UNDERTAKING CUM AFFIDAVIT

I, Ab Majeed Gojree father of Ashfaque Majeed Gojree R/o: Machipora Handwara Tehsil: Handwara Distt: Kupwara Date
of Birth 20-03-2000 do hereby solemnly affirm, declare and undertake as follows:

1. I have enrolled my son/daughter Mr/Ms Ashfaque Majeed Gojree with M/s. Centre for Social Responsibility &
Leadership (hereinafter referred to as the Centre) under project KASHMIR SUPER-30, for coaching of IIT/JEE Mains &
Advance& State Engineering Examination 2018.
2. That I have understood the complete rules and regulations of the Centre and have enrolled my child out of my own
free will & that my child would not enroll with any other private or Government sponsored coaching.
3. That I would provide to the Centre the complete details (photocopy of the form/ admit card) of the various forms filled
for all engineering examinations.
4. I shall be responsible for the conduct and behavior of my son/ward throughout the period of his stay in the
accommodation provided by the Centre for undergoing the course coaching. That Centre reserves the right to
remove/expel my ward/son on the grounds of discipline, insubordination and non performance in academics.
5. That in the event of expulsion of my ward/son on any of the grounds as mentioned above from the course I would be
unconditionally liable to pay to the Centre Rs. 1000/- per day from the date of starting of the course up to the date of
such expulsion.
6. I understand that my son is being given free coaching including free boarding and lodging facility on the solemn
assurance given by me on behalf of my son/ward that he shall sincerely and diligently pursue and complete the course in
the format laid down by the Centre therefore, in the event, My son/ward leaves the course in the middle or otherwise
without completing the same in the manner planned by the Centre then I would be unconditionally liable to pay to the
Centre Rs 1000/- per day from the date of starting of the course up to the date on which my son/ward leaves the
course. I undertake and agree to pay the same without any challenge or dispute.
7. That in case any other institution claims the name of my child to have been associated with that organization for
successful clearance then I would pay the complete cost spent by the Centre on my child towards course, lodging and
boarding , food during the entire coaching period at rate of Rs. 1000/- per day commencing from starting of the course.
