ENGAGEMENT TO SERVE

I, Myla D Gocotano, Filipino,(22) years of age and with residence at Purok Bird of
Paradise, La Suerte, Matanao, Davao del Sur for and, in consideration of my scholarship in
the PA Advance ROTC and the training thereof, do hereby bind myself and agree:
1. To strive to excel in all circular subject and remain as cadet in the PA Advance
ROTC Program until I shall have fully passed all the prescribed course of instruction
required for commission in the Reserve Force, Philippine Army unless disqualified or
separated by competent authority;
2. To render fifteen (15) days cumulative reciprocal duty for the cash incentive grant
I receive in a given semester as manpower augmented to assist in any reservist-
related training/activity, participants in any reservist training/seminar, filler NCO’s in
the Ready Reserve Units, or in any reservist activities as may be prescribed by the
Philippine Army;
3. To willingly accept the outright termination of my incentive and bar me from
pursuing further my training in the PA Advance ROTC Program in the event of any
breach on my part in the fulfillment of the above-mentioned obligations; as well as
violation Para p, Sec 6 (Policies and Guidelines) of SOP Nr 3 (Incentive
Allowance for Advance ROTC Cadets);
4. To immediately respond to fulfill my military obligations anytime when called to
active duty.
5. That I will pursue my advance ROTC and POTC courses and subsequently work
for my commission into the Reserve Force, AFP or as regular officer if qualified;
6. That upon my commissioned as military officer and my eventual call to
active duty in the military services I am willing to serve the Philippine Army for
a minimum of one (1) term as prescribed in para 4a of HPA Circular 1, dated 03
April 1987;
7. That I accept this incentive allowance fully aware that I shall enter the service
after concluding my studies and should I fail to finish the same I will refund all the
expenses incurred by the government for my incentive allowance; and
8. It is understood that the consent granted by my parents and/or guardians
appearing below and duly signed shall be in force and effect from paragraph 1 to 4.
(This refers to minors).

hereby declare or affirm:

1. That I shall be responsible for preserving the security of all classified

information which may be imparted to me as a result of my acceptance into the PA Advance
ROTC Program and undertake to comply with such regulation concerning security as may
from time be laid down;

2. That I shall not divulge any classified information gained by me as a result of

my training to any unauthorized person, either in verbal or written form;

3. That I shall not, without authority of the Chief of Staff, Armed Forces of the
Philippines, now or in the future, publish any classified information which I have acquired or
which I have had access owing to my training in the UM DIGOS COLEGE ROTCU Reserve
Officer Training Corps Unit whether orally or in any document, articles, book, play, film or
otherwise;

4. That on leaving the ROTC Cadet Corps, I shall surrender any sketch, plan,
model, articles, note, security tag/badge or document made or acquired by me in course of
my official duties, declared to be classified material/information, save such as I have been
authorized to retain;

5. That failure on my part of safeguards classified information can make me

liable to lose my security clearance, to prosecution under the Revised Penal Code and
Presidential Decree 51, be declared as a security risk and dismissal/cancellation from the
PA Advance ROTC Program.

I hereby certify that I have read and fully understood AFPR G 200-013, and Memo
Circular Number 78, HPA dtD14 August 1964 and I am aware of my responsibility for
safeguarding Armed Forces classified information.

I fully understood that the provisions of this Security Declaration apply not only during
the period of my PA Advance ROTC Training, but also after my commission as Officer in
Reserve Force and possible Call to Active Duty in the Regular Forces of the AFP shall have
ceased, and that I am liable to prosecution under the Revised Administrative Code,
Presidential Decree 51 and other existing laws regulations of the Republic of the Philippines,
if either by intent or negligence, I allow classified information to pass into unauthorized
hands.
