Affidavit of Support and Consent to Travel 

For parent/s giving consent to someone to accompany the travel or p​ assport application of their minor child. 
Requirements: 
1. Personal appearance of applicant/s 
2. Two (2) duly Accomplished Affidavit of Support and Consent to Travel forms 
3. Two (2) valid passport copies of parent/s 
4. Two (2) valid passport copies of adult companion

AFFIDAVIT
(Support and Consent/Parental Travel Permit)
