AFFIDAVIT OF EMPLOYMENT

I, MA. DIVINA S. GEMPARO, Filipino, of legal age, single and a resident of San Jose,
Malaybalay, Bukidnon, Philippines, after being duly sworn to in accordance with law,
hereby depose and state:

1. That I am an employee of MBBR Manpower Services located at

Greenfields Subdivision, Casisang, Malaybalay City, Bukidnon, from
August 2015 to April 2016;

2. That the said company ceased to operate last June 2018;

3. That the only evidence of my employment of the said company is my

record with the Social Security System (SSS);

4. As such, I am executing this Affidavit of Employment to attest to the truth

of the foregoing and for whatever legal purpose this may serve best.

IN WITNESS WHEREOF, I have hereunto set my hand this 18th day of October
2021 at Malaybalay City, Bukidnon, Philippines.
