CRIMINAL COMPLAINT AFFIDAVIT

March 12, 2015

I, Nor Hussien Radiamoda, the undersigned, 22 years of age, single, call center
agent at Mabuhay Call center and a resident of 237 Maliwanag St. Brgy. Madilim,
Quezon City, after having sworn to in accordance of the law, do hereby and state:

1. At around 1am of Tuesday, January 18, 2022. I, Nor hussien Radiamoda,

Together with Jenny Kidit, 23 years old, single, call center agent at Mabuhay
call center and a resident of 237 Maliwanag St. Brgy. Madilim, Quezon City
was about to go home after their duty.

2. While we’re waiting for a taxi along the highway of EDSA. I was holding
my phone on my right hand and wallet on the left arm, walking in a
moderate pace towards the loading area in Caluza St.
 3. That at Dims Street, before I cross Caluza St., a man riding a motorcycle
stopped in front of us, holding a knife which was pointed at me said, “akin
na ang cellphone at wallet mo kung ayaw mong masaktan”.

4. That I resisted and tried to run away, but he hit me with his motorcycle.
After getting hit, he immediately took my cellphone and wallet and escaped
towards the Malabo St.

5. That after he ran away, I screamed loudly the words “magnanakaw! Kinuha
mga gamit ko!”.

6. That Jenny Kidit noticed the robber’s ID fell in the street and she took it
immediately.

7. That the ID’s owner is Henry Montero, 25 years old, married, and a resident
at Masama St. Brgy. Maliwanag, Quezon City.

8. Due to the said incident, I suffered injuries and the following are:

(a)  Dislocated shoulder ball joint, requires treatment for 20 days.

(b) Crushed Kneecaps, requires surgery and replacement, and rest for 60
days.

9. I am executing this complaint-affidavit in order to file a case of Robbery with

Physical Injuries against Henry Montero.

10. To attest to the truthfulness of the foregoing, I, Nora Radiamoda, complainant

attaches my signature below.
