ACKNOWLEDGEMENT RECIEPT

            Received from Ciara Maicah Rigdao of Barangay Tarvet, Tarlac City, by recipient MR. JACKSON
CHUA JR. the amount of ONE HUNDRED TWENTY FIVE THOUSAND (P 125,000.00) PESOS as partial
payment for her obligation brought about by a vehicular accident damaging the truck of recipient that
took place last June 22, 2019

The recipient acknowledges that this amount is the subsequent payment from the initial
payment made by Ciara Maicah Rigdao amounting to FOUR HUNDRED THOUSAND PESOS (P 400,
000.00) last July 16, 2021.

It is also herein acknowledged by recipient that the remaining balance to be paid by Ciara
Maicah Rigdao shall be limited to ONE HUNDRED THOUSAND PESOS (P100, 000.00) only, payable on or
before the end of October 2022, and that the recipient is waiving any right to demand any amount
exceeding said amount.
