JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, EUTROPIA B. MASLOG, and ROWENA A. DALAY, of legal ages, both

Filipinos and residents of Carmen, Bohol, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and say:

1. That we personally know the person of JESSA LOU ORAPA MACHETE, who was
born on DECEMBER 15, 1988;

2. That as per JESSA LOU ORAPA MACHETE’s Certificate of Baptism issued by the St.
Anthony the Abbot Parish, Diocese of Talibon, Carmen, Bohol her true and correct name
is spelled as JESSA LOU ORAPA MACHETE;

3. That the name of JESSA LOU ORAPA MACHETE was erroneously spelled as JESSA
LORE O. MACHETE on her Confirmation Certificate issued by St. Anthony the Abbot
Parish, Diocese of Talibon, Carmen, Bohol due to inadvertence and mistake;

4. That the name JESSA LOU ORAPA MACHETE as per her Certificate of Baptism
issued by the St. Anthony the Abbot Parish, Diocese of Talibon, Carmen, Bohol and
JESSA LORE O. MACHETE as per her Confirmation Certificate issued by St. Anthony
the Abbot Parish, Diocese of Talibon, Carmen, Bohol, refers to one and the same person
living and existing in our community;

5. That we are not related to the said persons either by affinity or consanguinity and that we
have no interest in whatever outcome this presents may bring;

6. That we execute this affidavit voluntarily to attest to the truth of the foregoing statements
for whatever legal purpose this may serve.
