Why the client /family seeking services, and what are the client’s current concerns relative to
engaging in occupations and in daily life activities?
The father of the patient said that he admitted his son to the psychiatric hospital because he does a lot of
wrong behaviors such as taking drugs, breaking the house, television and light bulbs, defecates and
urinates on the bed and the house floor disturbs the neighbors and jumps over the neighbors’ houses. They
also explained that the reason is to get rid of his bad behaviors. He has also become socially isolated.

2. In what occupations does the client feel successful, and what barriers are affecting their success in
desired occupations?
The patient succeeded in returning to urinate in the toilet, shave and shower. He succeeded in curing the
drug. He succeeded in reorganizing the time and commitment to treatment and medication. Get rid of
social isolation and become involved in social activities such as a football match.

3. What is the client’s occupational history (i.e., life experiences)?

Age 42, single, has 3 siblings, studied the specialty of a cook at the university, and worked in
several jobs, including a cook in a restaurant, a barman, a cleaner in a restaurant, a cleaner in a
hotel, and he traveled to America in 1997-1998, and after returning for a while, his parents
noticed the change, as he did not eat or sleep well and loves solitude.

4. What are the client’s values and interests?

cooking, camping and going out with family and friends.

over time?

The patient was doing occupations normally before he had an accident and fell on his head, and
then he was diagnosed with schizophrenia after his behavior differed.
