AFFIDAVIT OF UNDERTAKING

I, Juana M. Rama, of legal age, single, Filipino Citizen and with residence at Irisan, Baguio City,
after having been duly sworn in accordance with the law, do hereby state:

That we are the friends of Hazel Witch and I personally know her likewise of legal age, single.
Filipino citizen and with residence and postal address at Brgy. San Luis, Baguio City and applicant as
teller with Rural Bank branch has not been involved in any irregularity so as to tarnish her character.

That in case Hazel Witch would be employed as a teller of Rural Bank Branch. We guarantee
her honesty and faithfulness in the performance of her obligations.

For this reason we undertake to answer of whatever obligation/liability that may arise from
any and all of her malfeasance/misfeasance.

That we are executing this affidavit to attest to the truth of all the aforementioned facts and
to support the application for the work of Hazel Witch as teller with Rural Bank branch as well as for
all legal intents and purpose it may serve.
