Scope: SCDR-A Topsides: The scope of work consists of Construction, Engineering, Procurement
and supply of materials, Fabrication, Inspection, Testing, and On-Shore Commissioning,
Loadout & Seafastening of the SCDR-A Topsides and Associated Appurtenances.
Project Background: The Shallow Clastics field covers an area of ca. 600km.sq., (40-km x 15-km)
consisting of alternating sand shale sequences in which eight-gas bearing intervals have been
identified to date. The field comprises two en-enchelon ENE-WSW trending culminations that
overlie the F13 and E11 carbonate fields in the northern and mid part of the field, respectively.
SCDR-A will be place on the SW area of the Shallow Clastics field with four highly deviated wells
and one back-up well and tied back to the E11K-A Platform via a single 6.4-km, 14-inch (0.35-m)
carbon steel pipeline. The gas and condensate from the SW area of the reservoir will be
transported wet as a multiphase mixture to the E11K-A facilities, where the fluids will be
separated, dehydrated and re-combined prior to export to MLNG DUA.
The SCDR-A is an unmanned drilling riser topsides design for a “minimum facilities” concept. The
minimum facilities consists of Flow lines, Wet Gas System, Wellhead Desander System, Vents and
Drains Systems, Fresh Water System, Pedestal Crane, Process Control System, IPS, Fire and Gas
System, Telecommunication System, etc.

9.0 Fabrication
9.1 Introduction
The SCDR-A Topsides Project structure was consists of Cellar Deck, Mezzanine Deck, Main
Deck, Wellhead Platform, Intermediate Platform and Flare Boom. The total weight is
approximately 1,700 metric tons. In additional to that, there is fabrication of ship loose
items for SCDR-A pipeline tie-in works at existing E11K-A Platform.

The test pressure of each package was as shown in the hydro test packages.
The extent of the package that hydrotested was barricaded and with warning
signboard during hydro test.
Hydro test results were recorded on pressure recorders and all data/charts were
clearly marked and compiled at the completion of the Flushing and Hydro-
testing activities. A Flushing and Hydro-Test Procedure was developed. After
hydrostatic testing, the piping systems was thoroughly drained to remove all the
water and following by air-drying.
8.4.4 Reinstatement Test
Reinstatement test was carried out after hydro-test. The following activities were
completed before the reinstatement test commenced:
All temporary components used during the hydro test were removed.
All piping, valves, instrument, equipment and line elements were installed.
The piping system within the reinstatement package was made up to the
final operating condition.
All reinstatement packages was developed and line checks was carried out
prior to reinstatement testing, which include sequence of building up test
pressure up to the specific test pressure and the sequence for depressurizing
including safety requirement was observed during the actual testing.

8.5.2 All mechanical supports/sitting fabrication works was done inside the workshop
taking into account the as-built measurement of the equipment skid supports
and the deck levelness. Specific installation heavy lift procedure for major
equipment was developed.
8.5.3 Prior to delivery, MMHE ensured that the equipment was inspected and tested
either at vendor premises or upon delivery at MMHE.
8.5.4 MMHE completed all required testing & inspection prior to installation.
8.5.5 Major equipment, which required PTW or special permit for lifting, were erected
to location pending its availability.
8.5.6 Equipments, which are sensitive to moist, humidity, extreme heat or duct, MMHE
provided adequate protection. The preservation covered throughout the
fabrication period.
8.5.7 Major Mechanical equipments installed are, but not limited to the following:

8.6.2 Prior to delivery, MMHE ensured that the equipments and devices were inspected
and tested either at vendor premises or upon delivery at MMHE.
8.6.3 MMHE completed all required testing & inspection prior to installation.
8.6.4 Equipments and devices with sensitive control devices and electronic devices
were stored in an enclosed air condition storage area prior to installation. The
preservation covered throughout the fabrication period.
8.6.5 All E&I supports, brackets and stands were fabricated inside the workshop as per
approved IFC dwgs. All supports, brackets & stands were full seal welded and
painted in accordance with SSB specifications unless shown otherwise on the IFC
dwgs.
8.6.6 All fabricated E&I supports, brackets & stands accepted by SSB were installed in
accordance with latest IFC dwgs. Full welding was done only after confirmation if
no obstruction/alteration within the areas concerned.
8.6.7 All E&I equipment were installed strictly on accordance with manufacturer’s
instruction and specification. Specific installation heavy lift procedure for major
equipment was developed.
8.6.8 The major E&I equipments installed are, but not limited to the following:

8.7.1 All raw fabricated materials (i.e. supports, brackets, stands, etc) were blasted
and applied approved Epoxy or Main Coating / Primer Epoxy Glass Flake prior to
erection.
8.7.2 For tubular, diagonal braces and columns were blasted and applied main coat
prior to erection.
8.7.3 All main steel assembled at ground level were spot blast on the welded
connection and welded attachment area and painted by the main coating
prior to lifting of the deck to its position.
8.7.4 The finished coat was applied right after the primary steel box-up and damage
portion thereafter was repaired and cosmetic painted during and after all
interdisciplinary components are installed, prior to MCD.

10.1 Testing & Pre-commissioning outline the final inspection and onshore pre-commissioning
of the package system including the equipment installation works on SCDR-A Topsides
Project.
Onshore pre-commissioning proved that all equipment available have been installed
and connected in accordance with IFC drawings and operation is in line with the
design concept.
All inspection and test results are recorded on the SSB format of check sheets, Shell EP
Asia Pacific (EPA) format. The check sheet overview for each system is provided in the
detailed procedure. MMHE assigned personnel that performed the testing, inspection &
onshore pre-commissioning of various types of system and equipment, followed the
approved detailed procedure.
MMHE requested all tests and witnessed by SSB Commissioning Team. MMHE recorded
all test and counter signed by SSB Representative after testing was completed.
Original copy of check sheets were handed over progressively throughout the pre-
commissioning period and were kept in accordance with the commissioning system
numbering for final documentation.
10.2.2 Function test of electrical protection relays such as earth fault relays, overload
relays and etc.
10.2.4 Loop checking and static function test of all control panels.
10.2.6 System function test inclusive of over-all testing and shutdown matrices.
10.2.10 Full commissioning of HVAC, Pedestal Crane, etc. and maximize the

C. Lessons Learnt
1) Design team should be the frontline in advising the project team at beginning stage for any
materials shortage or clashes or any related activities that are not highlighted by the client’s
consultant. The time frame for justification and advising the project team also should be at a
high-speed mode. Not to copy and paste only the client’s entire only consultant drawing
details.
Design should improve in quality of designing & be very strong in justifying related queries
and should work as a team among design team when finalizing the drawings.
2) Procurement department should improve in delivery the PO and should transparent in
reporting the delivery of materials.
3) Material Control should tally the information materials delivery to project team to avoid too
many urgent releases given during project implemented.
4) Planning should detail out all activities and proactive in advising the impact of planned work
delivered.

1.5 Housekeeping
The project workforce carried out daily housekeeping and cleaning activities at site
before/after work and weekly “Gotong Royong” on Saturday. These programs were aimed
at increasing job efficiency, improving safety performance, reducing risks of fire / explosion
and preventing trip / fall accidents
1.6 Drills
The project executed two drills, the first on 14th February 2007 for First Aid, Rescue and
Evacuation Drill and on 17th July 2007, and the second one on Rescue, Evacuation and Fire
Drill. Both exercises successfully done as per our expectation.

Toolbox Meetings were also being used as an educational forum to improve employee’s
knowledge and understanding of health and safety systems, rules and requirement.
The topic of the HSE Talk are predominantly focused on the risks, known hazards, unsafe
acts or practices related to the daily activities. The HSE talk also acted as a tool for
management and workers inter communication to convey information and to mobilize
action on current aspect of health safety and the protection of the environment.
All MMHE employees and subcontractors who are involved with the fabrication of SCDR-A
were required to attend the talk and attendance was taken. Any absenteeism without
good reason is considered non-compliance to HSE Plan. To ensure sufficient space to
accommodate all the workers and to protect the place from rain and sun, we erected a
zinc roof shed with a staged podium to ensure this important gathering is always carried out
despite the weather conditions.

Safety Audits / “Walkabout” were conducted on every Monday at 8:30 a.m. It was
participated by a group of personnel, which consists of SSB Company Site Representative
(CSR), MMHE Project Manager, Subcontractor Project Managers, Discipline Superintendent,
HSE Executive, HSE Supervisor, Discipline Engineers, Discipline Supervisors and Subcontractors
Supervisors. Any hazards or unsafe act/conditions will be highlighted and action will be
taken to eliminate or rectify immediately, unresolved findings will be discussed during the
HSE meeting for further action.

The purpose of the Walkabout is to ensure the work site is safe and in compliance with HSE
rules and regulations. The findings will be discussed in the HSE Weekly meeting.
Apart from the regular Monday Safety Walkabout, we have also implemented a twice-daily
safety task force walkabout among the subcontractors which was conducted by the MMHE
and subcontractor safety promoters at all work locations. The findings and action taken
were discussed in daily safety promoters meeting and submitted on a daily basis for records.
HSE education for all personnel involved in the project plays an important role in
promoting safety work practices, Although the HSE plan only requires once every two weeks
but we conducted on a weekly basis, however we have exceeded this requirement. We
were able to screen the 30 minutes safety video shows every Friday, 520 personnel have
attended. This was made possible by having our own TV and VCD player specifically
dedicated for the SCDR-A project. SSB staff generously lent the audiovisual items.
Two QHSE day celebrations were held during the onshore fabrication. The first milestone
achievement was celebrated on 10/05/07.after achieving 250,000 man-hours while the
375,000 milestone was recorded on 14/07/07.
HSE incentives in both cash vouchers and non monetary form such as t-shirts, caps and
mugs were handed out to the workforce on every Monday morning talk for the most
number of U-See, U-Act submitted during the week or month, HSE quiz. Apart from that we
also hand out tokens to those who were selected on the spot for good HSE practices where
PMT and clients top management were making their visits to site. The number of tokens
given is 215.
So far we have issued 51 SAC and 7 SVC to non-compliant at site. The objective is to
discourage workforce not to commit un-safe work practices.
The objective is to maintain a safe, healthy and productive work environment, the use and
abuse (sales, possession, trafficking & influence) of drug and alcohol. The number tested so
far, 341 all negative.

Two campaigns the first one PPE Awareness Campaign was launched on 10//05//07
and the second, PTW Management Campaign on 25/06/07. A unique feature towards a
pro-active HSE management was the launch of “HSE Commitment by Contractors and
PMT Signing Ceremony”. we were serious about Safety and would ensure that they would
carry out their work in safe manner and complete it on time. Beside the campaigns, all
SCDR-A Project workforces have to wear coverall at all time as per the contract.
HSE workshops were held during the course of the project for supervisors, workers and HSE
personnel. The workshops focused on the Hazard Identification (HIRAC), proactive
interventions (PIC) and refocusing on the HSE objectives for the project with an eye on
Improving HSE performances. MMHE HSE Executive and SSB HSE Advisor jointly animated
the workshop.
All types of incident and injuries, including LTI, MTC, RWC, first aid, near miss, property and
environmental damage were required to be promptly reported within 24 hours to
MMHE/SSB representative verbally and writing.
Since the project commenced on the First Cut of Steel, 12th September 2006 until MCD
date on 28th August 2007 and Sailaway on 27th September, 2007, SCDR-A Topsides has
achieved our vision of “Nobody Gets Hurt” and the target of ZERO INCIDENT.

Overview: Project is an EPCC and a lump sum procurement contract by MMHE. Client Sarawak
Shell Berhad. Target MCD is on 28 August 2007. Sail away by 5 September 2007. This
project is a Topside Drilling Flatform and Satellite Tie-in Flatform. Location is 200

Scope: D & E scope include, but not limited to, review and checking of Client-supplied AFC drawings,
PDMS and ACADD models, MTO’s, COG, Structural analysis and other calculations related to
the execution and final completion of the project. Scope also covers procurement and
technical evaluation of contractual scope of supply such as Structural steel, Piping bulk
materials, manual valves except ball valves 2” and above and other materials required in the
Contracts; deliverables such as fabrication drawings for all disciplines and reference drawings
from Client-supplied AFC drawings and soft copies.
Overview: The Shallow Clastics field, located 140-km NW of Bintulu, was discovered in 1969 by wells
F13-1 and E11-1. The field is one of the eleven fields under the MLNG-DUA PSC, which took effect on
30 June 1993. Under the terms of this PSC, Sarawak Shell Berhad as a joint venture arrangement with
PETRONAS Carigali Sendirian Berhad will operate the Shallow Clastics field until PSC expiry on 26 May
2015. Shallow Clastics Phase 3 is the development of the SW area of the Shallow Clastics field. SCDR-
A will be placed on the SW area of the Shallow Clastics field with four highly deviated wells and one
back up well and tied back to the E11K-A platform via a single 6.4-km, 14” carbon steel pipeline. The

Vendor supplied and delivered the equipments (Closed drain vessel and Condensate Pod)
to MMHE yard without completion of Non Destructive Test and improper compilation of
Manufacturer Data Record. Rectification should be done by Project Team in order to fulfill
the Code requirements and Client specifications. The repair activities such as re-alignment
on nozzle, re-Radiography, re-Hydro test and repainted. These kinds of repairs consume a lot
material, consumables, schedule and cost as well.
Recommendation:
• Site visit for ensuring capability and performance of the selected vendor / supplier for
the project critical equipment /mechanical package
• To conduct pre-execution audit prior to fabrication commencement to give more
confidence on availability and readiness of drawing, material, project
personnel/manpower, tools and yard facilities.
• Procurement to update vendor data monitoring registry to achieve final
documentation compilation in schedule.
• To increase the frequency of inspection to vendor yard for earlier detection if any

Project Overview: Generally SCDR-A Topsides is a profitable project but the project was largely hit
by the price escalation on Stainless Steel and Duplex Stainless Steel related
materials/equipment.
Few materials and equipment delayed of delivery due some prolonged
settlement on over-budgeted issues thus contribute to most of negative
progress of the project and catch-up progress by means of acceleration is
inevitable.

3.1.1 Material procured last minutes to cater miss-out and modification works
entails ex-stock material is secured at higher price.
Design MTO to be doubled check by Construction to cater for potential
unavoidable clashing/modification, site routing etc
3.1.3 Price escalation especially SS and DSS based materials. Further negotiation.
3.1.4 Delayed delivery project materials i.e. DSS pipe materials entails project to
be accelerated to regain the progress would cost extra money to the
project in terms of acceleration.
3.2.1 Procure project materials thru CO mechanism for faster delivery to catch-up
the schedule. Procurement, Design and Construction to double-checked
actual materials and to cater potential site routing/re-routing, clashing or site
obstruction.
