Description

On 03.03.2022 at about 15 20 hrs, crew of four personnel’s naming respectively Mr. Abhraham Kollie (Steel Bender), Mr
Archie Perry (Steel Bender), Mr. Enoch Freeman (Steel Bender) and Mr. Benedict (Steel Bender) have started manual
unloading of “loose bundled U shape steel bars (8mm thickness with 12meters in length)” from the trailer to the
designated location on instruction from site supervisor Mr. Hayatul Mondal due to unavailability of crane (which
supposed to be get used for mechanical unloading).

While unloading the steel from the trailer at about 15 45 Hrs; due to misjudgment among the crew, one of the steel bars
got slipped on ground from one steel bender’s hand and due to sudden jerk (due to vibratory motion) to the steel bar,
one end of the steel got swing back and moved just near to the face of Mr, Benedict.

Same near miss has been witnessed & reported by site supervisor. No injury/property damage reported.

Root Cause:
 Mechanical lifting/unloading has not been opted by the site supervisor.
 Lack of communication between the crew while doing manual unloading of the material.

Note: This near miss has a potential of high potential body injury.
