1. When you go to the Testing Area, you MUST have with you:
a) This Test Permit in 2 copies
b) 1 pc. 2x2 size photo with Nametag and White Background (with name tag)
c) Senior High School ID currently issued by your School Official (For senior high school graduating
students) or any valid ID
d) Photocopy of your Academic Records:
• For Current Grade 12 Student—Photocopy of Grade 11 Report Card
• For Grade 12 Completer—Photocopy of Grade 12 Report Card
• For ALS Graduate—Certificate of Completion
• For Transferee—Transcript of Records (TOR)/ Copy of Grades signed by Registrar with the GWA
of atleast 2.25
e) Photocopy of Certificate of Income of Parents or any of the following:
• For 4P’s member: 4P’s Certification from MSWD/DSWD
• For Indigenous People (IP’s) member: Certification from IP Chieftain
• Enhanced Comprehensive Local Integration Program (E-CLIP) Beneficiary/ PAMANA (Payapa at
Masaganang Pamayanan-Peaceful and Resilient communities) beneficiary: Certification from
DILG / Office of the Presidential Adviser on the Peace Process (OPAPP).
• Parent’s Employed in Government or Other Agencies: BIR Form 2316 (2020)
• Parent’s Working Abroad: Overseas Employment Contract stating the name of company and
monthly salary
• Parents as Business Owners: BIR Form 1701 (annual payments,2020)
• With Deceased parents: Death certificate
• Solo Parent: Solo Parent Certification from MSWD/DSWD or Notarized Sworn Statement
indicating the Source of Income
• Others (Not Stated above): Notarized Sworn Statement (Joint Affidavit indicating the Occupation
and Source of Income)

g) 1 or 2 pencil(s)
2. You must REPORT to the Testing Area at least one (1) hour before your scheduled time.

I agree to the Terms of Use, have read and understood the Privacy Policy, and confirm that the information I have provided to the
University are true and correct. The submission of my SLSU Entrance Examination application form constitutes my consent to the
collection and use by the Southern Luzon State University (SLSU) of the information I have provided as well as the transfer for
processing and storage at SLSU.

Furthermore, I agree and understand that I am legally responsible for the information I entered in the Online SLSU Entrance
Examination Application, and if I violates its Terms of Service, my information shall be removed and shall lead to the termination,
without prior notice of my application and everything associated with it.

I also understand that my admission in the University shall be based on passing the SLSU cut-off score and meeting the University
requirement, subject to the availability of slots.
