AFFIDAVIT OF LOSS
I, FREDO GALLO TAMDANG, of legal age, married, Filipino and a
resident of Brgy. New Poblacion, Buenavista, Guimaras, Philippines, after having
been duly sworn to in accordance with law, hereby voluntarily depose and state;

1. I am one of the legal heirs of the late DIONESIO C. TAMDANG married

to Candida Gallo;

2. That during the lifetime of DIONESIO C. TAMDANG he is the true

owner of a parcel of land situated in Brgy. New Poblacion, Buenavista,
Guimaras and covered by the Original Certificate of Title No. EP-3075 of
the Registry of Deeds for the Province of Iloilo, issued on September 30,
1974, described and bounded, as follows;

Bounde on the NE., along line 4-5 by Lot 3259; on the SE., along line 5-6 by Lot 413-W;
on the SW., along line 6-1 by Lot 413-E; along lines 1-2-3 by Lot 413-C; and on the NW.,
along line 3-4 by Lot 413-A all of subdivision plan. Beginning at a point marked “1” on
plan, being S., 35 deg. 54’ W., 520.67 m. from BLM No. 1-A Pls-723-D, Buenavista Cad.,
Municipality of Buenavista, Sub-Province of Guimaras, Island of Guimaras.

Thence N., 70 deg. 46’ E., 14.09 m. to point 2;

Thence N., 46 deg. 48’ W., 45.12 m. to point 3;
Thence N., 41 deg. 48’ E., 27.00 m. to point 4;
Thence S., 48 deg. 32’ E., 62.32 m. to point 5;
Thence S., 53 deg. 56’ W., 46.50 m. to point 6;
Thence N., 30 deg. 05’W., 15.00 m. to the point of beginning;

Containing an area of ONE THOUSAND EIGHT HUNDRED FOURTY NINE

(1,849) square meters, more or less.

3. That my father used to keep the Original Certificate of Title in our

residence;

4. That all diligent efforts I exerted to locate and retrieve it were vain; hence
for all legal intents and purposes it is now deemed lost beyond my
recovery;

5. I certify and confirm that the parcel of land covered by Original Certificate
of Title No. EP-3075 of the Registry of Deeds for the Province of Iloilo was
never mortgaged, sold, conveyed, disposed of or encumbered, to any other
person or entity.
 6. Furthermore, I certify and confirm that said document was neither
confiscated nor relinquished in favor of, any government and non-
government agency, or authority;

7. That I shall assume any responsibility or liability if in any event the said
Original Certificate of Title is used with or without my knowledge for
whatever illicit transactions;

8. That I attest to the truth of the foregoing statements to the best of my own
personal knowledge and based on authentic documents or records;

9. I am executing this Affidavit of Loss to support my application for a new

owner’s duplicate copy of Certificate of Title No. EP-3075 as well as for all
lawful intents and purposes it may serve.
