ON-THE-JOB TRAINING AGREEMENT

This agreement shall be entered into by the student(s) and his/her guardian, and shall be
approved by the OJT Coordinator, OJT Adviser, and the Dean/Director.

Terms of Agreement

(1) That the student agrees to follow company rules and regulations, maintain discipline and good
conduct, and hold in strict confidence all information to be gained during the course of the training
period.
(2) That the student also understands that the cooperating company may or may not give
compensation or allowances during the course of the training period.
(3) That the student can start/undergo on-the-job training during semester break or summer prior to
compulsory enrolment upon completion of company requirements and confirmation of
acceptance/accommodation from the cooperating company.
(4) That the training hours certified by the cooperating company shall only be credited upon
enrolment or confirmation of enrolment.
(5) That the certified training hours given by the cooperating company shall be nullified and will not
given credit if the student will not enroll the OJT subject as prescribed by the university.
(6) That the student shall comply with all the requirements prescribed by the school, the OJT
instructor and the cooperating company.
(7) That the student shall hold in confidence all information, formula or trade secret and the like of the
company that he/ she may acquire by virtue of his/ her function.
(8) That in case of non-fulfillment of the above-mentioned requirements, the Office of the OJT
Coordinators of this university shall have the right to reject the student’s training and ‘drop’ the
student from the roll during the training period and refuse acceptance of the student’s training
certificate.
(9) That the student shall be certified to be physically fit to work, without any underlying medical
condition, otherwise he/she shall submit medical clearance from a reputable hospital/clinic to the
OJT Office and to the cooperating company.
(10)Furthermore, the student waives his/her right to any claims for injury or accident that may be
sustained during the training period thereby keeping the school and company free from any
liability or claim.
