JUDICIAL AFFIDAVIT

I, MICHELLE EGARGO, 29 years old, single, a nurse of the Regional


Epidemiology and Surveillance Units (RESU) of the Department of Health
– Eastern Visayas, and a resident of Tacloban City after having been sworn
to in accordance with law, execute this Judicial Affidavit to serve as my
direct testimony in above-captioned complaint against REYMOND A.
CADIZ depose as follows:

PRELIMINARY STATEMENT

That in accordance with A.M. No. 12-8-8-SC, which prescribes the


use of judicial affidavits to serve as the direct examination testimony of a
witness, on the basis of which the adverse party may conduct his cross-
examination on such a witness, I hereby execute this judicial affidavit in a
question and answer format.

That conformably with Section 3(b) of A.M. No. 12-8-8-SC or Judicial


Affidavit Rule, I state that it was ATTY. DICK GUILLER DELFIN, who
conducted the examination of the undersigned affiant at his office at Curry
Avenue Extension, Poblacion 6, Catbalogan City, Samar.

That also pursuant with Section 3(c) thereof, I hereby state under
pain of perjury that in answering the questions asked of me as appearing
herein below, I was fully conscious that I did so under oath, and that I may
face criminal liability for false testimony or perjury.

PURPOSE OF TESTIMONY

This Judicial Affidavit is being offered for the following purposes:

1. To serve as my direct testimony in the above-mentioned complaint


to corroborate the testimony of the accused REYMOND A. CADIZ
that he was swabbed on November 11, 2021 at RHU Paranas.

2. To identify and testify on relevant and material documents; and

3. To prove other relevant and pertinent matters.

QUESTIONS & ANSWERS

Q1: Do you swear to tell the truth and nothing but the truth?
A1: I do.

Q2: Are you aware that you may face criminal liability for false
testimony or perjury if you will not tell the truth?
A2: I’m definitely aware of such consequences.

Q3: Please state your complete name, age and occupation.


A3: I am Michelle Egargo, 29 years old. I am a nurse by profession
assigned at Regional Epidemiology and Surveillance Units (RESU) of the
Department of Health – Eastern Visayas.

Q-4. Do you know the respondent in this case?


A-4. Yes, sir. He is Mr. Reymond A. Cadiz.

Q-5. What is your relationship with Mr. Reymond Cadiz?


A-5. He used to be my workmate in the Department of Health –
Eastern Visayas but later on he was deployed as a COVID Nurse under the
Nurse Deployment Program (NDP) at the Rural Health Unit (RHU) of
Paranas, Samar.

Q-6. When was the last time you had a contact with Mr. Cadiz?
A-6. I met Mr. Cadiz on November 11, 2021 when our team was
tasked to conduct swab testing in Paranas, Samar and Mr. Cadiz was there
as he was one of the close contacts of a COVID-19 patient.

Q-7. What else do you want to say, if any?


A-7. That would be all sir.

2022 at Catbalogan City, affiant exhibiting to me his PRC ID issued in,
Tacloban City with number 083099 as competent evidence of identity,
known to me to be the same person who executed the foregoing
instrument and acknowledged to me that the same is his free act and deed.
