1. General
This requisition is prepared for the following equipment and/or material required for KARUN
PETROCHEMICAL HYCO PLANT and vendor shall develop their technical quotation in strict
accordance with conditions stated here and any attached documents. Exceptions, deviations and
alternatives will be valid only by writing approval of Purchaser.
1.1 Definitions:

Throughout this Material Requsition, the words "will", "may", "should” and "shall" when used,
have meaning as follows:
"will" is used normally in conjunction with a request from CONTRACTOR;

"may" is used where alternatives are equally acceptable;
"shall” is used where a provision is mandatory;
"should" is used where a solution is preferred
4. Scope of Supply and Works
Vendor’s Scope of supply and works shall be in accordance to Attachment #1 of this Material
Requisition. Exceptions, deviations and alternatives will be valid only by written approval of
Purchaser.
For general terms and conditions of material/equipment purchasing (e.g. delivery considtion,…)
reference should be made to Purchaser’s inquiry package.

5.2 In case of any discrepancy, the order of priority of documents is per the following:
(1) Purchaser’s / Contractor’s approved Clarifications, MOMs and “Exception, Deviation &
Alternative List”
(2) Data Sheets / Drawings (if any)
(3) Technical Specifications / Purchaser’s documents attached to MR
(4) This Material Requisition
(5) International Code & Standards referred to in the Specification and attachments

If Vendor discovers any inconsistency or conflict within the documents comprising this
Requisition which could not be resolved after considering above mentioned precedence, the
most stringent requirements shall govern. Any ambiguity or conflict shall be referred to the
Purchaser and written approved solution shall be applied.
5.3 During proposal stage, bidder shall verify all information and documentation provided by
Purchaser/Owner. Should any information or documentation be either insufficient or inacurate,
bidder shall notify the Purchaser / Owner promptly. Any changes arisen from the failure in
verification by the vendor shall not constitute a variation order in the Vendor’s scope of work or
supply.
5.4 Vendor shall sign and stamp all pages of this Material Requisition document and all pages
of attachments and to return with their technical proposal.
6. Deviations From Purchaser’s Requirements
Vendor's technical proposal shall be in strict conformity with Purchaser/Owner's requirements.

Other than those exceptions, deviations or alternatives which are clearly identified or listed in
Vendor's technical offer as per format of "EXCEPTION, DEVIATION and ALTERNATIVE
LIST" (Attachment #3), the requirements and conditions in this Material Requisition shall be
deemed as accepted by Vendor.
Deviation, exceptions and alternatives shall have written approval of the Purchaser / Owner.
If Vendor has no deviation or exception, "NO DEVIATION" shall be clearly stated in above
mentioned list and to be signed, stamped and submitted with technical offer. If Vendor is
awarded the order, Vendor shall comply with the applicable documents as revised only to
reflect the deviation and alternatives listed and accepted by Purchaser/Owner.

Purchaser/Owner has no responsibility to discover any deviations entered in Vendor’s proposal

other than the listed deviations. Those deviations which are not listed in the consolidated
deviation list will be disregarded.
7. Documents Required During Bid And After Purchase Order
For minimum requirements of Vendor technical documents during bid stage and after purchase
order placement, refer to attachments #2 of this Material Requisition.
Vendor shall note that only those vendor documents with Purpose of issue (POI) for Review (R)
will be subject to Purchaser’s review. Such documents will be considered as the basis of
equipment and/or material supply and delivery to Purchaser. Documents with POI for
Information (I) shall be in full compliance with “for Review” (R) documents. Any conflict,
error or discrepancy in “For Information” (I) documents is Vendor’s liability and shall be
rectified without any charges.
8. Inspection And Tests
Inspection and tests shall be performed by Vendor in accordance with:

The Purchaser and Third Party Inspector reserve the right to inspect and independently test
materials in Vendor's and sub-Vendor's premises.
9. Gurantee and Warranty
For Gurantee and Warranty Terms and Conditions, reference should be made to project’s
General Terms and Conditions of Purchasing.
All equipment and component parts shall be guaranteed by the vendor against defected
materials, design and workmanship for 18 months after start-up or 24 months after delivery of
the last consignment of equipment, whichever occur first. Warranty period shall be at least 5
years.
10. KOM and Pre-Inspection Meeting
10.1. Kick-Off Meeting and/or technical coordination meeting
After the order is awarded, Vendor shall, upon receipt of Purchaser’s notice, dispatch sufficient
number of qualified personnel to Purchaser’s office at Vendor’s cost to hold the Kick-Off
Meeting to confirm the procedure at least for the following categories:
- Detail procedure, such as the correspondence, the document delivery plan. Purchaser’s
approval method shall be established.
- Shop drawing preparation procedure
- Fabrication
- Quality Control
- Time Schedule
- VPIS (Vendor Print Index Schedule)
- Other Technical Points

10.2. Pre-Inspection Meeting
Before starting the fabrication and inspection jobs, a pre-inspection meeting (PIM) will be held
at the Vendor’s shop.
10.3. HAZOP And SIL
VENDOR shall have attendance at HAZOP (Hazard & Operability) and SIL (Safety Integrity
Level) review meetings at the Owner / Contractor office. VENDOR shall have overall
responsibility for the proper design and safety of the equipment supplied. Any items required as
a result of the HAZOP actions are to be fully implemented by VENDOR.
VENDOR shall ensure the relevant personnel are available during the course of the HAZOP
sessions. The Owner / Contractor may be required to participate to further reviews such as
Constructability and Maintainability for the equipment to be supplied.
Vendor shall include in the package cost, the cost of his participation in the HAZOP & SIL
study which will be conducted by the OWNER/CONTRACTOR. Any change results of this
HAZOP review meeting shall be at Vendor’s care and cost.
11. Special Notes
11.1. Technical Quotation shall include the following items as a minimum:

(4) Special tools and test equipment list
(5) Completed Purchaser's Data Sheets
(6) Description of System
(7) Completed TBA Form

(8) Completed Attachment #1 (Filled designated cells)

(9) Filled, signed and stamped Exception, Deviation and Alternative list
(10) Signed and stamped pages of this Material Requisition and all pages of attachments
(11) Time schedule
(12) Origin of material being quoted
(13) Spare part list ( Capital (if any), Commissioning & 2 years)
11.2. All technical documentation and Drawings shall be in English language.

11.3. Purchaser or Client’s acceptance of Vendor’s drawings or data with or without comments
does not relieve the Vendor of the responsibility of complying with all terms, conditions,
codes, standards, requirements of Purchase Order and this requisition and project
specifications.
11.4. Purchaser/Owner reserves the right to review and comment on vendor documents and
drawings that have previously been accepted with or without comments based on project
requirement.
11.5. Proven technologies only shall be considered in the selection of equipment. Vendor shall
submit reference list with at least five years trouble-free running experience in similar duty
and environment to those specified within quotations stage.
11.6. This Requisition sets out the minimum requirements and does not relieve the Vendor of his
full responsibility for the design and reliable operation of the elements supplied. Therefore the
Vendor will be liable for the correct operation of any auxiliary elements involved.
11.7. In the preparation of their proposal, Vendor shall fulfill all the provisions of this Requisition
even if they differ from the standard supply. Whenever the Vendor cannot comply with the
present Requisition for technical reasons, it shall be stated in detail in the Exception,
Deviation and Alternative list (Attachment #3).
11.8. For satisfactory system performance and full compliance with the attached specification, all
required accessories and/or components shall be provided by Vendor whether or not
requested devices are herein specified.
11.9. Bidders may supplement their basic quotations with alternative design of significant
advantages such as improving performance, delivery date, costs, etc. The supplement shall be

provided with proper evidence/justification and shall be subjected to Purchaser’s/Owner’s

written approval prior to start of the work although the base offer shall be always in line with
the inquiry.
11.10. Any comments on drawing and/or Vendor data which will be submitted in accordance with
Vendor document approval process shall be incorporated by Vendor and shall not affect the
contracted price as far as they are in line with the requirements of this specification and/or
good industrial engineering practice.
11.11. Purchaser reserves the right to change the delivery sequence, interval and timing in order to
meet the requirements of the site erection or overall construction sequence, etc. Upon
Purchaser’s change requests of the delivery sequence, interval and timing, Vendor shall
control and expedite his work and delivery process.
11.12. No protection, paint or other filling shall be applied to the contract works before they have
been inspected, tested and approved.
11.13. Spare parts for erection, pre-commissioning, commissioning and start-up:
The erection, pre-commissioning, commissioning and start-up spare parts shall be supplied
together with the equipment (If requested as per Scope of Supply of this MR). All spare parts
shall comply with the same standard and spec. as the original equipment and shall be fully
interchangeable with the original parts.
11.14. Supply of Spare Part for two years of operation (If requested as per Scope of Supply of this
MR), shall be within Vendor’s scope of supply. All spare parts shall comply with the same
standard and spec. as the original equipment and shall be fully interchangeable with the
original parts.
11.15. Vendor shall provide utility summary within proposal stage.
11.16. OWNER and/or CONRACTOR shall have free access to all vendor and sub vendor plant,
where work on or testing of equipment is in progress.
11.17. All documents and drawings shall state the name of client (End user) ,purchaser ‘s No.
Requisition No. Item No. and name of the equipment and or materials.
11.18. Supplier shall be responsible for detail design, procurement, materials, fabrication, inspection,
testing, cleaning, and painting, preparation for shipment, delivery and documentation in
accordance with the specified requirements.

11.19. All material certificates shall be included in a certified material record report, including
chemical analysis, mechanical properties, heat and cast numbers for all pressure parts. This
report shall include a list of materials with a sketch showing the position of the component
cross-referenced to the relevant certification. Material test certificates for all pressure
containing parts and attachments to pressure containing parts shall be as per BS EN 10204
Level 3.1
11.20. All package (equipment, interconnecting piping, ...) paintings including final painting shall be
carried out at vendor shop prior to delivery to site as per project paint specification.
11.21. The package shall be preserved for shipment and long term outside storage. Inhibition
materials against internal corrosion (if required) for transportation and long term storage at
stockyard shall be provided by Vendor.
11.22. Vendor’s quotation shall be in strict conformity with conditions stated in this material
requisition and any attached documents. Exceptions, deviations and alternatives will be valid
only if approved by Purchaser. VENDOR shall confirm full compliance with the project
specification requirements except the deviation listed and Purchaser has no responsibility to
discover any more deviations or alternative in other parts of Vendor proposal than the agreed
submitted list.
11.23. Materials of construction shall be as per ASTM/ASME standard. However DIN, BS or any
other well-known material as substituted materials can be selected if they are equivalent or
superior to ASTM/ASME Ones. The material equivalent comparison table with ASTM &
certificate shall be submitted. For major components vendor shall provide details as chemical
composition & mechanical properties in case of Non-ASTM /ASME materials.
11.24. The minimum ship loose items to be considered and informed to the Purchaser.
11.25. No fabrication work is allowed without approved fabrication drawings/calculation books.
However vendor print is marked with “Approved as noted”, fabrication activity can be
performed if purchaser’s comments incorporated as indicated.
11.26. Platform and ladder layout will be provided to vendor at job stage. Vendor shall design
platform and ladders in accordance with purchaser’s standard drawings, provide fabrication
drawing and determine lug arrangement to be incorporated into vessel design lugs shall be
welded to vessels by vendor at shop, while platforms and ladders will be fabricated by others.

11.27. Unless otherwise specified on other project specifications, all structural bolt and nuts shall be
galvanized.
11.28. Unless otherwise specified on other project specifications, all structural bolt and nuts shall be
galvanized.
11.29. All bolting thread used shall be of unified system.
11.30. ll flanged opening shall be provided with bolted blinds with gasket for transportation purpose.
11.31. Sub vendor’s shall be as per owner’s AVL, However in case of Non-AVL Owner approval
shall be adopted
11.32. Owner/Purchaser has the right to reject sub-vendor, If his capability is judged poor or not
proven.
11.33. After P.O, native files of all documents/drawings which will be specified in VPIS shall be
submitted by Vendor based on Owner/Purchaser request.
12. Duty And Responsibility Of Vendor’s Supervisor
12.1. General
Vendor's supervisor, who is to be dispatched to site for installation, start-up inspection, repair
and maintenance of equipment supplied by Vendor, shall perform his duty for all equipment
including materials provided by his sub Vendor.
12.2. Qualification
Vendor's supervisor shall be sufficiently qualified to perform his duty. Vendor shall provide a
resume of expected personnel for Purchaser's / Owner’s approval. Purchaser/Owner has right to
reject the supervisor proposed by Vendor and to appoint alternative personnel.
12.3. Duty
The duty of the Supervisor shall include, but not be limited to the following:
- Witnessing of unpacking inspection of the goods

- Supervising and giving direction of assembling and installation
- Checking of the leveling and alignment

- Checking of the equipment performance

- Adjustment of safety device and instruments
- Supervising of preparation of start-up
- Supervising and giving direction of start-up operation
- Executing and/or supervising and giving direction for assembling disassembling, inspection
and repair.
- Preparing of inspection and test records and miscellaneous reports
- Advising of time schedule
- Supervising and giving technical assistance for maintenance service(s).
12.4. Responsibility
Vendor shall identify and be responsible for damages to properties and/or personnel injuries
resulted from the gross negligence of the Supervisor while performing duty. For any other
points refer to Purchase Order (PO).

1. Sub-Vendor List (based on attached Owner vendor list along with items which has not been
listed - If applicable) shall be specified in Attachment #5 which will be subjected to
Purchaser's/Owner’s Approval.
2. Vendor shall confirm and clarify suitability of above mentioned scope of supply based on
technical aspect, safe operation, his experience and industrial normal practices.
