 Accompanying this application must be:

(1) a copy of your report card from the last school semester. A school
generated printout can also be used as grade verification.
(2) $10.00 donation/dues (ASB)
 Turn in your completed application before school, after school, or at lunch.
No applications will be accepted during class time and no application will be
accepted late.
