AFFIDAVIT OF ACKNOWLEDGEMENT OF FATHER

I, Armando G. Pacay, of legal age, Filipino, and a resident of Tabanlo, Mariano, Philippines,
after having been duly sworn to in accordance with law, hereby depose and state:

That I am the father of Ailyn Pacay who was born on June 12, 1994 at the Tabanlo, Mariano,
Philippines.

That I hereby affirm and attest to the truth of the fact that the I am the father of the above-
mentioned child.

That I also declare that under the laws of the Philippines, I and Geraldyn Arellano were not
incapacitated to marry each other at the time of the conception or birth of the said child, and that
on July 21, 1997, we got married .
