solemnly affirm and declare on oath as under:

1. That we all permanent residents of J&K State.

2. That we are partners of firm M/S BHAT SALES CORPORATION

With its Principal place of business at Baghat-e-
Barzulla, Srinagar-190008.

3. That we are not working in any Govt. or Semi Govt.

Department nor are we on the rolls of any educational
Institution.

4. That whatever has been stated in form VAT 10 and form A

are true and correct to the best of our knowledge and
belief. Nothing in it is concealed.

5. That we are maintaining account with The Jammu and

Kashmir Bank Ltd. B/O Bagh-e-Mehtab, Srinagar, Kashmir
under A/C No. CC/32.

6. That we are not already registered with Sales Tax

Department nor are we partners of any other firm
registered with the Sales Tax Deptt.

7. That the premises are acquired on rental basis.
