I, POCHOLO PATRICK LUNGAY y REMOLACIO, of legal age, Filipino

and a resident of #51 Everlasting St., Brgy. Payatas A, Quezon City, after having been
sworn in accordance with law, depose and say:

1. That I was born with the registered name POCHOLO

PATRICK LUNGAY y REMOLACIO, on January 5, 1996, at
the City of Taguig of my parents Belma S. Remolacio and Pablito
C. Lungay;

2. That in my Certificate of Registration and in the records of the

University Registrar of Polytechnic University of Quezon City,
my first name was recorded or indicated as
POCHOLOPATRICK (one word; without space), due to
mistake and inadvertence when I accomplished my
registration/application form;

3. That I am the same POCHOLOPATRICK LUNGAY y

REMOLACIO indicated in the said above-mentioned record;

4. That I am executing the foregoing affidavit for the purpose of

identification and to clarify the confusion as regards my name
and in connection to my request for correction of my name
appearing in the above-mentioned records and to attest to the
truthfulness of the foregoing statements and for all legal intents
and purposes.
