Greetings, Ma'am/Sir

I would like to submit a formal written request for a copy of my junior high diploma. In
the year 2017-2018, I finished my junior high education at Commissioner Luis R. Asia
National High School.

The original copy of my junior high diploma I gave to this school to continue studying
grade 12, but unfortunately I did not finish my grade 12. Because I had to work for my
family.

I hope you can help me with this request. I appreciate you taking the time and effort to
do this for me.
