JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, DARIO A. ANIÑON, and ROSALIO J. ANAGON, of legal ages, both Filipinos
and residents of Carmen, Bohol, Philippines, after having been duly sworn to in accordance with
law, do hereby depose and say:

1. That we personally know the person of ELVIRA CELLERO BERNANISA, for we are
friends thereof;

2. That as per her Member Data Record issued by the Philippine Health Insurance
Corporation her true and correct name is spelled as ELVIRA CELLERO
BERNANISA;

3. That the name of ELVIRA CELLERO BERNANISA was erroneously spelled as

ELVY C. SELLERO as per her Birth Certificate issued by the Local Civil Registry of
Dauis, Bohol due to inadvertence and mistake;

4. That the name ELVIRA CELLERO BERNANISA as per her Member Data Record
issued by the Philippine Health Insurance Corporation and ELVY C. SELLERO as per
her Birth Certificate issued by the Local Civil Registry of Dauis, Bohol, refers to one and
the same person living and existing in our community;

5. That we are not related to the said persons either by affinity or consanguinity and that we
have no interest in whatever outcome this presents may bring;

6. That we execute this affidavit voluntarily to attest to the truth of the foregoing statements
for whatever legal purpose this may serve.
