CONSULTATION REPORT

CONSULTING PHYSICIAN: Lena Kushner, MD

REASON FOR CONSULTATION: Rosacea.

HISTORY OF PRESENT ILLNESS: This patient was referred to me by Doctor Donnie Dukes
for evaluation and recommendations regarding the patient rotation.

Mr. Sines was a a 62-year-old gentleman who presents with complaints of validation on his face.
He states that he had tried topical gel (Metrogel 1% ) but have not believed this has alleviated his
rosacea. He states he had laser treatments for his rotations 3x at the university and 2x at Metro
Hospital, most recently 6 months ago, and several other instances at other institutions. He
presents daily foliation for further rosacea treatment.

Past medical history includes status post thyroidectomy and status post blefaroplastia at the
university med. He has a history of skin cancer and a status post fixational Veneto carcinoma of
the right upper extremity approximately 9 months ago. He also has a history of tinnitus
hypothyroid and cardiovascular disease. Status post 4 retro kalash 2 years ago, status post
angioplasty, status post angiogram, hetero seizures, hetero Chol cholesterol, hearing loss with
tinnitus, status post female imbue 3x, status post back surgery 2x, the most recent laminectomy,
this year, status post laminectomy of the child, status post left ankle surgery. He has no history of
diabetes or heart retention.

FAMILY HISTORY: Significant for diabetes.

SOCIAL HISTORY: He is not married. He doesn't smoke, drink alcohol, or use illicit drugs.

MEDICATIONS: Synthroid 145 mg q.d., Crafter 10 mg q.d., Tegretol 100 mg PO b.i.d., Aspirin
85 mg q.d., and Metrogel 1% topical, which was stopped a few weeks ago.

ALLERGIES: IVP dye and the history of environmental allergies.

PHYSICAL EXAMINATION: The patient is a pleasant 63-year-old gentleman, his face is


symmetric, he has a well healed iridectomy inside along with the ulterior ocular region. The
patient has same fain tear as of ravage along the lip, nasal tip left medial cheek and right cheek
along the angle of the mandible, the fibrous are not significantly inflamed nor are they
apparentexcept on close inspection. He does have evidence of lower laded lapel surrounding of
his lateral cat eye.

IMPRESSION: Mr. Sines verbal consultation note the significant cosmetic defector at this point,
he will be recommended that he continue the Metrogel 1% topical. However, he states that he
will not use this. We discuss with him that we will investigate further treatment and will let him
know if we have further treatments of this and discuss a proper execution.
