Day 1
6:30 – 7:00 Travel time ( from XU to DTRC)
7: 00 – 7:30 Preparation of the activity area
7:30 – 8:00 Pre-conference with CI
8:00 – 10:00 Courtesy call/ Orientation and physical tour of DTRC
10:00 – 10: 30 Assigning of students to clients who are for student interaction
10:30 – 11:30 Chart Reading
11:30 – 12:00 Short conference to choose client for CP
12: 00 – 12:30 Lunch break
12: 30 – 1pm Preparation of activity area ( arrange chairs, tarpaulin , decoration, audio / sound system)
1:00 – 1:15 Cell out clients and assist in ADL’s
1:15 – 1: 20 Usher to activity area and make client comfortable
1: 20 – 1: 30 Build rapport and take vitals signs
1: 30- 1: 45 Program proper ( Prayer, National Anthem, Intro of EMCEE’s, setting mode of the program)
1: 45 – 2:00 Opening remarks, Kamustahan / introduce self / contract setting / introduce theme of the day
2:00 – 2:10 Exercise to be facilitated by SN’s
2: 10 – 2:50 Socialization / therapy proper
2:50 – 3:00 Processing of clients R/T the therapy for the day
3:00 – 3:30 Snack time
3:30- 3:45 NPI / mental status exam
3:45 – 4:00 Wrap up, closing remarks , prayer, cell in
4:00- 4:15 After care of the activity area
4:15 -5:00 Travel time back to XU
5:00 – 6:30 Post conference, feedback R/T the activities of the day, submission of Requirements, planning
for Day 2 ( therapy, THEME for Day2 , EMCEE, decoration, snacks, exercise, logistics)

Day 2

7:00 – 7:40 Preparation of activity area ( Physical Set- up)
7: 40 – 8:00 Preconference
8:00 – 8:15 Cell out and assist client’s in ADL’s

8:25 – 8:30 Preparation for the program / activity of the day
8:30 -8:45 Start of program of activities ( Prayer, National anthem )
8:45- 8: 50 Kamustahan / introduction of EMCEE’s

Duty Requirements to be submitted thru online portal:

Note: Pplease check if all of these are submitted because we will be providing a certificate of
complication for this group of students.

Committee Students Task

1. Program and Exercise > develop steps appropriate for client’s age.
> choose appropriate music for the exercise.
> assign 2 students who will be the EMCEE for the day.

> Plan out and write on the Manila Paper or cartolina the
structured activity.

2. Decoration and physical Set Up> Make decorations for the activity of the with consideration of the THEME.
> Arrange chairs /tables/extension wires /tarpauline/ sound system.

3. Food comm > prepare snacks for the patients.


> bring jug of water/plastic cups/ plastic spoon for patients use.
> bring trash bags

4. Therapy Group > plan out and execute the therapy with clear mechanics on how
to go over on the said therapy for the day
> assign student Nurses who will explain the therapy
and process the client's.
