Revised 2017).

d. Photocopy of scholastic academic record , such as but not limited to

Transcript of Records (TOR) .

e. Certification from the School of General Weighted Average (GWA), If none ,

the applicant must present a computation of GWA.

e.1 For Education Graduates - GWA of Baccalaureate degree

e.2 For Non-Education Graduates – GWA of baccalaureate
degree and the eighteen (18) professional units in education.

f. Copies of Service Records or Certificate of Employment.

g. Updated PRC License ( authenticated / certified by PRC)

h. Photocopy of PRC Ratings, Passing and Good Standing

i. Certificates of Seminars / Training attended after the baccalaureate

degree

j. Certificate of Specialized Training , If any.

k. Certified copy of Voter’s ID and /or any proof of residency as deemed

acceptable by the District Screening Committee.
