FIRST WARNING LETTER

Dear Puan Nurin,

Re: Late Renewal of Insurance

The above matter refers.

Based on email, you had requested quotation for the general insurance for the Company
from our insurance broker on 25 November 2021 and received the quotation on 12
December 2021.

This insurance cover expires on 14 December 2021 but the renewal quotation was only
presented to the Management after the due date. At the same time, no extension of
insurance cover was done from 15 to 20 December 2021, exposing the Company to loss
from unforeseen circumstances.

The Management does not tolerate such negligence and expects quotation be obtained 2
months before expiry date and insurance renewed timely. This letter serves as an official
written warning to you.
