TWO DISINTERESTED PERSONS

We, JESSICA ENRIQUEZ and NADIA ESTEBAN, both of legal age, Filipinos, and
residents of Barangay Orange, Tabanlo, Mariano, Philippines, respectively, after having
been duly sworn to in accordance with law hereby depose and say, THAT –

1. We have personally known the person of DIOSDADO MACAPAGAL for a

long period of time;

2. We know for a fact that the person by the name of DIOSDADO

MACAPAGAL, appearing in some documents and records and the name DIVINO
MACAPAGAL, as appearing in her Certificate of Live Birth, refer to one and the same
person and that her true and correct name is DIOSDADO MACAPAGAL;

3. As such, we execute this Affidavit to attest to the truth of the foregoing

facts and for whatever legal purpose that this Affidavit may serve.
