INTRODUCTION
Title: Finger caught in between Sling and Eye Pad
Location: On-land Drilling Location
Loss/ Outcome: Loss of Middle Finger.
BRIEF OF INCIDENT
During Inter Location Movement of a workover rig at night hours, crew was lifting Shale Shaker
Tank. One crane on each side of the tank was deployed. Two slings were placed on one side
of the tank and another two were being placed on the other side of the crane by two floor man.
The waste pit side was very slushy as mud was thrown out recently and there was no proper
lighting on that side. The injured person was holding the sling to stop it from coming out from
the lifting eye pad. Crane operator lifted the load on instruction of supervisor who was not in
line of sight of Injured Person (IP) and the finger of the IP was caught in between lifting eye pad
and sling. IP shouted loudly and crane operator released the load immediately.
He was taken to medic room and first aid was provided. After that he was taken to the nearby
hospital, where his middle finger of right hand was amputated.
OBSERVATIONS
 Accident site was disturbed and the tank was shifted to the new location.
 The sling used for the lifting of shale shaker tank were in very bad condition and twisted.
 The IP was holding the sling after putting the sling into lifting pad to avoid sling coming out
from lifting pad.
 There was no proper lighting arrangement where the Injured Person (IP)was working.
 IP was not visible to crane operator because he was not in line of sight and the supervisor
was standing between the two cranes from where also IP was not visible.
 There was no solid place to stand for putting the sling due to mud spilled all around and no
place to run for escape.
 There was lack of awareness about the Standard Operating Procedure (SOP) among the
crew.
ROOT CAUSES
 Non adherence of SOP: Loading and transportation of heavy material at night was done in
poor lighting.
 Improper housekeeping: Due to spillage of mud there was no place to stand, to put sling
into lifting pad and to escape in case of any emergency.

 Improper supervision: Crane operator lifted the load on the instruction of supervisor who
was himself not in line of sight of IP.
RECOMMENDATIONS
 Proper supervision should be done in giving signal to Crane Operator after making sure that
all workers are safe and in ready position.
 If workers putting sling on lifting pad are not in line of sight to Crane Operator, adequate
number of persons should be deployed to give signals.
 Lifting slings should be inspected regularly and inspection reports should be available at
site. Refer: OISD-STD-187, clause: 8.0 & 8.5
 Loading and unloading of heavy items should be avoided during night hours.
 There should be proper housekeeping of the drill site for safe movement of personnel.
 Proper lighting arrangements should be made for operation in night. Refer: OISD-GDN-218,
clause: 3.5(e).
