dusts, and fumes cannot be kept
below the Threshold Limit Values, welders should wear suitable respirator. Should oxygen be
deficient, self-contained breathing apparatus or hose masks with blowers are necessary.
 Use local exhaust ventilation or general ventilation systems in confined areas, such as tanks
and pressure vessels. This precaution should maintain toxic gases, fumes, or dusts below the
maximum allowable concentrations.

 Local exhaust removal or general ventilation should be used when welding involves coatings
or fluxes containing base metals that contain zinc, fluorine, beryllium, lead, or cadmium, and
their compounds, so that the concentration of any toxic fume generated is below the TLV.
 Local exhaust or general ventilation should be used to maintain the concentrations of oxides
of nitrogen within safe limits.
 When ultraviolet radiation passes through the air, ozone can be formed in the welding area.
General ventilation will usually control ozone levels in the welding area.
 Inert gas-shielded arc welding requires that precautions be taken to provide proper
respiratory protection, positive ventilation, local exhaust removal, or approved respiratory
equipment, or a combination of them.
9.12 GAS STORAGE:
A level base of compacted earth, concrete or paving slabs shall be provided and surrounded by
a secure chain link fence at least 2m high, with a hard standing provided for the delivery and
dispatch of cylinders. If the compound is more than 12sqm two exits should be provided in
opposite corners of the compound. If less than 12sqm one gate will be sufficient. Gates should
open outwards and always be unlocked when someone is in the compound. There should be
sufficient shelter to prevent cylinders from being exposed to extremes of weather.
Signs must be clearly displayed indicating the type of gas cylinders and prohibiting of smoking
and the use of any naked flame in the area of the store.
Cylinders must be stored with their valves uppermost, and not mixed with any other gases or
substances.
9.13 MANUAL HANDLING:
Lifting and moving loads manually are one of the most common causes of injury at work.
Always follows safe method for manual handling.
No workers lifts by hand or carries overhead or over his back or shoulders any material, article,
tools or appliance exceeding in weight the max limit in the following table:

9.14 WORKING AT HEIGHT
Where work at height is required the team leader will ensure that all such work is properly
controlled and that safe access and working platforms are provided in accordance with statutory
requirements. Use of full body harness is mandatory for persons working at height of 2 meters or
above. If working on top of platform, lifeline to be installed for hooking the safety belt. In difficult
location, fall arrestors are required to be used. Where such alternative methods are used a

system of work will be devised to ensure that all aspects satisfy the relevant statutory provisions
and provide the personnel involved with a safe place and system of work.
9.15 SCAFFOLD ERECTION AND DISMANTLING
Scaffolding is the structure (wood or metal) which supports an elevated working platform. Use of
wooden scaffold shall be discouraged. The following general requirements are applicable: -
 Scaffolding shall be erected, moved, dismantled or altered under the supervision of a
competent person. Scaffolding work to be done only by trained and experienced personnel.
 Never interchange the scaffolding components of different manufacturers.
 Maintain scaffold components (clamps, boards, standards etc.) in clean condition. Inspect
boards before use.
 Handle scaffold components with care. Do not throw or drop items.
 The footing must be level, sound, rigid and capable of carrying the maximum intended load
without settling or displacement. Unstable objects such as barrel, boxes, loose bricks or
concrete blocks are not to be used to support scaffolding or planks.
 Guard-rails, mid-rails and toe boards must be installed on all open sides and ends of platforms
more than 2000 mm above the floor.
 Ladder access shall extend one meter above the landing.
 Where persons are required to work or pass under the scaffolding, such scaffolds shall be
provided with a screen between the toe-board and the guardrail extending along the whole
opening.
 Overhead protection shall be provided where necessary.
 Slippery conditions on scaffolds shall be eliminated immediately
 The poles, legs or uprights of scaffolds shall be plumb, and securely and rigidly braced to
prevent swaying and displacement.
 Scaffolding planks must extend over their end supports for a distance of 150mm to 300mm.
 All planking of platforms shall be overlapped a minimum of 300 mm and secured from
moving.
 Scaffolding or its components shall be capable of supporting load without failure, at least four
(4) times the maximum intended load. The manufacturer should be able to indicate the safe
working load of his product.
 Any scaffold or component of a scaffold that is weakened or damaged must be repaired or
replaced immediately.
 An access ladder or equivalent safe access shall be provided.
 Inspected by a competent person at least once a week and after inclement weather.
 “Safe to use” [Green Tag] or “Unsafe for use” [Red tag] signs placed near to access ladder.
Weekly scaffold inspection sheet shall be completed and suitable Green / Red tag to be
placed near access ladder. Any adverse remarks related to scaffold shall be promptly
communicated to concerned in-charge.
9.16 LIFTING OPERATIONS
All crane operations and the lifting of loads will be under the experienced supervisor/ or appointed
Lift Coordinator.

The Contractor will only employ crane and lifting equipment that has been tested and certified as
fit for purpose. All crane operators and riggers will be adequately trained. The Contractor will
keep records of tests and certification of all lifting equipment crane employed on the Works. In
case of the load exceeding 20 MT load, lifting plan and work permit is essential. Safety limit
switches and maintenance records of all cranes shall be routinely inspected by the Contractor
and made available for Employer audits.
9.17 TRENCHES AND EXCAVATIONS
Before commencing any excavation work the Site In-Charge will ensure that the proposed works
have been adequately assessed and planned to ensure that they are executed safely and
without risks to Health. The factors to be assessed and planned will include: -
 A thorough survey of the area to determine the presence of any underground or overhead
services. The survey will include a review of drawings provided by the relevant statutory
undertakings and the Client and a physical survey using service locating equipment.
 The nature and stability of the material being excavated and the need for temporary support
of walls and/or roof.
 The effect of excavation on nearby structures.
 The foreseeable presence of hazardous contaminants.
 The proximity of mobile plant.
 The provision of edge protection (fall prevention of people and materials)
 Access and egress
 Flooding
 Ventilation
In addition, any excavation of significant depth (More than 2 meters) will be subject work
permit. Complexity of excavation location may require a written method statement detailing
the sequence of work and the relevant procedures and precautions to be observed.
9.18 CONCRETE AND MASONARY WORK
All construction workers should be alert to the potential hazards associated with concrete and
masonry operations.Among the most critical unsafe practices are:
 premature removal of formwork;
 failure to brace masonry walls;
 failure to adequately support precast panels;
 Inappropriate operation of equipment;
 failure to guard the end of reinforcing steel;
 Inadequate shoring, which can lead to formwork collapse.
General safety requirements should always be followed on construction, demolition, alteration or
repair worksites-notjust because of OSHA standards, but because serious accidents and injuries
may happen if they are not.
 Construction Loads: Construction loads must not be placed on a concrete structure unless it
has been determined that the structure is capable of supporting the intended loads, based
on information received from a person who is qualified in structural design.

 Reinforcing Steel: All protruding reinforcing steel, onto which an individual could fall, must
be guarded to eliminate the hazard of impalement.
 Concrete Buckets: Under no condition may employees ride concrete buckets.
 Working under Loads: Never work under concrete buckets while the buckets are traveling,
being elevated or lowered into position. Elevated concrete buckets must be routed so that
no employee is exposed to the hazards associated with falling concrete buckets.
 Personal Protective Equipment: Protective head and face equipment must always be worn
when applyingcements grout through a pneumatic hose. A safety harness must be worn or
equivalent fall protection providedwhile placing or tying vertical reinforcing steel and
working more than 6 feet above any adjacent working surface.
 Masonry Construction:
Whenever a masonry wall is being constructed, a limited access zone must be
establishedprior to the start of construction. The limited access zone must be as follows
9.19 FIRST AID / MEDICAL CARE

Qualified First aiders and First aid facilities shall be at each workplace. Every injury shall be
reported and treated. First aiders shall be in possession of a valid first aid certificate issued by a
competent authority.
The first aid facility shall be in accordance with applicable Laws. Alternatively the agreements
shall be made with nearby hospital for treatment of injured or persons suffering from sickness. A
first aid log will be maintained for injuries and sickness. Agreement should have provision to
provide ambulance service on call basis or for deputing at site in case of critical activity on
standby during work hours.
At every Work place First aid facilities shall be provided and maintained. The first aid box shall be
distinctly marked with a Red Cross on white background. The first aid box shall be kept by work
supervisor and by a responsible person who shall always be readily available during the working
hours of the work place.
9.20 FIRE PREVENTION AND PROTECTION

It shall be ensured that adequate preventative and protective measures are taken to control fire
hazards. These arrangements shall include:

Unauthorized fires are not permitted on the construction site. Burning shall only be done in
designated areas.
FIRE WARDENS
To ensure site safety, the designated persons shall be assigned to act as fire wardens. Fire wardens
shall be given instruction and training on all emergency procedures, firefighting equipment
familiarization by Site HSE Manager. In association with Site Manager, periodic fire drills shall be
carried out with minimum frequency of every 6 months and all fire suppression equipment shall be
checked and certified.
9.21 CONFINED SPACE WORK
In general case, work will be carried out without resorting to entry by personnel. When working
in a confined space is unavoidable, a detailed safety procedure shall be prepared. Information
shall be detailed under the following headings:
 Definition of confined space and vessel entry work.
 Hazards.
 Duties of key personnel.
 Formal application to do confined space and vessel entry work.
 Withdrawal of plant from service.
 Isolation.
 Cleaning and purging.
 Provision and use of gas monitoring equipment.
 Testing method for gas or fumes and the adequacy of oxygen supply.
 Calibration of portable gas analyzers.
 Self-contained breathing apparatus.

9.22 LIGHTING
Illumination is essential for safety, security and production as it related to every workplace,
approach, dangerous opening and lifting appliance.
 Walking and working areas shall be adequately illuminated during periods of occupancy.
 No dark spots shall be present in workplace.
 Illumination level shall be sufficient for detail of work performed. Recommended lighting
levels shall be as follows:-
 General work area involving sites clearance and rough work – 50 lux.
 Craft work such as reinforcing concreting – 100 lux
 Fine craft work such as all work with power tools, electrical, welding – 300 lux
 Emergency lighting for escape – 50 lux
 No faulty light fixtures / bulbs shall be fitted.
Hand lamps where required shall be provided with protective cover and 25 volts.
9.23 VENTILATION
Heat, Dust and Fume Control
 Risk areas clearly identified. Physical condition of air is consistent with comfort and health
requirements.
 Ventilation shall be adequate where toxic or irritating substances are present such as welding
fumes, vehicle exhaust fumes, carbon dioxide, carbon monoxide, ammonia, chlorine, paints
and solvents, silica dust, talc, reparable dust, road tar vapors / wood preservatives
 No dust concentrates on floors, beams and pipes.
 Doors exist between different environmental systems.
 Local extract ventilation systems (hoods, ducting, filters, cyclones, scrubbers, electrostatic
precipitators, dust control sprays and fans) if required shall be installed and operated in safe
condition, not obstructed with accumulation of materials.
 Extract ventilation systems on preventative maintenance program.
 Regular inspection of ventilation systems.
 No food kept / eaten in toxic environment.
 Flow of air currents occurring in proper direction.
 Dust, fumes, smoke or gases removed by hoods, air channels, exhaust fans etc.

- Entry to workplace containing harmful smoke, gas, fumes or dust only permittedif
effective respiratory apparatus worn so as to prevent the inhalation of such smoke,
gas, fumes or dust.
 Ventilating air should:
- Be free from dust, smoke or other impurity.
- Provide safe and healthy environmental conditions to all areas used by workmen.
- Be adequate and capable of diluting and rendering inflammable, noxious gases and
dusts harmless.
- Not re-circulate.
- Be circulated adequately over and around chemical mixing tanks.
9.24 DEMOLITION
Before allowing any demolition of existing structures to proceed, a thorough survey of the
relevant structure will be undertaken and a comprehensive risk assessment shall be carried out.
Based on the results of the survey and risk assessment, controls shall be communicated to all
concerned persons. In case of criticality of structure to be demolished, a detailed method
statement will be prepared detailing the proposed method and sequence of demolition and the
procedures and precautions to be observed.
9.25 PLANT AND EQUIPMENT
Plant and equipment are potential sources of pollutants, so it is important to be concerned of the
location and its role.
The potential source of pollutants, which will come out from different construction activities
would be suspended particulate matter (SPM). The main sources are:
 Exhaust coming out from different equipment’s.
 Dust generated as the result of different construction activities.
 Dust and suspended particles during transportation of material.
CIPL will implement its standard procedures designed to ensure that all such plant and
equipment is free from defect, has the necessary guards in place, that all statutory tests,
examinations and inspections, where appropriate, have been carried out and that it is safe for
its intended use.
 To this end the Project Manager will appoint a suitably qualified and / experienced person
to carry out initial checks of all company owned or hired plant upon arrival on site.
 Each item of plant and equipment will be subject to a Preventative Maintenance Program to
ensure that it remains safe for use.
 All contractors will be required carry out similar checks and documentation of the plant for
which they are responsible, alternatively CIPL, P&M representative shall perform the same.
 All plant and equipment will be operated only by personnel trained and competent to do so
and, where applicable, by holder of the relevant certificate of training issued by a
competent authority. They shall be approved by P&M In-charge before permitting to work
at site.
 All mobile plant operators will be issued with identity card for authorization to operate
relevant plant and equipment on site.

9.26 COMPRESSED AIR AND AIR POWERED TOOLS, HAND TOOLS & POWER TOOLS
COMPRESSED AIR AND AIR POWERED TOOLS
Compressed air whilst being a valuable industrial aid is also capable of being a serious hazard. The
improper use of compressed air has resulted in internal injuries, perforated ear drums and even
death. It has also led to severe damage to machinery.
 Never clean clothes and the work area with compressed air - use brush or vacuum
equipment.
 If an airline or coupling comes apart, it can whip about dangerously. Always fit a safety chain
to couplings.
 Do not drive vehicles over air-hoses. Provide two planks either side of the airline where they
cross roads, walkways.
 Always shut off air pressure before disconnecting air-powered tools.
 Air powered tools such as jackhammers, drilling machines, grinders, and hoists and riveting
guns subjected to scheduled maintenance inspections by competent person.
 Eye protection worn by operator and if noise is a hazard then hearing protection worn.
HAND TOOLS & POWER TOOLS
Hand tools are non-powered. They include anything from axes to wrenches. Today’s power tools
offer more power, adaptability and dependability than ever before.
Following 5 basic safety rules can prevent all hazards involved in the use of tools:
 Keep all tools in good condition with regular maintenance (CHECK INSPECTION STICKERS).
 Use the right tool for the right job.
 Inspect each tool for damage before use.
 Operate according to the manufacturer’s instructions.
 Provide and use the proper PPE.
The greatest hazards posed by hand tools result from misuse and improper maintenance. The
tool may also become unsafe to use. In fact, it is estimated that hand tool mishaps are
responsible for about 1 out of 12 workplace injuries-including cuts and bruises, punctures,
fractures, even loss of finger, hand, or eye.
Precautions
 Employees have the responsibility for properly using and maintaining tools.
 Appropriate PPE should be worn due to the hazards that may be encountered while using
portable tools.
 Avoid dangerous environments. Don’t use power tools in a damp, wet and/or explosive
atmosphere.
 Around flammable materials, sparks produced by iron and steel hand tools can be ignition
source. Where this hazard exists, use spark-resistant tools made from brass, plastic, aluminum, or
wood.
Employees should be trained in the use of all tools-not just power tools. They should understand
the potential hazards as well as the safety precautions

 Never carry a tool by the cord or hose.


 Never yank cord/hoses to disconnect from receptacle.
 Keep cords/hoses away from heat/oil, sharp edges.
 Disconnect tools when not in use, before servicing, when changing accessories such as
blades/bits cutters.
Employees using electric tools must be aware of several dangers; the most serious is the
possibility electrocution.
Among the chief hazards of electric-powered tools are burns and slight shocks, which can lead to
injuries or even heart failure.
GRIT BLASTING CLEANING
Grit includes particles of chilled iron globules, powered quartz, emery or other hard granular
material used in blast cleaning. Blast cleaning means the blowing of grit using a system energized
by compressed air, water or steam with the purpose of removing scale, rust or old coatings from
a surface prior to painting or coating.
 Grit blasting shall only be done under the supervision of an experienced and competent
person.
 Personnel doing grit blasting work must be competent and experienced in this work.
 Breathable quality air, free from oil mist, moisture and toxic gas shall be supplied to the
operator’s hood. This is enabled by passing the compressed air through a regulator and
suitable pre-filter. The pre-filter shall be inspected daily. Additionally, if there is any danger
of inhalation of dust an appropriate dusk respirator shall be worn by persons at risk.
 The supervisor in charge shall ensure that grit-blasting personnel are properly instructed
before starting such work.
 The supervisor in charge shall ensure that equipment is maintained in safe and good working
order.
 The workplace shall be a restricted area and a rope barricade shall enclose it. Warning signs
indicating “DANGER -GRIT BLASTING” should be posted on the barricade.
 Provide suitable materials to act as curtains (tarpaulins, plastic sheeting etc.,) to enclose the
work area so as to retain grit within the area and protect personnel outside the area.
 Provide suitable and secure covers to protect instruments, gauges, airline valves and similar
items which are not to be cleaned.
 Protect electric cables and airlines, which energize blasting equipment so that they are not
likely to be damaged both within and outside the workplace.
 Establish a good housekeeping system to ensure that excessive debris and spent grit is
removed as early as possible, and in any case at the end of the day’s work.
 Grit blasting of steel surfaces close to flammable or explosive mixtures is hazardous,
principally due to static electricity, which may be generated.
 No grit blasting shall take place in the immediate vicinity of operating electric motors or air
intakes to operating internal combustion engines. Such items of equipment must be stopped
and protected from grit blasting particles.
 Grit blasting procedure available and communicated to painting personnel.

PAINTING
Painting includes the application of a protective coating material by the use of a compressed air
spray painting system, the hydraulic atomization (airless) method: the use of brushes, rollers,
sponges or similar equipment. Hazards associated with painting include dust, noise, fire,
explosion and the inhalation / ingestion / absorption of toxic solvents.
 Painting should only be done under the supervision of a competent person. Those personnel
doing the painting must also be competent to do the work.
 The supervisor in charge shall ensure that equipment is maintained in safe and good working
order.
 Breathable quality air, free from oil mist, moisture and toxic gas shall be supplied to paint
personnel if required. This is enabled by passing the compressed air through a suitable paint
filter respirator or regulator and suitable pre-filter. The pre-filter shall be inspected daily.
 The workplace shall be a restricted area and a rope barricade shall enclose it. Warning signs
indicating “DANGER - PAINTING AREA” should be posted on the barricade.
 Provide suitable materials to act as curtains (tarpaulins, plastic sheeting etc.,) to enclose the
work area and protect the newly painted surface until it is dry.
 If paint components are flammable, then no painting will be done in the vicinity of electrical
lighting and open electrical circuits unless such equipment is explosion / vapor proof or
switched off and locked out.
 Symbolic signs for “NO SMOKING” and “NO NAKED FLAMES” posted in areas where paint /
flammable liquids are in use
 Provide suitable and secure covers to protect instruments, gauges, airline valves and other
equipment which is not to be painted.
 Establish a good housekeeping system to ensure that excessive debris and paint containers
and masking tape is removed as early as possible, and in any case at the end of the day work.
 Painting within confined spaces or vessels shall always be controlled by the work permit.
 Material safety data sheets for each product held and observed. A product data sheet is
inadequate. Copies to be kept by painting supervisor.
 Unwanted flammable materials including paint thinners, rags and cotton waste impregnated
with flammable spirits or paint are to be kept in metal drums with close fitting lids and
disposed of in a safe manner away from spray painting areas.
 Painting procedure available and communicated to painting personnel.
 Suitable respirator used by crew to protect them from hazard solvent / paint vapors.
9.27 ELECTRICAL SAFETY
Only authorized and competent personnel will install all site electrical supply systems. All
electrical equipment shall be inspected and certified as safe for use prior to commissioning.
Electrical supply systems shall be maintained in safe condition; all portable electrical equipment
will be 220 volt or lower. For requirement of high voltage Client shall be contacted for
permission. All overhead supplies will be protected with suitable goal posts and prominently
placed precautionary signs. Regular checks and inspections shall be carried out on all electrical
equipment’s and recorded with the notice on the piece of equipment confirming it is safe.

Power Supply
Electrical supply systems shall be maintained in safe condition as per good industry practices. All
portable electrical equipment shall be 220 volt or lower. For requirement of high voltage Client
shall be contacted for permission. All overhead supplies shall be protected with suitable goal
posts and prominently placed precautionary signs. Regular checks and inspections shall be carried
out on all electrical equipment’s and recorded with the notice on the piece of equipment
confirming it is safe. Besides this following precautions shall also be taken:
 All switch boards, extension boards etc. should be protected from rain and water. No water
logging shall be allowed around switchboards.
 Earth leakage, circuit breakers shall be provided on all distribution boards and main
switchboards.
 All fuses shall be of good quality and confirm to correct ratings. Use of MCBs shall be
preferred.
 Working on energized circuit / live wires is strictly prohibited.
 Electrical maintenance workman must use wooden platforms insulated tools and rubber
boots
 While crossing overhead electric lines, all operational movements of excavators, cranes, side
boom tractors and all other tall items of equipment have to be carefully observed
Temporary electrical installations
Only authorized and competent personnel shall install all site electrical supply systems and shall
be responsible for the control of all maintenance and repairs to any electrical switchboard
distribution board, hand tools. All electrical equipment shall be inspected and certified as safe for
use prior to commissioning. The provision of all connections and equipment required shall be in
accordance with these safety conditions and comply strictly with applicable Electricity Rules.
Power Lines:
When work is to be performed in the vicinity of overhead power lines, the following precautions
shall be observed:
 Proper signage identifying the hazard is to be erected.
 Safe clearance distances are to be observed based on the voltage in the transmission lines.
 Goal post with suitable clearance shall be installed.
9.28 HEALTH & HYGIENE
Drinking Water
Drinking water shall be provided and maintained at suitable places. It should be accessible to
labor and employees with sufficient supply fit for drinking. All water supply or storage shall be
away from any toilet drain or other source of pollution. Regular analysis of water shall be done
to ensure its portability.
Eating Facilities (In Plant Catering)
Meals shall be provided within a non-metallic container and shall be consumed only in the
designated messing area. Proper waste disposal facilities shall be provided. Waste shall be
removed immediately from the site after the lunch period and the area kept neat and clean at all
times.

Good House Keeping
Used canisters, trash or other materials shall be collected and removed daily. Flammable
materials will be properly stored. Areas will be maintained free of tripping hazards or excess
materials.
Provision of Shelter during Rest
At every place, shelter shall be provided for meals and rest.
Occupational Health
i) All operations shall be identified that can adversely affect the health of workers and
mitigation measures shall be implemented.
ii) To eliminate radiation hazard, it shall be ensured that Tungsten electrodes used for Gas
Tungsten Arc Welding shall not contain Thorium.
iii) Inhalation of air borne contaminants like silica, asbestos, gases, fumes, etc. shall be
avoided by using appropriate respiratory protective devices.
iv) Improper lifting, carrying, pushing & pulling of heavy loads shall be avoided. Wherever
possible, manual handling shall be replaced by mechanical lifting equipment’s.
v) Power driven hand-held tools shall be maintained in good working condition to minimize
their vibrating effects and personnel using these tools shall be taught how to operate them
safely & how to maintain good circulation in hands’.
Working Hours
Normally the work shall be carried out during daylight hours. If during the course of the project night
work is required, all support facilities such as the lighting shall be provided in order to keep the safe
working conditions. No persons shall be permitted to work continuously day & night.
Smoking
Smoking is prohibited in Public Place as per prevailing legislation. The same rule shall bein effect
at the work site. Smoking shall be strictly prohibited at all locations designated by “NO
SMOKING” signs.
Inspection
All the welfare facilities shall be inspected periodically to ensure its effectiveness by admin/HSE
personnel jointly.
9.29 BEHAVIOUR MODIFICATION AND DISCIPLINARY ACTION
Non-conformance with HSE requirements can be focused on people, either the supervisor or the
worker. The real or basic cause of non-conformance occurs because of either a lack of knowledge
or skill or an improper motivation or attitude. It follows that the solution for lack of knowledge or
skill is proper training and coaching. In the case of improper motivation or attitude, it is the duty
of the person in charge of the offender to establish the cause by using counseling and coaching.
When people are knowledgeable and counseling and coaching have not provided a solution then
disciplinary action shall be taken. The team leader is responsible for implementation of coaching,
counseling and formal discipline of subordinates.
9.30 SUBCONTRACTOR’S HSE MANAGEMENT SYSTEM
Subcontractors shall be required to establish a HSE plan approved by CIPL or Client, in line with
this plan incorporating all requirements of CIPL / Client and also follow COMPANY’s additional
directives at site, as required. Alternatively, they shall adopt CIPL HSE Plan in totality.

10.0 ENVIRONMENT MANAGEMENT
Air, Water and Land Pollution, contamination and environmental damage is a major focus area
and techno-commercial efforts shall be made by all CIPL employees, to have effective control
measures in place and to avoid it. For reducing waste generation, waste management plan shall
be followed wherein the waste generation shall be controlled at source. However, inevitable
waste shall be kept in segregation and efforts shall be made for its reuse. The disposal shall be
done in the location identified by client or as per local applicable legislation. Proper storage and
utilization methodology of materials that are detrimental to the environment shall be ensured.
Where required, only bio degradable and environment efficient materials are selected and
emphasis shall be given on recycling of waste materials such as metals, plastics, glass, paper, oil &
solvents. The waste that cannot be minimized, reused or recovered shall be stored and disposed
of safely. In no way, toxic spills shall be allowed to percolate into the ground.
Any chemical including solvents and paints, required for construction shall be stored in
designated bonded areas around the site.
The main objectives of Environment Management System are:-
 To access the environmental condition of the area where the construction is carried out.
 To provide timely indication if any environmental control measure fails.
 To achieve lay down / acceptable standards.
 To monitor effectiveness of environmental mitigation measures.
 Ensure compliance with regulatory requirements and similar laws and codes as applicable for
control of environmental pollutant’s (such as dust generation, construction debris, waste oil,
used batteries, steel and wood scrap, disposal of used / waste water as applicable).
10.1 Mitigation measures including contingency plans
Transportation of material from one place to another generates air pollution, which degrades the
atmosphere of the area. To minimize the pollution, precaution will be taken to minimize visible
particulate matter by sprinkling water and optimizing material movement. Fuel efficiency of all
construction equipment’s shall be measured and monitored using Microsoft Excel software.
Based on the analysis of fuel consumption data, proactively the equipment shall be taken for
preventive maintenance.
Dust Control
Covering, arresting possible leaking points or Water spraying shall be undertaken to control dust
during handling of cement/concrete. Appropriate precaution shall be taken to minimize dust by
engineering control.
Contingency Plan
If any environmental ascendance of limiting value (either accidental or through inadequate
implementation of mitigation measures) does occur, the cause will be quickly identified and
remedial action shall be taken immediately.
Water Pollution Control
 In the drainage system at the constructed site oil traps shall be installed to control the oil
quantity in water below 10 mg/l.
 The oil from these drains shall be collected for suitable disposal.

 Used / waste oil from the equipment’s shall be collected in drums and shall be disposed of
two authorized recyclers of Pollution Control board of respective state.
 Used batteries shall be collected and shall be returned to the agent / dealer.
In the event of any spillage, the principle is to recover as much material as possible before it
enters a water source or land contamination and to take all possible action to prevent spilled
materials from running off the site. CIPL shall use appropriate clean-up techniques.
10.2 WASTE MANAGEMENT PLAN
The contractor shall develop a program to identify and record waste that will be generated during
construction and commissioning and shall develop procedures based on criticality of waste for its
disposal in compliance with applicable regulations and best practices.
The impact of waste shall be reduced through segregated collection. Besides during mobilization,
construction, demobilization and commissioning of construction projects the contractor shall
minimize the waste through optimum utilization and reuse wherever possible.

11.0 SECURITY PLAN
11.1 PHYSICAL BARRIERS
Premises such as the office / lay down area shall be enclosed with a physical barrier such as a
fence or wall. Entrances shall be provided through which vehicles and personnel shall be
controlled.
ACCESS CONTROL
Access to the project area shall be controlled at the gate provided on the perimeter fence. CIPL
shall provide security guards to monitor entry of all employees or as per the requirement of the
client.

11.2 IDENTIFICATION: PERSONNEL, VISITORS AND VEHICLES
The work supervisor shall conduct daily roll call before leaving the base camp and after the day’s
work. Records of such shall be maintained. Employees and authorized visitors of Client and CIPL
and their Sub-Contractor shall be required to prove their identification for registration purposes.
In the case of vehicles fuelled using unauthorized LPG shall not be permitted.
Security personnel shall examine gate and vehicle passes at the controlled access gate. If they are
in order then access shall be permitted. All visitors on site shall be registered by site security and
only then issued with a visitor pass or permitted to enter site. All visitors onto any site shall be
accompanied at all times by a suitably experienced member of staff. When on site, visitors shall
be accompanied at all times by an authorized person and all sub-contractors shall conform to this
rule.
11.3 CONTROLLING MOVEMENT OF MATERIALS
The main purpose of security is protecting the assets of any organization from the risks of fire,
theft, sabotage and vandalism. Therefore close attention by security shall be given to the
movement of materials both in and out of the premises.
Entry of vehicles (Particularly delivery trucks) shall be checked. Delivery notes shall be examined
and loads shall be directed to the authorized recipient and where necessary an escort shall be
provided. Vehicle entry and exit shall be recorded by security.
Any materials to be removed from the area shall be the subject of a material movement gate
pass. Security shall examine such materials against the material movement gate pass and if in
order shall allow it to proceed by countersigning the pass with his signature, date and time. A
copy of such pass shall be maintained by security.
11.4 COMMUNICATIONS
Telephone shall be available at the gate / check post. Arrangements shall be in place to monitor
security personnel at regular intervals to ensure their welfare and safety.
Periodic checks of each security post shall be made during the day and at night by supervision. An
entry shall be made in the occurrence book whenever each post is checked.
12.0 TRAFFIC MANAGEMENT
Any unsafe driving practices observed will be subject to disciplinary action. Designated drivers
shall have a valid driving license and approval by P&M Site In-charge. Each driver shall be
instructed on Safe Driving, Company and Plant traffic regulations. Traffic regulations and laws
shall be adhered to at all times
 All drivers shall hold a valid driving license for the class of vehicle to be driven and be
registered as an authorized CIPL driver with the HR Department.
 Drivers and passengers must wear seat belts.
 All vehicles and powered mobile plant shall be equipped with reversing alarms, and banks
men shall be deployed during reversing.
 Securing of the load shall be by established and approved methods, i.e. chains, slings, nylon
straps or other tightening equipment for steel/heavy loads. Sharp corners on loads shall be
avoided when employing ropes for securing.
 All overhangs shall be made clearly visible and restricted to acceptable limits.
 Load shall be checked before moving off and after travelling a suitable distance.

 All vehicles used by CIPL shall be in road worthy condition and in conformance to the Land
Transport requirement.
 All vehicles shall have reflectors and speed controllers in good working condition.
 A P&E supervisor shall be designated by CIPL so as to ensure that site traffic proceeds within
acceptable speed limits and through predefined path without disturbing surroundings.
12.1 TRANSPORTATION OF THE PERSONNEL
Transportation of personnel shall be done by Bus / Jeep from site camp to work location. Before
use of any Bus / Jeep, it shall be ensured that it is inspected and approved by Plant & Equipment
department of CIPL. The loading of personnel shall not exceed maximum permissible capacity of
the vehicle.
12.2 TRANSPORTATION OF PERSONNEL BY ROAD
Transportation of personnel shall be done in buses/Jeeps in accordance with following conditions:
 Driver must possess valid driving license.
 No vehicle shall be operated above the speed limit of 60 kmph during transportation on NH.
 The vehicle shall be equipped with valid road permit etc. as per the local governing rules.
 The driver must drive in a safe and steadily manner and safety belts are to be fastened.
 Vehicle speed shall not be more than 20KMPH inside work location /premises.
 Vehicle must be in good condition and in conformity with legal requirements. A first aid kit
must be kept at a suitable location within the vehicle.
 The passenger capacity shall not exceed.
 Compliance of local traffic rules shall be ensured.
 Personnel shall not be allowed to travel in standing position.
 All personnel shall fasten their seat belt during transportation.
 In the case of transportation by open vehicle the detachable / operable panel shall be secured
properly.
 If there is no panel or flap to close the rear end of the vehicle bed or platform, a strong strap
shall be provided across the width to prevent fall.
 Vehicle used for transportation of personnel shall undergo scheduled inspections on frequent
intervals. Such inspections shall be carried out by trained personnel particularly for steering,
brakes, lights, horn, doors etc.
 Driver shall record defect or malfunctioning of any part of the vehicle in a log book kept within
the vehicle judged/ found during operation.
 It shall be ensured that personnel are not boarding or getting down the vehicle until vehicle
comes to a halt.
 Prior to start the vehicle the driver shall ensure that passengers have taken their proper
position.
12.3 TRANSPORTATION OF MATERIALS BY VEHICLES / BOATS
 All drivers/operators shall hold a valid driving license for the class of vehicle to be driven and
be registered as an authorized CIPL driver/operator with the Administration Department.

 Load shall be secured of the established and approved methods, i.e. chains with patented
tightening equipment for steel / heavy loads.
 Sharp corners on loads shall be avoided when employing ropes for securing.
 All overhangs shall be made clearly visible and restricted to acceptable limits.
 Load shall be checked before moving off and after travelling a suitable distance.
 On no account is construction site to be blocked by parked vehicles, drivers of vehicles shall
only stop or park in the areas designated.
 Warning signs shall be displayed during transportation of material.
12.4 SAFE DRIVING/ DRIVING AWARDS
Safe driving is mandatory topic to all contractor’s and sub-contractors. This is covered during their
HSE orientation program. A copy of the Traffic Safety and Defensive driving pamphlet shall be
issued to each driver. In addition to encourage safe driving incentive program shall be
implemented at site.
13.0 LEGAL AND CONTRACTUAL OBLIGATIONS
CIPL and its sub-contractors shall put maximum efforts to observe all applicable legal duties.
Additionally, each sub-contractor is responsible for ensuring that all personnel under their
control observe the legal requirements.
All employees of CIPL and its Sub-contractors are authorized to stop any operation, activity or
erection of plant, equipment etc. if they observe and unsafe act, practice or condition having
significant risk in consultation with the respective area In-charge, HSE Manager or Project
Manager.
The Sub-Contractor and all personnel under his control shall obey instruction given by an
authorized CIPL representative or Manager, HSE Engineer, HSE Officer or clients representative
in respect of occupational health & Safety and Environment.
The Sub-Contractor shall comply with each requirement or prohibition contained within
applicable laws, contract documents or HSE documentation supplied by CIPL to the Sub-
Contractor in connection with his contract or Sub-contract.
CIPL and its Sub-contractors will ensure that their HSE manuals, statutory notices, registers,
guidance notes, HSE procedures and all other HSE documents are provided, made accessible to,
and understood by their personnel. Omissions in this HSE plan will not absolve of their
legislative and contractual responsibilities.
Individuals who fail to comply with HSE practices will be subject to counseling coaching and / or
disciplinary action.
At least once in six months period during the construction period, the Project Manager and the
HSE Head / Manager, shall review the Environmental, Health and Safety Plan and where
necessary shall revise and re-issue the plan.
14.0 GENERAL HSE GUIDELINES
 Where work activities may conflict with or impact upon work activities of others, take steps
to discuss the matter with relevant parties and establish a consensus. Ensure that both
parties and their teams are fully aware of potential hazards together with appropriate
precautions.

 All plant and equipment shall be inspected before work commences and at periodic intervals
thereafter. All plant and equipment shall be safe before and during use.
 Only competent personnel are permitted to operate plant and equipment and such persons
shall be tested by a competent person and issued with an Appointment Certificate
authorizing them to operate specified plant and equipment.
 Only qualified and appointed personnel (such as riggers and scaffolders) shall do skilled
work. Such skilled workers shall always be identified.
 Method statements / risk assessments shall always examine the people, equipment,
materials and the environmental aspects of the proposed work. Such method statements /
risk assessments shall always be referred to sub-contractor / contractor HSE departments for
review and comment.
 Ensure that safety nets are installed on the periphery of the tower to prevent [possible fall
from height. Persons requiring to work near edges shall use life jackets.
 Maintain good housekeeping at all times. Provide and maintain adequate hygiene and
welfare facilities such as toilets, hand washing measures, drinking water, and canteen shelter
and food waste bin/s.
 A specific task related risk assessment shall be completed for critical work activity and the
controls shall be identified and implemented before and during work execution.
 Hazardous area should be barricaded with the attachment of appropriate warning symbols.
 Work area should be always kept clean. Unwanted scrap or tools should not be left
unattended that may be hazardous to others.
 Personal protective equipment should be used wherever required.
 All warning symbols should be well obeyed and regarded.
 It should be ensured that all-electrical cord; hoses and leads are well protected or elevated
such that there are no obstruction to stairways and walkways.
 All project emergency procedures should be thoroughly understood and obeyed.
 Only proper ladders, ramps, stairways should be used and barricaded area should never be
crossed.
 Scrap materials should be disposed of immediately as these create fire and accident hazards.
