PARTICULAR
Supervision is a process of facilitating the professional growth of a teacher, primarily by
giving the teachers, department heads and school heads feedback about classrooms’
/schools’/learning centers’ interactions and helping them to make use of the feedbacks in order to
make teaching/learning more effective.

SDO Pangasinan II will adopt a uniform template in gathering inputs and data in the
schools/learning centers. Supervisory Plan and Report shall be submitted during the first week of
each month for the consolidation/gathering, interpretation and analysis of data; and shall be used
as bases in providing Technical Assistance (TA). The said report shall also be included as Means
of Verification (MOV) for the Individual Performance Rating of SDO Supervisors, Specialists,
School Heads and Department Heads.

Attached is the Supervisory Plan and Report Template and samples of focus of
observation.
