AFFIDAVIT OF INSERTION

I, GERARDO O. MENDOZA, of legal age, single, Filipino and resident of 2602 A

BSA Tower, Mandaluyong City after having been duly sworn to in accordance with law,
depose and say:

1. That I am the BUYER in a Deed of Absolute Sale executed on October 4, 2021

before Notary Public Atty. Rosalinda A. Montenegro entered in her notarial
register as Doc. No. 132; Page No. 22 ; Book No. X; Series of 2021;

2. That however, out of excusable negligence and clerical error the word “of legal
age” was inadvertently omitted on the said document pertaining to my identity;

3. That I am executing this affidavit to request the authorities concerned to rectify

the same and make of record the word of legal age to the said document;

4. This is being executed further for all intents this may be of legal worth;
