AFFIDAVIT OF UNDERTAKING

I,_______________________, of legal age, residing at

_________________________________a member of Philippine National Police,
presently assigned at ___________________________ designated to the position of
_______________ _____after having duly sworn to in accordance with law, do hereby
depose and state that:
1. That I am fully aware that during the election period I shall not directly or
indirectly, intervenes in any election campaign or engages in any electioneering or
partisan political activity, except to vote or to preserve public order;
2. I shall not form organizations, associations, clubs, committees or other groups for
the purpose of soliciting votes and/or undertaking any campaign for or against
candidate;
3. I shall not hold political caucuses, conferences, meetings, rallies, parades or other
similar assemblies, for the purpose of soliciting votes and/or undertaking for or
against the election of any candidate for public office;

4. I shall not make any speeches, announcements or commentaries, or holding

interviews for or against the election of any candidate for public office;

5. I shall not Publish or distribute campaign literature or materials designed to support

or oppose the election of any candidate;

6. I shall not wear of t-shirts or pins, caps, or any other similar election paraphernalia
bearing the names of the candidates and/or political party except as authorized by
the Commission on Elections;

7. I shall not bear, carry or transport firearms or deadly weapons outside residence or
place of business, and in all public places, including any building, street, park, and
in private vehicles or public conveyances, even if it is licensed or authorized to
possess or carry the same, unless authorized by the [Comelec];

8. That I am only allowed to carry and possess firearms while in agency-prescribed

uniform and performing official functions during the election period;

9. I shall not employ, avail myself or engage the services of security personnel or
bodyguards to any candidate for public office, including incumbent public officers
seeking election to any public offices, unless authorized by the [Comelec]; and

10. Any other circumstances that may be considered analogous with that of the
aforementioned

That I attest to the truthfulness of the foregoing declaration and submit myself to
the legal consequences thereof, if ever the statements above are wanting in truth and in
substance. Affiant declares that he/she has read and understood the affidavit of
undertaking and that the same is his/her act and deed.
