Greetings!

We would like to endorse for your approval of the following items:

 Rewinding of 2 unit 5HP feed pump at STP having the amount of PHP 9,000.00 (VAT
INC)

Justification:
The said pump is used for feeding sewage water going to SBR Tank for treatment is
malfunctioning due to foreign debris making its way through our fine screen and being
sucked by the pump and, checkup of our maintenance team; the pump needs to undergo
rewinding repair immediately since there will be no back up pump if anything goes wrong
with our current pump.

 Emergency de-sludging of our Cluster 4&6 Holding Tank also having the amount of PHP
9,000.00 (VAT INC).

Justification:
For our Cluster 4&6 Holding Tank, we needed an emergency de-sludging from a 3 rd Party
Contractor due to our Tank is at almost overflow level also from a clogged ejector pump
going to STP because of foreign materials being flushed down the toilet by unit owners at
Cluster 4&6.
