Male nurse posted at NTPH Ashpora do hereby solemnly affirm and declare on oath
as under:

1. That I am working as Male Nurse in Health department and I am presently posted at

NTPH Ahspora under the control of local Health Authority Block Langate.

2. That unfortunately I have used harsh language with my officer (M.O).

3. That I will not commit such Act. next time.
4. That I will obey the rules and regulations of the department and my said officer.
5. That if I will commit such blunder at next time then deponent shall be liable for any
punishment and serious action of the department.
