Welcome to Pine Meadow Nursing Home!

INTRODUCTION

On behalf of everyone here at Pine Meadow Nursing Home we would


like to introduce ourselves and extend a warm welcome to our
Home. In an attempt to better assist you and answer some of your
questions, we have developed this welcome package.

Pine Meadow is a 64 bed, not-for-profit nursing home that is owned


by Land O’Lakes Community Services in Northbrook and managed
with the help of consulting services from Extendicare (Canada) Inc.
We are governed by the Ministry of Health and Long Term Care,
Ministry of Labour and the Ministry of the Environment. We are an
accredited facility, having met or exceeded a number of national
standards set by the Commission on Accreditation of Rehabilitation
Facilities (CARF).

Built in 1993, the home has a park-like atmosphere, in a quiet rural


setting. Our home has been redeveloped and is now classified as a
“New Home”. Pine Meadow is proud of its caring service to the
community of Northbrook and surrounding area, extending
throughout Addington Highlands. The goal of our home is to provide
high quality resident directed care working toward the maintenance
and/or improvement of the general health and lifestyle of our
residents. We encourage the active involvement of the family in the
care of our residents.
You, the resident, are the most important person in our home. This
booklet has been prepared to help you become acquainted with the

services provided for your well being. We encourage you to share
this information with your family members.

Please familiarize yourself with the procedures that have been


established with your care, safety and comfort in mind. We would
like your stay here to be as pleasant and enriching as possible.
Should you have any questions regarding any of the information
in this booklet, please ask. We encourage you to talk to any one of
our staff at anytime, should you have questions or concerns.

ADMITTING PROCEDURE

In order to be admitted to Pine Meadow you must first submit an


application through Placement Coordination Services at the Local
Heath Integration Network (LHIN) formally known as Community
Care Access Centre (CCAC). This application gives the information
needed to assess the specific care that you require and to determine
if your needs can be met at Pine Meadow.
At the time of your admission we will need the following
documentation:

 Power of Attorney (POA), and Power of Care


 A valid Ontario Health Card Number (OHIP card). We can help
you change the address to Pine Meadow Nursing Home.
 your drug benefit number if you have private or some other type
of insurance coverage
 your Notice of Assessment (NOA) from Revenue Canada for the
current taxation year (required only if you are requesting a room
rate reduction)

ACCOMMODATIONS

We have different types of rooms at Pine Meadow. Meals, laundry


and nursing services are included, no matter which type of
accommodation you choose.

Private - A private room is 1 person in a room and a bathroom to


yourself.
Semi-Private - Semi-Private means that 2 people share a room and
one bathroom (larger square footage).

Basic - Basic means that 2 people share one room and one
bathroom.
Short Term Respite - Respite can provide short-term care. It is
designed for individuals requiring shorter periods of care to a
maximum of 30 days at one time and 90 days annually.
It is available for those individuals who may be recovering from
surgery/illness and not able to care for themselves in their own
home or it can benefit caregivers by providing them with relief from
their care giving responsibilities. The resident can enjoy all of the
benefits of the programs and services offered by our home.

After admission, if you wish to change your accommodation (i.e.


basic to private or private to basic), a written request for this change
should be given to the Administrator. Your request will then be
added to the waiting list on a priority basis.

Items that are not covered with your accommodations

 Hairdressing/barber services (fee for service)


 Services provided by Foot Care Clinics (this is care outside of
the basic foot care, which is provided by the Home)
 Installation/monthly billings associated with resident
telephone service.
 Individual newspaper subscriptions

 Dry cleaning
 Satellite television (Shaw is the Provider)
 Non-prescription drugs, dispensing fees, prescription fees for
medication and treatment products/supplies not available

through Ontario Government Pharmaceutical and Medical


Supplies. All prescribed drugs and/or drug products must be
received from the pharmacy retained by the Home.
 Tobacco/liquor
 Dental work/dental hygiene/denture services are available (fee
for service). To arrange an appointment, please contact the
charge nurse.
 Optometry services/eye glasses and hearing devices (fee for
service)
 Personal incidental items such as clothing
 Travel/transportation to medical appointments/hospital
 Escort service (PSW to assist for appointments)

FINANCIAL ARRANGEMENTS

On the day of admission, or shortly thereafter, you will be asked to


sign an admission agreement accepting these billing conditions. A
copy will be returned to you once it is signed. Annually, you will be
asked to review your contract and sign a letter verifying that you
have done so. This is required by law. If you have any questions or
concerns, please contact the Administrator.
You or your guarantor will be responsible for the resident’s portion
of your nursing home care. Room rates are set by the Ministry of
Health Long Term Care and are the same for all nursing homes in
Ontario.

Monthly room charges are payable in advance at the beginning of


each month. If you wish, charges for foot care, escort to
appointments or hairdressing can also be billed on your statement
at the end of each month. An invoice will be mailed to you monthly
by Extendicare and payments are made payable to Pine Meadow
Nursing Home. Pre Authorized payment is recommended.

Where a long-stay resident of a long-term care home has accessed


all sources of income to maximize his or her annual net income,
the resident may apply for a reduced amount payable by the

resident for basic accommodation. In order to obtain a rate
reduction, an application form must be completed. The business
office will assist you in completing the application form.

Please note that the reduced rate application cannot be completed


until you submit a copy of your Notice of Assessment issued
under the Income Tax Act (Canada) for the resident’s most recent
taxation year. You must file an Income Tax Return to Revenue
Canada for the previous year in order to obtain a Notice of
Assessment. The rate reduction would take effect on the first day
of the month in which your application was completed.
If you are over the age of 65 you are required to apply for Old Age
Security (OAS). You may also qualify for Guaranteed Income
Supplement (GIS). Please call Service Canada at 1-800-277-9914
and follow the prompts to get application and qualification
information.

If you are receiving OAS and have a spouse but you are not living
with your spouse, then you can increase your OAS benefits by
applying for what is called an “Involuntary Separation Agreement”.
“Involuntary separation” is a term used only to indicate that, as a
result of circumstances beyond their control, married couples are
required to live apart. This application does not change your marital
status – it only separates your finances for purposes of calculating
the OAS. To ensure that you are receiving the maximum benefits
available to you, you are required to apply in writing for “involuntary
separation” if you are not living with your spouse. Call Service
Canada at 1-800-277-9914, press zero and ask for the number of
your regional office.

ABSENCES & VACATIONS

We encourage you to remain a part of the community and visits to


family and friends are also encouraged. Please contact the charge
nurse on duty as far in advance as possible if you will be leaving
Pine Meadow to visit. If necessary, she will see that you are given
your medication to take with you and you will be required to sign
casual leave responsibility consent.

Visits may be for overnight or up to 48 hours per week with no
effect on your extended care status. Vacation leave for up to 21
days per calendar year (pro-rated for residents admitted during
the course of the calendar year) is allowed but requires
authorization by your physician.

The Ministry of Health allows residents of long term care casual,


vacation and medical leave. The total length of a resident’s vacation
absences during the calendar year must not exceed 21 days.
Casual leave, which is separate from vacation leave, consists of
48 hours leave per 7-day period (between midnight on a Saturday
and midnight on the following Saturday). Whether you go out for
a few
hours, or a few days, it is important that we know where you are
going and when you will be back. We ask that you sign in and
out in the leave of absence book at the nursing station.
Medical leave may be taken as often as needed but may only be used
for hospital care. No individual instance of medical leave can exceed
30 days.
Psychiatric leave is available for a 60-day period for the purposes of
receiving psychiatric care.
While residents are on casual, vacation, medical or psychiatric leave,
the government will continue to pay its portion of funding provided
to long-term care facilities. The resident will continue to pay their
usual monthly accommodation charges.

date policy.
VISITORS

We do not have restricted visiting hours at Pine Meadow. Visitors


are welcome at any time provided it does not cause a disturbance in
the home. Please sign in the Visitor sign-in book at the front
entrance.
In addition to the main residents’ lounge, we have a private dining
room and there are two smaller lounges which can be used while
visiting with family and friends. The outdoor picnic shelter may also

be used for family functions. They may also be booked in advance
for celebrations such as family birthday parties. Of
course, visits may also be held in resident rooms or
lounges.
Visitors may accompany you to concerts, church
services or any other special events taking place at Pine Meadow.
You may also stay to have meals for a nominal charge that is posted
in the Home.

Please note: Visitor Policy may be restricted due to Covid-19


Pandemic. Please contact Sarah Thompson, Resident Program
Manager at 613-336-9120 ext. 226 for the most up-to-date
policy.

Pets are always welcome at Pine Meadow. Please ensure that all
pets have up to date vaccinations and are kept on a leash while
visiting. All dining rooms are prohibited for pets at all times.
Please note: Visiting Pet Policy may be restricted due to Covid-
19 Pandemic. Please contact Sarah Thompson, Resident
Program Manager at 613-336-9120 ext. 226 for the most up-to-
date policy.

WHAT TO BRING

Space for personal belongings is somewhat limited. However, as


room permits, you may want to bring some favourite items such as
an easy chair, dresser, pictures, quilts, portable TV or radio. Fabric
covered furniture must be inherently fire retardant or treated for fire
before being brought into Pine Meadow.
You will require nightgowns or pajamas, and daytime wear,
including comfortable fitting shoes and Slippers. Everything,
including dentures and eyeglasses, must be labeled with your name.
Simply leave your items at the nursing station and we will be happy
to label them for you.
We do supply items such as toothbrushes, toothpaste, disposable
razors, deodorant, body lotion and shampoo, but you may prefer

your favourite brands of these items. We also supply incontinence
products, pillows and bedding.

Valuables and Cash


Please do not bring valuables with you to Pine Meadow.
We cannot be responsible for cash and valuables kept in
your room. If you wish, the Administrator can establish
a trust account for your convenience. Money deposited
in this non-interest bearing account may be withdrawn for daily
needs, Monday through Friday from 8:00 a.m. to 4:00 p.m.

Alcohol

Alcoholic beverages may not be kept in your room. With a doctor’s


order, alcohol may be kept in the medication room and dispensed
by the nurse on duty.

SERVICES

Shaw Satellite

Please contact Shaw Direct for the satellite package you would
prefer. Contact them at 1-800-806-4830 or at
http://www.shawdirect.ca/english/
You are responsible for payment of whatever package you choose.
There are televisions available in the lounges for those that would
prefer to watch TV in a common area.

A private telephone can be installed in your room by contacting Bell
Canada directly. They will need to know your room and bed number.
Your phone bill will be mailed directly to you by Bell and is payable
to them. If you do not wish to have a phone in your
room, you are more than welcome to receive and
make long distance calls from one of the
telephones at the nursing station, and local calls in
the main lounge at the front entrance.

Mail

Your mail is sorted in our business office and delivered daily. Please
give your address of (POA) for any bills that family may be
paying or Government documents if a POA is looking after
finances. The correct mailing address to give to your friends and
family is:

Outgoing mail can be dropped off at the main office for posting each
weekday evening.

Tuck Shop

The Residents Council maintain a small Tuck Shop which travels


around the building every other Tuesday evening and is also
available upon request through the Activity Department. You can
purchase small items such as potato chips, chocolate bars, gum etc.
Proceeds from the Tuck Shop go to the Residents Council to be used
as they wish. Activity staff will gladly assist you with your Tuck
Shop purchases.

Newspapers

You can make arrangements to have newspapers of your choice
delivered to you by contacting the business office of the newspaper
and giving them your name and mailing address listed above. We do
receive several Frontenac News and The Napanee Guide on a weekly
basis.

Beautician/Barber

Hairdressing and barber services are available through an


independent hairdresser at the beauty salon. You may make regular
standing appointments for hairdressing services or simply make an
appointment when desired. Costs for these services are posted at
the salon or are available from the office. For ease of payment, we
encourage you to arrange to have these charges posted to your
monthly statement. To avoid confusion the hairdresser does not
accept cash. These services are provided on Wednesday and
Friday’s.

Laundry

Family members may continue to do your laundry or we will be


pleased to provide laundry services at no extra charge.
All of your clothing must be labeled and we will apply labels to all of
your articles of clothing upon admission. All clothing brought in
after admission must also be given to a staff member to
be labeled.

Industrial laundry equipment is hard on clothing, so


please consider purchasing more resilient fabrics.
There are brochures for adaptive clothing if that is
required.

Maintenance & Electrical Equipment

All electrical equipment must be approved and tagged by the


maintenance department before being brought into your room.

Please leave all electrical equipment at the nursing station for
inspection.

Please Note: Due to fire regulations, we cannot allow the


following types of appliances in resident rooms: kettles, coffee
makers, space heaters, hot plates, toasters, heating pads, electric
blankets, humidifiers, hair dryers, curling irons.
We regret that we also cannot allow the use of extension cords.
However, a CSA approved power bar cord, with breaker, is
acceptable for installation by the maintenance department.
You are welcome to bring a TV and DVD player and Maintenance
will assist with installation as well as hanging your pictures.
When in doubt, please just ask.

Housekeeping Services

Residents’ rooms and common areas are cleaned on a daily basis.


We will try to do this with the least amount of disruption for you as
possible. Laundry service is provided at night and clothing is
brought back to put away for resident daily.

Menus are posted outside the dining rooms and


on the bulletin boards located half way down each
wing of the building.
Nourishment snack carts are also provided three
times a day at 10:30 am, 2:30 pm and 7:30 pm.
We have a Dietician that comes in once a week, usually on

Thursday’s.

Treats

Friends and Family members can bring special food/snacks into


Pine Meadow for you. To avoid attracting unwanted pests, we ask
that all food that is brought in is in a sealed or covered container.
Please check in with the Nurse in charge, so they are aware. Also,
please check in with the nurse before sharing treats with other
residents. Please note that items brought in for residents cannot be
reheated.

A week at glance menu is posted in the corridor of each wing and
the menu for the current day is posted outside the dining rooms.
There are 2 (two) choices offered at every meal, in case the main
entrée
does not appeal to you. Every attempt is made to accommodate
therapeutic diets as well as your likes and dislikes.

Guest Meals

Friends and Family are welcome to join you for a meal for a nominal
charge which includes entrée, desert and beverage. Please give the
Kitchen staff 1 (one) hour notice when possible. Please contact the
Dietary manger for large groups and special events.

Programs

We have a strong Activity Department, who work with the residents


in the planning of a wide range of Activity programs, designed to
meet a variety of needs. Activity Calendars, which detail the events,
are posted in every resident’s room, the main hallways and copies
are available at the Nursing Stations, Front Entrance or email by
request. Programs are offered each day. Daily Activities and times
are posted on the White Board on the Lounge door as well as by
each dining room. Special programs for those residents who have
special needs are planned on an individual basis.

The department, which is staffed by trained recreationists, organizes
a variety of programs to help you enjoy your leisure time. Residents
may become involved in the planning of the activities, by attending
monthly Residents Council meetings.

Access to Outdoors

There is a beautiful fenced in area off the small activity room


at the front of the Home that has gardens, Gazebo,
walkways, sitting areas and a picnic shelter. You are free to
enjoy the flowers, stroll the path or tend to the flowers at
your leisure. To exit or re-enter the building you must press
the wheelchair access button. You may also ask a staff
member for assistance.

Library

The Library is located in the main activity room, and can be


accessed at any time. There is information on a variety of
topics, including Alzheimer’s disease. This is open to
residents, family, the community, as well as the staff. We
also have a variety of DVD’s that can be viewed or borrowed.
We can access books on DVD for those with poor vision.

Volunteers

Pine Meadow is privileged to have committed volunteers,


helping
with activities and resident programs, tuck shop,
gardening and
visits, to enrich the lives of
our residents.

Church Services

Church Service is on Wednesday morning at


10:30 in the Activity Room. There are several
different local Ministers that attend the
home on a rotational basis to lead these
services. On Sunday morning there is

a Resident Run Worship Service and on Tuesday evenings
there is another Resident Run Prayer Group.

Pastoral Care

Pine Meadow has an active Pastoral Care Committee, whose


goal is to ensure that the spiritual needs of our residents are
identified and met. Services are scheduled on Program
calendars, available throughout the home. Most of the local
clergy lead these services on a rotational basis. The Pastoral
Care Committee also plans and puts on special holiday and
Memorial services. The churches also have active visiting
programs.

YOUR CARE

At Pine Meadow we follow “person-centred” care. Your care


at Pine Meadow will be planned on an individual basis by
interdisciplinary team members. The team consists of
Medical Director, RAI/MDS Coordinator, Director of Care,
Nursing, Dietary, Activation, Restorative Care, Laundry and
Housekeeping staff, as well as yourself and your family
members. The team will focus on your physical,
psychological, spiritual, intellectual and social needs to
develop a written plan of care. You are a very important part
of this team.

Initial Care Conference:

Within the first six weeks after you arrive, an appointment


will be made with the resident and their family members to
attend what is referred to as a “care planning conference”.
The purpose of the care conference is to develop a plan of
care, which is consistent with the abilities, preferences and
specific needs of the resident, while taking into account their
safety. The conference gives the resident, family members,
and staff an opportunity to discuss together the resident care
plan and needs of the resident. Participants at the Care

Conference may include the Resident and Family members,
Nursing staff, Dietary staff, Programs and Activation.

Annual Care Conference

An annual care conference will also be arranged for each


resident with the family member, and care conference team.
The plan of care for the year will be evaluated and discussed
to determine what changes (if any) are required for the plan
of care for the following year. This also provides an
opportunity for residents and family to discuss any concerns
or provide suggestions. We do hope that you bring these to
us at any time during the year.

Medical Care

Local Doctors from Lakelands Family Health Team are


attending
Physicians at Pine Meadow Nursing Home. If you are
not already a
patient of one of these physicians, one will be appointed
for your
care on admission. Doctors visit on a weekly basis and
are available
for emergencies 24 hours a day, when necessary. We have
phlebotomist visits weekly as well as a
Physiotherapist who attends the home
weekly. Our Oxygen supplier is Medigas. We
are connected with a team of resources such
as Behavioral Support Outreach, Pain and
Symptom Management Consultants and
Geriatric Psychiatrists. Collaborative
meetings are held monthly that includes all
these team members as well as all
departments.

Nursing Care

Pine Meadow provides 24 hour Nursing care with


Registered Nurses,
Registered Practical Nurses and Personal Support
Workers.

Medications

If you are presently taking medications prescribed by your


family
Doctor, once you are admitted to Pine Meadow, all
medications are kept and distributed by Registered staff.
Registered staff takes care of acquiring your medication from
our Pharmacy, and distributing them to you at appropriate
times. Not all prescribed
medications are covered the Ontario
Drug Benefit Program. Any
additional cost of medications is
your responsibility. As
well, you will periodically receive
an invoice for dispensary charges
from Medisystem. To avoid delays in payments please set up
the account with the address of the POA that will be looking
after this bill.

Diagnostic Tests

At times your physician may refer you outside of Pine


Meadow for specialty consultations or tests. We will be happy

to assist you with making the transportation arrangement for
such visits, however
transportations costs on the visit are the responsibility of you
or your family. If you need an escort (staff to assist) and your
family member is not able to accompany you, arrangements
can be made for assistance at a cost. Please ask the charge
Nurse for further information.

Foot Care

Pine Meadow offers monthly Foot care Clinics in the home for
the convenience of our residents. This is provided by a
registered staff with special training in this area. The cost is
$22 and you must register for this service in the main office.
The charges can be added to your monthly bill.

Advanced Directives

The advanced directives form is your opportunity to tell your


family, your doctor and the staff at Pine Meadow how you
would like your care managed when you are no longer able,
because of poor health, to speak

for yourself. By documenting your decision,


staff will be prepared to fulfill your
wishes should you suddenly become ill.
Upon Admission, you will be asked to sign a document,
which states your wishes with respect to the care you wish to
receive under these circumstances. For a detailed
explanation of the 4 (four) levels of care you must choose
from, please refer to the booklet “Advanced Directives”,
included with this package. We, as partners with you in
the care team, appreciate how difficult it can be for you to
reach such a difficult decision at any time, especially as your
condition changes. Let your wishes be known to your health
care team so we can respect and support your wishes.

Policy on Minimizing of Restraining

Pine Meadow has a written policy about restraints. We


are a least restraint facility. Please see the brochure in your
welcome package entitled “Minimizing Restraint Use”. This
includes bed rails due to entrapment risk.

Call Bell System

All bedsides are equipped with a call bell on a cord, which


should be kept close to the resident at all times. This cord is
connected to the call system at the Nursing station and
pagers, and once activated can only be shut off at the
resident’s bedside. There are additional call bells in the
bathrooms and shower rooms. The nursing staff will show
you how to use the call bell. You are encouraged to use this
system for your safety. A staff member who is identified with
a name badge will answer your call bell for assistance.

Vaccines

The success of flu vaccines has encouraged Pine Meadow to


participate in an annual flu vaccine program. Vaccines will
be given by one of the Registered staff with your consent.
Pneumococcal, tetanus, and diphtheria vaccines are also
available and free of charge. All residents admitted to a Long
Term Care Home are required to have a chest x-ray

either 90 days prior to admission or 14 day’s after admission.


For more information please talk to the charge
nurse or Director of Care (DOC). The shingles
vaccine is available from Public Health for a fee
as well.

Wheelchair and Walkers

We have a supply of both wheelchairs and walkers, which
can be used for a short time frame only. If you require full
time use, of either piece of equipment, we can make the
necessary arrangements for an assessment and purchase of
equipment through our physiotherapist.

Care Levels - Palliative Care

Palliative Care can be provided in our palliative care room or


you may remain in your room, if that is your wishes. We
have dedicated, trained staff who care for our residents at
the end of their life journey. Since they have known the
residents for their duration of his or her stay at Pine
Meadow, they are able to accommodate the resident’s likes
and dislikes regarding day to day activity.

SAFETY CONCERNS

Pine Meadow has an active Health and Safety program to


ensure the building is safe for the residents and employees.
Every attempt is made
To keep things free from clutter. We require 18inch clearance
on top in you closet for Fire Safety reasons and our sprinkler
system.
Staff require enough room to be able to provide care that is
required, such as mechanical lifts, toileting items
(commodes), and the bedside table to a care basin etc.
For infection control reasons, housekeeping need to be able
to clean areas such as bathroom vanities and tops of night
stands. Please be mindful of this when storing personal
items on top.

Regular fire drills are required by law in Long Term Care
Homes (MOHLTC). They are held monthly on all three (3)
shifts. These regular tests of the systems and procedures,
helps to ensure sufficient fire protection for your safety.
When the fire alarm sounds, most of the doors close
automatically, including some of the resident room doors.
Generally we ask that you stay where you are, when the fire
alarm goes off unless you are danger, until a staff member
comes to assist you. As part of our recent redevelopment
project, we now have wet and dry sprinkler systems
throughout the building that have significantly added to the
safety of our building. We do have planned evacuations, and
times fires frills that our Fire department are involved in
annually.

Safety and Security

Some residents with impaired memories may tend to be


disoriented and could wander off the premises and become
lost or injured. Rather than having a locked ward, the whole
building is secure with a wander guard system. Any resident
that has the potential to wander will be provided with a
bracelet for their ankle or wrist. If a resident with the
bracelet attempts to leave the building through any exit, the
alarm system will be activated. The system identifies the
resident that has exited the home as well as which exit door.

Smoking

Residents who are able to smoke independently without


supervision
are permitted to smoke outside in the smoking area. They
must be 9
meters away from an entrance or any exit door. Please
refer to the
Smoking Policy for more information.

YOUR RIGHTS

Bill of Rights

In May 1987, the Ontario Government passed legislation that


guaranteed all the residents in Ontario Nursing Homes
certain rights. They also require that
these rights be presented to
residents in booklet form that details
their rights and responsibilities. A
copy of this booklet is included in your
welcome package and is posted at the
front door and in the hall way. Resident’s rights are reviewed
at each Residents Council meeting.

Residents Council

The Residents Council is intended to facilitate


communication between the staff and residents at Pine
Meadow. It is composed of residents, who are assisted by an
advisor in the Program Department. The Council receives all
monies taken in through the Tuck Shop, pop machine, coffee
shop, and other fundraisers that occur periodically. If you
are interested in becoming involved in Residents Council,
please mention it to the Program staff for more information.
The minutes from the meetings are posted in both hallways.

Family Council

The main purpose of the Family Council is to improve the


quality of life of the residents and to give families a voice in
decisions that affect them and their loved ones in the Home.
All interested family members are invited to join.

If you would like additional information about the Family


Council and its role, you may wish to contact them.
Information is provided on the Family Council board located
near the front lounge.

When you have a concern

Your care at Pine Meadow will be planned with you and


your family.
There may be times when problems arise or you have
concerns. We
ask that you give us the opportunity to address these
concerns by
meeting with us promptly, so that any issues can be
resolved. It is
our philosophy to work with you and your family to
provide the best
care possible, we ask that we be given the first opportunity
to try and

you can

the residents’ quality of life through the care and services we
