The questions below are of great importance to the organisers’ team. With your answers, especially the
ones from those that will be selected, we can design the training to your realities, your organisations and
your questions. Therefore, we would like to ask you to take your time and to better give a short and

By signing the present document I commit myself to participate in the whole process, including:
- Preparing myself carefully for the youth exchange
- Taking part in full duration of the youth exchange
- In case of selecting me to the seminar I declare that I will participate during the whole
time of the activity. Otherwise I can be invoiced with the individual cost of the activity
- Participating in evaluation process
- Making Health and Social Insurance

use photographs, films and recording done during the project. These photographs, films and
voice recordings may be included and archived in the European Union’s online database,
accessible to public use.
