JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, VICTORIANO M. ADLAON, and JEAIMEE B. DUMAYAC, of legal ages, both

Filipinos and residents of Carmen, Bohol, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and say:

1. That we personally know the person of EVANGELINE ANAGON EDUSMA, for we

are friends thereof;

2. That as per EVANGELINE ANAGON EDUSMA’s Certificate of Live Birth issued by

the Philippine Statistics Authority (PSA), Bohol her true and correct name is spelled as
EVANGELINE ANAGON EDUSMA;

3. That as per Certificate of No Marriage issued by the Philippine Statistics Authority

(PSA), EVANGELINE ANAGON EDUSMA’s name was spelled as EVANGELINE
MARANGGA ANAGON due to mistake and inadvertence;

4. That the name EVANGELINE ANAGON EDUSMA as per her Certificate of Live
Birth issued by the Philippine Statistics Authority (PSA) and EVANGELINE
MARANGGA ANAGON as per the Certificate of No Marriage issued by the Philippine
Statistics Authority (PSA), refers to one and the same person living and existing in our
community;

5. That we are not related to the said persons either by affinity or consanguinity and that we
have no interest in whatever outcome this presents may bring;

6. That we execute this affidavit voluntarily to attest to the truth of the foregoing statements
for whatever legal purpose this may serve.
