GENERAL INSTRUCTIONS GOVERNING THE WORKING OF THE
ELECTRICAL POWER SYSTEM OF DES
1. These operating regulations shall apply to all operating, maintenance and construction
on the MV/HV apparatus of the Department of Electrical Services. Their purpose is to
ensure the safety of all persons and to safeguard both apparatus and the continuity of
supply.
2. Every employee issued with this book must make himself thoroughly acquainted with,
and shall be held responsible for compliance with, the regulations contained herein.
3. A copy of this book shall be issued to all such employees and other persons as the
Senior Engineer may decide upon and the procedure governing the issue, production
upon request, and return of such books shall be in accordance with the rules approved
by the Director.
5. These regulations are an extension of and must be read in conjunction with the
Government Gazette Part II as amended and the regulations made there under.
7. In cases where, for practical reasons, these regulations cannot be complied with and
where safety is not compromised, an extension must be applied for in writing to the
Director.
8. Any word in these regulations implying the masculine gender shall also indicate the
feminine and vice versa.
9. These regulations will come into force on the 1st April 2011.
10. Authorisation certificates shall be valid for a period of three years and such
certificates will be withdrawn should employees no longer carry out switching
operations as part of their duties.

(b) To take care of this book of System Operating Regulations and always have it
readily available;
(c) In the event of the book being lost, to immediately report the matter to my
supervisor. A charge could be levied for the replacement of the book.
(d) To amend and update the System Operating Regulations promptly as directed;
I understand that no changes will be made to these rules unless they have been incorporated
in these System Operating Regulations or authorised in writing by the Director.

SECTION 1
DEFINITIONS OF TERMS
1.1 PERSONNEL
1.1.1 AUTHORISED PERSON shall mean a person appointed in writing by the Deputy
Director, or his nominee to carry out switching, isolating, testing and earthing
procedures on Medium/High Voltage mains and/or apparatus in liaison with the
System Control Operator and to initiate Switching Requests and issue Permits to
Work in respect of such mains and/or apparatus.
Operating voltage limits and any restrictions to this authorisation will be indicated on
the authorisation document. The Authorised Person shall only operate within the
limits and restrictions of his/her authorisation certificate and it is his/her responsibility
to advise the System Control Operator if the switching operation is beyond the limits
and restrictions of the authorisation.
1.1.2 COMPETENT PERSON shall mean a person who has been adequately trained and
is deemed to have sufficient technical knowledge and qualifications to enable him/her
to discharge their duties in accordance with his/her respective duty schedules and
where applicable, to perform work safely on the Power System and to appreciate the
dangers associated therewith. A ‘Non-Competent Person’ shall mean a person who
does not comply with the definition of a Competent Person.
1.1.3 CONTRACTOR shall mean any person or group of persons, not in the employ of the
Utility, who has been engaged to undertake work on behalf of the Utility.
1.1.7 GENERATION ENGINEER shall mean the Engineer and/or any other Engineers,
appointed by the Utility for controlling the electrical power generating system and its
associated switching devices.

1.1.9 POWER PLANT SHIFT OPERATOR shall mean a person appointed by the Utility
to assist the Power Plant Shift in-charge to operate the generating plant and control
switching operations on the distribution system carried out by others and authorised to
carry out operations of switching devices in the generating plant including
synchronizing at 66kV.
1.1.10 POWER PLANT SHIFT IN-CHARGE shall mean a person appointed by the
Utility to direct the operations of the generating plant and the control of switching
operations associated with generation, including synchronizing at 66kV.
1.1.12 SPECIFICALLY TRAINED PERSON shall mean a person who has been
specifically trained to undertake certain tasks on or near live electrical apparatus
without being under the constant supervision of a Competent Person. Such persons
shall have received a letter of appointment from the Deputy Director, wherein the
exact limits of their duties and responsibilities shall be clearly set out.
1.1.13 SUPERVISOR shall mean an employee who is responsible for the supervision of
work.
1.1.14 SYSTEM CONTROL ENGINEER shall mean an engineer appointed by the Utility
and employed to carry out duties at the System Control Centre of the Utility, for the
purpose of controlling the generation of electrical energy and its transmission and
distribution throughout the Utility's High and Medium Voltage System.

1.2.1 ALIVE OR LIVE shall mean that any mains and/or apparatus so described is
electrically energised or charged.
1.2.5 BARRIER shall mean any device designed to restrict approach to areas where
the safety of persons may be endangered due to the presence of live electrical
conductors/apparatus, excavations or other dangerous conditions. Barricading/
barricade shall have a similar meaning.
1.2.9 BUS COUPLER shall mean a circuit breaker or isolator designed to connect
or disconnect duplicate busbars.
1.2.10 BUS SECTION shall mean a circuit breaker or isolator installed to connect or
disconnect sections of a busbar.
1.2.11 CABLE shall mean any conductor, including its terminations, used for
distributing electrical energy.

1.2.14 CIRCUIT MAIN EARTH - An earth connection to the circuit and apparatus
on which work is to be carried out and for which a Permit to Work is issued.
1.2.17 CONTROL SWITCH shall mean the switch or device, which controls the
operation of circuit breaker, switch, link or other apparatus.
1.2.18 CONTROL POINT shall mean a position on the system where a main and/or
apparatus can be switched, isolated and earthed.
1.2.19 DANGER shall mean risk or loss of life, limb or health from electric shock,
burn or other injury or from fire attendant upon generation, distribution or
utilization of electrical energy.
1.2.23 DANGER NOTICE shall mean a general notice in approved form calling
attention to the danger of approach to or interference with mains and/or
apparatus.
1.2.24 DEAD shall mean at or about zero potential and isolated from any live system.
1.2.25 DES shall mean the Department Of Electrical Services, Prime Minister’s
Office.
1.2.26 DES CONTROL POINT shall mean the switch or device belonging to the
DES which controls the operation of a circuit breaker, switch, link or other
apparatus and such a switch or device is directly connected via a cable to a
main 11kV DES substation.
1.2.28 DISTRIBUTION PILLAR shall mean a device where two or more circuits
can be connected together within a cubicle.
1.2.29 DISTRIBUTION SYSTEM shall mean all apparatus and feeders used in
connection with distribution of electricity.
1.2.30 EARTHED shall mean connected to the general mass of earth in such a
manner as will ensure at all times an immediate safe discharge of electricity.
1.2.31 EARTHING OR “TO EARTH” shall mean the operation of connecting any
circuit to earth after it has been isolated and tested.
1.2.34 ENCLOSURE shall mean an indoor or outdoor site where electrical apparatus
is enclosed and the access locked to prevent unauthorised entry.
1.2.35 FEEDER shall mean a line or cable into or from a substation or other
apparatus.
1.2.39 HIGH VOLTAGE (abbreviated HV) shall mean a voltage of 66 000 volts.
1.2.40 IN COMMISSION shall mean the state of apparatus, which is available for
immediate use although it may not actually be in service.
1.2.41 INTERLOCK shall mean an electrical and/or mechanical device for ensuring
that certain operations are performed in a pre-determined sequence.

1.2.42 ISOLATE shall mean physically disconnecting the apparatus from all
possible sources of electrical potential. In this context isolating may be
obtained by:
1.2.43 ISOLATOR shall mean the apparatus provided for the purpose of isolating
mains and/or apparatus.
1.2.44 LINE shall mean an exposed overhead feeder, and includes the supports and
the cable, where provided, between the supports and/or the substation
apparatus.
1.2.45 LINK shall mean a device other than a switch provided for the purpose of
isolating electrical mains and/or apparatus.
1.2.47 LINK STICK shall mean an approved insulated rod designed for specific
voltages, used for the application of portable earths, testing and operating links
and drop out fuse links.
1.2.49 LIVE ENCLOSURE shall also mean any room, chamber, yard or enclosed
area in which it is possible for a person, from ground or floor level, to make
inadvertent contact with or infringe safety clearance to live conductors or
apparatus. Any room, chamber or enclosed area fitted with automatic fire
protection equipment shall be treated as a live enclosure until the automatic
fire protection equipment is isolated.
1.2.50 LIVE TESTER shall mean a device, which is required to make contact with
the conductor under test in order to determine whether the conductor is LIVE
or DEAD.
1.2.51 LOCK OUT shall mean that an auto-recloser has automatically gone through
its tripping and closing sequence to its final open state.
1.2.52 MAINS shall mean the cables or overhead lines forming part of the utility’s
distribution system.
1.2.56 OPERATING shall mean the carrying out of switching, isolating, testing and
earthing of electrical mains and/or apparatus.
1.2.58 OUT OF COMMISSION shall mean the state of any apparatus, which has
been taken out of service and is not available for immediate use.
1.2.61 PROXIMITY TESTER shall mean a device which is not required to make
contact with the conductor under test in order to determine whether the
conductor is LIVE or DEAD.
1.2.62 RING MAIN UNIT/PANEL shall mean apparatus comprising two or more
interconnected switches.

Note 2: The term Close Proximity shall mean the breaching of the Safety
Clearance as defined.
1.2.66 SAFETY TESTING shall mean the testing of mains and/or apparatus to
ascertain whether it is alive or dead by means of approved equipment provided
for the purpose.
1.2.68 SHUTTERS shall mean the mechanically operated safety covers for or on
orifices preventing access to live parts of metal clad switchgear.
1.2.69 SUBSTATION shall mean the site on which any transforming, switching or
linking apparatus forming part of the power system is situated. The term
substation includes the associated buildings and control equipment.
1.2.71 SWITCH shall mean a device other than a circuit breaker, designed for
making or breaking a limited flow of electric current. The term includes air
break switches disconnects and similar devices.
1.2.72 SWITCH PANEL shall mean all the apparatus comprising any one circuit of
a switchboard from and including the busbar to the cable.
1.2.75 SWITCHBOARD shall mean two or more switch panels with interconnected
busbars.
1.2.76 SWITCHPILLAR shall mean a device at which two or more circuits can be
connected together within a cubicle (see distribution pillar).
1.2.79 SWITCH YARD shall mean any outdoor enclosed area in which live

conductors or live parts of electrical apparatus working at high voltages are
accessible, but situated in such a position that inadvertent human contact
therewith is not possible from ground level.
1.2.80 SYSTEM shall mean the complete electrical network operated by the DES
(Distribution Electrical Services).
1.2.81 TEST (SAFETY TESTING) shall mean the use of approved equipment to
ascertain if electrical apparatus, conductors or cables are energised or dead.
1.2.83 WARNING NOTICE shall mean any notice provided to show that work is in
progress on mains and/or apparatus and/or indicate any special operating
conditions applicable thereto.
1.2.86 WORK shall mean repair and maintenance work, additions, modifications to
existing equipment, testing and any other work which involves working on
electrical conductors, secondary and control equipment and wiring which form
part of the electrical system.

2.0 WORK IN AND ACCESS TO SUBSTATIONS AND LIVE ENCLOSURES
2.1 No person other than a Competent Person, whether DES employee or other shall
enter, or be required or permitted to enter, any substation or live enclosure unless all
live conductors, which are not adequately insulated against inadvertent contact, are
effectively screened off temporarily. A Competent Person may, however, be assisted
by other persons under his immediate personal supervision, and furthermore, Non
Competent Persons will be permitted to enter and work in substations, or live
enclosures, if under the immediate personal supervision of a Competent Person.
2.2 In order to comply with the foregoing, every Competent Person working in a
substation or live enclosure shall:
2.2.1 Accept responsibility for all persons allocated to assist him in the course of his
duties, and for Non Competent Persons personally supervised by him,
2.2.2 At all times exercise proper control over these persons and issue explicit and
proper instructions,
2.2.3 Ensure that only necessary persons enter a live HV yard or enclosure,
2.2.4 Ensure that all persons under his control are warned of the danger of
inadvertent contact with live conductors and apparatus,
2.2.5 Be responsible after entering such premises for ensuring that no unauthorised
person shall gain access thereto,
2.2.6 Be responsible for the barricading and/or demarcating of any doors or gates
left open during the work period,
2.2.7 Erect temporary barriers to prevent persons coming into inadvertent contact or
close proximity with adjacent live mains/apparatus,
2.2.8 On leaving the substation or live enclosure be responsible for ensuring that the
door or gate is locked and that the key is removed there from,
2.2.9 Where adjacent equipment is live, delimit the section which is set aside for
work to be carried out by the use of approved barriers arranged so that the
Safety Clearances set out in 1.2.63 are complied with.
2.2.10 Delimit the area at ground level and if applicable the structure or apparatus on
which the work is to be carried out.
2.3 If the work cannot be carried out without leaving ground level, or platform or access
ways, the minimum Safety Clearances must still be maintained. (Refer 1.2.63).

2.4 If the work is such that these Safety Clearances are insufficient to avoid danger, other
suitable arrangements shall be made to provide the requisite degree of safety.
2.5 Columns supporting structures carrying live electrical apparatus or conductors shall
not fall within a delimited safe working section. Whenever possible the access path
to the safe working section shall also be delimited by barriers on either side of the
path in accordance with these requirements. If this is not done, persons moving from
the live yard or enclosure entrance to the safe working section, and return, shall do so
under the supervision of a Competent Person.
2.6 When it is impracticable to provide adequate barriers and screens, arrangements shall
be made for a Competent Person/s to watch continuously the men at work to ensure
that they incur no risk.
2.7 Use of portable ladders and long objects where there are exposed live
conductors. (Gantries)
2.7.1 Portable ladders and other long objects shall not be used without the
permission of an Authorised Person who shall define the conditions of use to
the person in charge of the work. The movement and erection of such ladders
shall then be carried out only under the direct supervision of a Competent
Person, and when moved at ground level ladders shall be carried only in a
horizontal position and as near the ground as practicable.
2.7.2 When a ladder is in use an Authorised Person shall supervise all movements
within the area.
2.7.3 When work is to be carried out under a Permit to Work and it involves the use
of a ladder for work on dead electrical conductors, or on non-electrical items
when there are other live electrical conductors in the same substation or other
areas, the above authority to use a ladder shall be given by the Authorised
Person in writing on the Permit to Work.
2.7.4 Ladders must not be kept in substations or other areas where there are exposed
live high voltage conductors unless they are securely locked to a secured
anchorage, and may only be released by an Authorised Person, who shall be
responsible for making sure that the ladders are returned to their storage
positions and locked when work is completed or suspended.
2.7.5 When ladders, cranes or other equipment such as preformed scaffolding are
taken into or out of a substation, the route to be followed shall be agreed by an
Authorised Person. Cranes or other equipment shall be effectively connected
to the substation earthing system as soon as practicable. The limits of
operation of such equipment shall be defined by an Authorised Person and
described to the person in charge of the work and thereafter the equipment
shall be erected or moved only within these limits under the direct supervision
of a Competent Person.

2.8.2 Before being permitted access, the manner and extent to which it is safe to
enter and work shall be explained to, and understood by, the non-employee
who is to be the Person-in-Charge of the work.
2.9 Visitors must be issued with a Permit (see Annexure 3) before being allowed access
to substations and live enclosures and they must be accompanied by a Competent
Person. They will not be required to sign off the completion portion of the permit.
Where a general indemnity form has been signed by the visitor, no permit will be
required.
2.10 Personnel shall ensure that exit from their working or operating position is
unimpeded whilst they are in any substation building or yard. Where operations are
taking place within any switch room, doors at opposite ends of the switch room shall
be open wherever practicable.
2.11 Obstructions created by the placing of materials, clothing, tables, stools and personal
equipment are not permitted.
2.12 Strict precautions shall be taken to ensure that flammable materials such as spilled
oil, compound, cable wrappings, waste, etc. is collected and removed from the
vicinity of substations.
2.13 On the completion of work, the interior and exterior of substations shall be left in a
clean and tidy condition.
2.14 Substations and live enclosures must be kept closed and locked when unattended.

SECTION 3
WORK IN CONFINED SPACES OR ENCLOSURES
3.0 A confined space is any enclosed space, above or below ground, where a hazard to
health may exist due to the lack of oxygen, the presence of a suffocating, toxic or
flammable, or an actual or potential hostile environment. A confined space may
include manholes, vessels, transformer tanks, chambers, oil tank compartments,
substation basements, generator exhaust stacks, water or diesel fuel storage tanks etc.
3.1 NO PERSON shall enter a confined space or enclosure until the degree of oxygen has
been established by utilising an approved monitor, and as the case may be, it has been
adequately cleared of any dangerous liquid or purged with air to clear any dangerous
concentration of gas or fumes by using ventilation fans;
3.2 In the case of vessels, transformer tanks, chambers, etc., at least one other person shall
remain in attendance, outside of, and, next to the entrance thereof whilst any person is
within. Once an access point into a confined space (e.g. manhole) has been created the
area operator shall put a tag holder in place next to the access point. The text on the
Tag Holder is “DO NOT ENTER” in red letters.
3.3 Entry into the confined space by the Permit Holder is not permitted unless the area
operator or Person-in-Charge:
· has taken the gas test including other specified tests in the permit (e.g. H2S) for
every relevant entry point
· has taken all precautions just prior to entry and has found them fulfilled
3.4 Where there is the possibility of the presence of flammable liquids, gases or fumes,
effective precautions must be taken to prevent ignition by the avoidance of open
sources of heat or light and the formation of sparks by ferrous tools, the generation of
static electricity by clothing or by any other means.
3.5 Confined space entry Permit must be issued before accessing the confined space, and
a plan of rescue must be stated on the Permit (Annexure 11).
3.6 A confined space permit may only be issued by a person to other persons who have
been trained and certified to enter confined spaces.

SECTION 4
4.1.1 Odourless, colourless, heavy gas that spreads slowly in the atmosphere,
although non-toxic, will not sustain life.
4.1.4 Although inhalation of a small quantity of the gas is not harmful, the
decomposition by-products are toxic in greater amounts and can be a health
hazard. In excessive quantities over long periods however, unconsciousness or
even death may result from oxygen starvation.
4.2 Persons experiencing any irritation of the skin, eyes, nose or mouth, or who become
aware of a pungent smell or unpleasant odour when in the vicinity of SF6 filled
apparatus, must move away into the fresh air or make use of approved breathing
apparatus and protective clothing.
4.3 When opening up an SF6 filled chamber for the first time after it has been in service,
or when handling molecular sieves or special filters designed to absorb gaseous
breakdown products, approved protective clothing must be worn as described in the
above-mentioned Code of Practice. After use, protective clothing must be properly
washed and de-contaminated.
4.4 As SF6 is a heavy gas, its presence in trenches and basements must always be
considered where this can occur. If there is any doubt, the presence of sufficient
oxygen must be established by using an instrument approved for the purpose.

4.5 Before entering or working in a basement, or an enclosure that has previously been
charged with SF6 gas, the requirements of the System Operating Regulations must be
complied with (hard hats are to be worn on entrance to substation basements).
4.6.1 Where necessary exhaust the gas to atmosphere, away from the equipment
building or enclosure and take special care to ensure that there is no collection
of gas at low level points (e.g. cable trenches or basements). Use forced
ventilation to achieve this where necessary.
4.6.2 If the SF6 gas is to be recovered for re-use. Storage in receivers shall be
effected by means of gas recovery equipment observing the following
requirements:
The weight of SF6 to be stored shall be less than 1/40 of the volume of the
receiver (expressed in cubic inches)
4.6.3 Fill the compartment with compressed air and drain to atmosphere, repeat 2 or
3 times, for effective removal of any entrapped gas. In compartments where no
electric arcing has taken place previously (i.e. containing pure SF6) forced
ventilation of the compartment is sufficient.
4.6.4 Open the compartment and use a vacuum cleaner to remove any powdery
residues of gas decomposition (metal Fluorides and Sulphides). Goggles,
protective clothing and respirators (if necessary) shall be worn in this
operation.
4.6.5 Before entering compartment, check for gas free condition by one of the
following methods:
4.6.5.2. Drain gas from the lowest point of compartment into a glass jar.
Insert a burning flame into the glass jar. If gas is present the flame
will be extinguished.
4.6.6 Whilst work proceeds inside the compartment, standby help shall be available
at all times in case immediate assistance is required. Rescue ropes and lifting
tackle shall be provided for this purpose where necessary.

Due to the modular design of GIS switchgear and in particular, the segregated gas
compartments, special safety measures are necessary prior to carrying out work on
one or more of these gas compartments:
4.7.1 Prior to either the topping up or evacuation of gas from any GIS compartment,
the correct isolation of all electrical conductors or apparatus contained within
the compartment shall be carried out prior to the gas work commencing.
4.7.3 Where the work on a gas compartment is restricted to the topping up of gas
only, it is not necessary to electrically isolate the adjoining gas compartments.

5.2 No work shall be performed on the automatic or remote controlling equipment whilst
work is in progress on the controlled equipment.
5.3 Whenever “Danger Do Not Make Alive” labels have to be hung in order to comply with
these System Operating Regulations, such a label shall be hung at each point at which the
equipment can be operated or controlled, with the exclusion of SCADA control
equipment operated by a System Control Operator in a control centre.

SECTION 6
6.2 Cables spiked in error must be reported to the Deputy Director immediately.
6.3 If there is any doubt about the identity of the cable, the matter must be referred to the
Supervisor before the cut is made.
6.4 Subject to the provisions of these System Operating Regulations, the cutting of a
cable that has been laid and/or installed, or the completion of the cutting thereof may
only be done by, or under the constant personal supervision of an Authorised Person,
a Competent Person or the Person-in-Charge.
6.5 Subsequent cutting of a previously cut cable may be done without following the
procedures laid down in Section 6.1, provided that this is done by, or under the
constant personal supervision of the Person-in-Charge, and provided further that the
cut end of the cable is in sight, and the cable is entirely exposed, and can be
physically traced from that end to the point of cut.

SECTION 7
SUBSTATION KEYS
7.1 Substation keys may only be issued to Competent Persons who are competent to enter
substations and live enclosures.
7.2 The Deputy Director is responsible for the issue of all substation keys.
7.3 Keys will only be issued on written request of the Section Head of the person
concerned and keys may not be lent or handed over to Non Competent Persons.
7.4 Keys shall only be issued to competent non-employees who are in possession of a
letter of authority issued by the H.O.D. Transmission and Distribution or who are
permitted to work in an area in a substation or live enclosure that has been made safe
without the constant watch of a Competent Person.
7.5 Any person to whom a substation key has been issued is responsible for the
safekeeping of that key and shall return it on demand on leaving the service or when it
is no longer required, to the Deputy Director.
7.6 The loss of a key shall be reported as soon as possible to the Deputy Director.

SECTION 8
USE OF MV/HV MEASURING RODS
8.1 Only approved measuring rods, link sticks, telescopic sticks or electronic measuring
instruments may be used for measuring the heights of live overhead conductors.
8.2 Telescopic devices must be fully rated for the voltage on which they are to be used.
Those of the telescopic type must be sufficiently extended to comply with their
voltage rating.
8.4 Telescopic sticks may be extended section by section in the vertical position. The
stick should then be marked and the number of extended sections counted. The stick
should then be collapsed and extended again in the horizontal position when the
measurement should be taken.
8.5 Under no circumstances may measuring tapes or other items be attached to the rod or
link stick when the measurement is taken. (Most linen measuring tapes have metallic
threads running through them and are conductive).
8.6 Measuring rods and link sticks must be in good condition and must be cleaned and
examined before use.
8.7 Only Authorised Persons may use MV/HV measuring rods and sticks and then only
for those voltages for which they are authorised, unless exempted in writing by the
H.O.D. Transmission and Distribution.

SECTION 9
MV/HV OPERATING PROCEDURE
9.1.1 In the case of 66kV or 11kV, switching authorised by the Senior Engineer:
Transmission and Distribution Operation or, in his absence, the Head of
Department, Transmission and Distribution.
9.1.2 Any Emergency Switching where any delay in switching may lead to
catastrophic consequences, such as injury or death to personnel or damage to
the Power System.
9.3 Any person carrying out work on the Utility's MV/HV system or, entering the Utility's
premises in which there are high or medium voltage apparatus, shall ensure that he
has a means of communication with the System Control Operator on duty at all times,
e.g. by portable hand phone.
9.4 Immediately before any switching, isolating, testing and earthing is commenced on
MV/HV mains and/or apparatus, the System Control Operator shall be notified and
the Authorised Person carrying out the operations shall confirm that he is at the
correct operating location.
9.5 The Authorised Person shall carry out switching operations in liaison with the System
Control Operator and in compliance with clause 9.6 below. The Authorised Person
shall without undue delay, inform the System Control Operator of the completion of
each operation, or set of operations.
9.6 All communication from and messages to the System Control Operator relating to the
operation of apparatus shall be given in detail by the Authorised Person, without
abbreviation, stating the location, name of the apparatus and the nature of the
operation as is specified on the approved switching programme where applicable. The
System Control Operator shall repeat it in similar detail, and the Authorised Person
shall confirm the correctness of the repetition.

9.7.1 They must be familiar with the System Operating Regulations and
specifically the safe operation of MV/HV mains and/or apparatus.
9.7.2 They must verify that the Authorised Person is at the correct isolation/earthing
device.
9.7.3 They must be conversant with the switching and/or earthing operations to be
carried out.
9.7.4 They must observe that the switching operations are carried out in the correct
sequence and in accordance with the approved switching programme.
9.7.6 They must be conversant with emergency and first aid procedures.
9.8 On any one occasion, only one Authorised Person shall be responsible for all
switching, isolating, testing and earthing at a control point.
9.9 In the case of DES control points interconnected with a BPMC substation,
switching, isolating, testing and earthing must be performed by a DES employee, but
must be witnessed by a BPMC Authorised Person. The earthing must be inspected by
the Authorised Person and found to be satisfactory in terms of these regulations. The
isolation, testing and earthing of DES control points must be communicated back to
the System Control Operator by the Authorised Person who witnessed such
operations.
9.10 Whenever MV/HV mains and/or apparatus are to be earthed, the circuit must be
isolated at each source of supply before testing and earthing, or where testing is not
possible (RMU’s), earthing only is carried out. Upon completion the Authorised
Person shall inform the System Control Operator accordingly.
9.11 The foregoing procedure is to be continued until all the operations necessary to
switch, isolate, test and earth the mains and/or apparatus to be worked on, have been
completed.
9.12 Except in the case of emergency switching, the Authorised Person shall complete a
switching request form indicating the designation of the mains and/or apparatus
required to be taken out of service and shall submit this to the System Control
Operator.

9.12.1 A minimum of 48 hour notice is required by the System Control Operator for
planned work. Requests for operating procedures to be carried out for planned
work in instances where switching request forms have not been submitted in
advance, may only be made by the Senior Engineer: Transmission and
Distribution Operations or a more senior official.
9.12.2 The Authorised Person shall confirm with the System Control Operator that
his request has been granted and ascertain whether there are any special
conditions applicable to the granting of this request.
9.12.3 Should a request not be granted, the System Control Operator shall advise the
Authorised Person without undue delay, the work shall then be re-scheduled to
a mutually acceptable date and time.
9.13 It is the responsibility of the Authorised Person to return the switching to normal in
liaison with the System Control Operator and in accordance with the approved
switching programme.

SECTION 10
10.1.2 Keeping a record of the time and details of all operations carried out in liaison
with the Authorised Person in the approved recording system in the Control
Centre.
10.1.3 Keeping a record of each Permit to Work issued, which includes the name of
the Authorised Person, the serial number, and the mains and/or apparatus
concerned.
10.1.4 Keeping a record of earths applied at control points at all sources of supply.
10.1.5 Keeping a record of the times and details of emergency switching together with
the names of persons who have carried out these operations and the reasons
therefore.
10.1.6 The updating of the system diagrams in the control rooms in such a way as to
indicate at all times the state of the system, positions of control point earths
throughout the system and the names of the persons to whom mains and/or
apparatus have been handed out. The operating position of all spare circuit
breakers/apparatus shall also be indicated.
10.1.7 Giving to the relieving System Control Operator all information as to the state
of the system as may be necessary to ensure the proper discharge of his duties.
It shall also be the duty of the System Control Operator taking over to obtain
this information and to familiarise himself with the relevant entries in the
records.
10.2.1 No routine switching, isolating, testing and control point earthing operations on
the MV/HV system shall be carried out at any point without informing the
System Control Operator. The person authorised to carry out such operating
shall not delegate his authority to any other person, provided that Trainee
Authorised Persons may switch, isolate, test and apply earths under the direct
personal supervision of the Authorised Person who shall remain fully
responsible for the operations.

10.2.2 Competent persons may apply personal working earths on overhead lines only,
or may assist the Authorised Person in doing so.
10.2.3 Whenever MV/HV mains and/or apparatus are to be earthed at control points,
the Authorised Person shall “test” before earthing”, provided that testing is
possible before earthing.
10.2.4 When it is required to energize any mains and/or apparatus that have been
isolated and earthed, Authorised Persons shall verify by personal inspection of
the points at which the mains and/or apparatus are earthed and then in liaison
with the System Control Operator remove the control point earths.
10.3.1 System Control operations relating to routine switching, isolating, testing and
earthing operations shall be given in detail by the Authorised Person without
abbreviation, stating the location, name of the apparatus and the nature of the
operation. The Control Operator shall repeat it in similar detail, and the
Authorised Person shall confirm the correctness of the repetition. The
completion of any operation, or set of operations, shall be reported without
undue delay to the System Control Operator.
10.3.2 Subject to the provisions of the Section dealing with “MV/HV Operating
Procedure” all operating procedures shall be performed in their correct
sequence and no operation, or set of operations shall be carried out by the
Authorised Person until the preceding operation, or set of operations have been
completed.
The system diagram in the control centre shall be altered, where applicable on, the
completion of any operation, or set of operations, by the System Control Operator,
and such operation, or set of operations, shall not be deemed to be completed until the
system diagram has been altered to show the correct operating conditions of the mains
and/or apparatus affected.

SECTION 11
11.1 Work shall not be performed within Safety Clearances of MV/HV mains and/or
apparatus unless such mains and/or apparatus have been isolated and earthed and a
Permit to Work has been issued.
Persons-in-Charge shall understand that they have a responsibility for both their own
personal safety and that of others working with them, and shall decline to accept the
Permit to Work unless entirely satisfied.
11.2 Personal working earths shall be applied to, and retained on, all overhead conductors
as an additional safety measure by the Person-in-Charge when working at any
position remote from the control point earth applied by the Authorised Person.
Personal working earths may only be applied to items of equipment other than
overhead conductors (i.e. busbars) on the written instruction of the Authorised Person
as recorded on the Permit to Work.
11.3.4 Minor repairs to the external parts of transformers and switchgear and
ancillary substation equipment that can be performed safely and without
infringing Safety Clearances.
11.4 An Authorised Person shall comply with the following when carrying out isolation,
testing and earthing on MV/HV mains and/or apparatus:
11.4.2 The mains and/or apparatus must wherever possible be tested using
appropriate approved testing equipment in order to ensure that the mains
and/or apparatus are dead.

11.4.3 The mains and/or apparatus must be earthed in accordance with Section 13 -
Safety Earthing.
11.4.4 The earthing equipment at the control point/s must be left on the mains and/or
apparatus until the work is completed, provided that if it is necessary to
remove earthing equipment at the control points to carry out testing, including
fault location, continuity, insulation, pressure testing, phasing and sheath
testing, or any other work that cannot be performed with the earthing applied,
this must be communicated to the System Control Operator who shall record
the removal of earths. All associated safety procedures shall be observed and
the earthing is to be re-applied immediately after completion of the work.
11.4.5 “Danger Do Not Remove Earth” notices must be applied at control points that
have been isolated and earthed, and “Danger Do Not Make Alive” notices
shall be applied at control points that have been isolated only, at sources of
supply.
11.4.6 Personal padlocks must be applied at control points that have been isolated
and earthed, or isolated only, at sources of supply, if locking facilities exist.
11.4.7 Control points at private consumer substations, which have been isolated and
earthed by the consumer’s staff, shall be checked by an Authorised Person to
ensure that the operations have been correctly executed in terms of these
System Operating Regulations, and if the Authorised Person is satisfied,
“Danger Do Not Remove Earth” or “Danger Do Not Make Alive” notices and
personal padlocks shall be applied.
11.4.9 In the case of CABLES, in the presence of the Competent Person, identify the
cable at the work place by means of approved cable identification equipment
or other approved means if cable identification equipment cannot be used,
provided that this requirement may be omitted when the cable can be
physically traced throughout its length from a control point at which it has
already been isolated and earthed, to the position where it is intended to work
on the cable. Thereafter the Authorised Person shall prove the cable dead at
the work place by the application of approved cable spiking equipment.

11.7 Personal working earths shall be applied to, and retained on, all overhead conductors
as an additional safety measure by employees or contractors when working at any
position remote from the other approved earthing equipment applied by the
Authorised Person. Personal working earths shall be placed in such a position that it
is visible from and applied as close as possible to the point of work.
11.8 In instances where the circuit has to be broken during the course of the work, steps
must be taken to ensure that both sides of the circuit remain earthed at the workplace.
If the temporary removal of the earths is unavoidable in order to carry out the work,
the earths must be replaced as soon as circumstances permit.
11.9 In instances where access to substation basements are necessary, safety hard hats shall
be worn at all times.

When work is carried out on an isolator, both sides of the isolator must be isolated
from all live conductors by opening other isolators and both sides of the isolator must
be earthed
A busbar must be isolated from all live Conductors and earthed before any work may
be carried out on the busbar.
For switchgear equipped with draw-out type circuit breakers, ALL circuit breakers
which can be connected to the busbar must be withdrawn from their cubicles and the
busbar earthed before any work is carried out on the busbar. All circuit busbar
shutters shall be locked off prior to any work being carried out on the busbars.
11.14.1 Programme
The Authorised Person responsible shall satisfy himself that the live
conductors are isolated, discharged and earthed and all conductors are
bonded together by means of a portable earthing device at a point on
each side of the section of line on which work is to be carried out. All
earthing points shall be visible from the point at which work is to be
carried out.

In the event of the approach of a lightning storm, all work shall cease
immediately and the Authorised Person and/or Supervisor shall be
informed as soon as possible. No person shall work on or patrol an
overhead line when lightning strokes are probable.

SECTION 12
SWITCHGEAR
12.1 Whenever work, including the insertion of test leads, is to be performed on the spouts
of MV/HV switchgear, or a busbar chamber, or other chamber containing MV/HV
conductors, is to be opened:-
12.1.1 The person doing the work on the spouts, or opening of the chambers, shall
ensure that the whole switchboard has been isolated and earthed, and that all
sources of supply have been isolated, earthed and locked off, and danger labels
have been applied, and shall be issued with a Permit to Work.
12.1.2 Before commencing work the Authorised Person and Persons-in-Charge shall,
by observation and examination familiarise themselves with the switchboard
or portion of the switchboard to be worked on.
12.1.3 The Authorised Person shall comply with the prescribed procedures for work
on MV/HV mains and/or apparatus as well as the following requirements:-
12.1.3.1 Shutters of live spouts shall be locked in the shut position. They
may be temporarily unlocked during live cable phasing or testing
and thereafter immediately relocked.
12.1.3.3 Spout contacts shall be earthed with equipment provided for the
purpose at the point of work and at all points of isolation from the
supply.

Rubber gauntlets shall also be worn when test prods are inserted
into or withdrawn from a circuit that remains earthed.
12.2 Only approved earthing equipment supplied for use with the switchgear shall be used
for earthing busbars and circuits.
12.3 The insertion of the hand, any tool or lead into contact spouts for this purpose is
forbidden.
12.4 Whenever a switch truck, or the withdrawable portion of switchgear, is removed from
its housing, the shutters of all spouts whose contacts are live, or have not been
earthed, must be locked off. If a Permit to Work has not been issued for the circuit,
universal padlocks may be used.
12.5 Work can be performed on a withdrawable circuit breaker, which has been
disconnected from every source of supply and withdrawn from its normal live
position without such apparatus having to be earthed. It is not necessary to issue a
Permit to Work for work to be performed on such apparatus under these
circumstances.
12.6 Spare MV circuit breakers and switches must normally be left in the isolated position.
They may be left in the circuit earth position if this is essential.
Shutters must be locked off and notices applied indicating the reason.
Spare MV circuit breakers and switches must be shown on all the circuit schematic
diagrams

SECTION 13
SAFETY EARTHING
13.1.3 Verify that the circuit to be earthed is dead by testing each phase.
13.1.5 The earthing leads shall be connected first to the earth system before being
secured to the phases. They shall only be secured to the phases by means of
an insulated rod of an approved type and voltage rating. Care must be taken to
ensure good metal to metal contact is made.
13.1.6 All phases shall be earthed, even if work is to be carried out on only one
phase.
13.1.7 Where possible, a circuit breaker or an earthing switch with adequate fault
making capacity shall be used to make the final earth connection. After
closing, steps shall be taken to lock off any means of tripping or opening the
circuit breaker or earthing switch.
13.1.8 When removing earth leads, they shall be disconnected first from the phase
conductors and then from the earth system.
The System Control Operator on duty shall record in his logbook the time of
application and the location of Control Point Earth connections. Thereafter he shall
record the time of removal and reapplication of control point earths.
13.3 The following minimum earthing requirements of MV/HV mains and/or apparatus on
or in close proximity to which work is to be performed will apply:
13.3.1 Except in the case of underground cables a minimum of one Control Point
Earth is required and shall be affixed to mains and/or apparatus. Should local
circumstances require it, the Authorised Person shall apply Personal Working
earths.

13.3.2 Mains and/or apparatus must be earthed at a control point at each source of
supply or as follows.
13.3.2.1 On an overhead line having more than one source of supply, and
having fixed earthing switches or devices, the requirements of
Sub-section 13.3.1 and 13.3.2 will be deemed to have been met if:
13.3.2.4 A control point at each source of supply is isolated and locked off
and “Danger Do Not Make Alive” labels are applied.
13.3.3 When earthing a busbar the requirements of 13.3.2 will be deemed to have
been satisfied if all the following have been complied with:
13.3.3.1 At least one earth has been applied at a control point, and a
“Danger Do Not Remove Earth” label has been affixed.
13.3.3.2 All sources of supply have been isolated and locked off.
13.3.3.4 Where applicable, all busbar spouts have been locked off.
13.3.3.5 A “Danger Do Not Make Alive” label has been applied at each
control point.
13.3.4 All medium/high voltage windings of a transformer shall be earthed from the
nearest accessible point.
13.3.5 If on an exposed MV/HV conductor system an earth is not visible from the
workplace, additional earthing equipment, which is clearly visible, must be
applied.
13.4 The Authorised Person/Person-in-Charge shall be responsible for ensuring that only
approved appropriate portable earthing equipment is applied to mains and/or
apparatus and that such equipment is of adequate cross section and in good working
order and for ensuring its removal before the circuit is made alive.
13.5 It is the responsibility of the Authorised Person to confirm that all the working earths
have been removed before accepting back a Permit to Work after the work has been
completed.

SECTION 14
When pressure tests, fault location tests or tests of a specialised nature carried out by the
Person-in-Charge, are performed on mains and/or apparatus that are connected to the MV/HV
system all applicable requirements of the System Operating Regulations shall be observed.
14.1 The Person-in-Charge of the test shall be a Competent Person who has been deemed
competent in test procedures.
14.2 The Authorised Person requesting the test shall ensure that:
14.2.1 The mains and/or apparatus are in a condition ready for testing,
14.2.2 All personnel in his charge are clear and have been warned that a test is to be
applied.
14.2.3 All Permits to Work issued in respect of the mains and/or apparatus have been
withdrawn.
14.3 The Authorised Person shall undertake the switching, isolating and earthing of the
mains and/or apparatus at all control points.
14.4 The Person-in-Charge of the test shall accompany the Authorised Person to all the
control points, and:
14.4.1 Satisfy himself that the mains and/or apparatus have been isolated and earthed
at each control point;
14.4.2 Satisfy himself that personal padlocks have been applied to switchgear and
earthing equipment in order to prevent unauthorised operation, and all
busbar and voltage transformer shutters associated with the mains and/or
apparatus have been locked in the closed position;
14.4.3 Ensure that a personal “Danger Do Not Make Alive” notice has been
attached at each control point, and shall himself apply a personal “Danger
Test In Progress” notice at each control point; and
14.4.4 On completion of his inspection of the safety measures taken at the control
points, indicate his satisfaction to the Authorised Person by signing a Permit
to Work/Test.

14.5 The Authorised Person shall then issue the Permit to Work/Test to the Person-in-
Charge of the work and/or test, and hand him the keys to the padlocks applied at the
control points.
14.6 After accepting the Permit to Work/Test the Person-in-Charge of the test shall be
responsible for ensuring that all necessary safety pre-cautions are taken during the test
in regard to personnel and equipment, and that all exposed conductors are suitably
guarded and labelled. The Person-in-Charge of the test shall be responsible for the
safety of all persons assisting him.
14.7 The removal of earths in order to allow the test to proceed shall only be done by the
Authorised Person under the personal supervision of the Person-in-Charge of the test.
This must be communicated to the System Control Operator who will record the
removal of the control point earth.
Conductors and spout contacts must be safety tested using an approved tester before
being touched, after the earth has been removed.
14.8 Whenever it is necessary for work to be done on the spouts of switchgear, including
the insertion of test prods, this shall be done under the supervision of an Authorised
Person in accordance with Section 12 - Switchgear.
14.9 On completion, the Person-in-Charge of the test shall earth any conductors to which a
test voltage had been applied, and shall then remove his “Danger - Test in Progress”
notices from all control points. He shall then sign off and return the Permit to
Work/Test to the Authorized Person, and hand back the keys. He shall also ensure that
the mains and/or apparatus are left in the same condition in which they were handed
to him.
14.10 The Authorised Person shall then cancel the Permit to Work/Test.
14.11.1 General
High Voltage apparatus shall not be connected to the Utility's system until it
has passed tests approved by the Utility and until any associated protective
gear has been proven to function properly.
14.11.2 Permit
Earthing conductors shall not be removed for testing purposes until all
Permits to Work relating to the apparatus have been cancelled and the
Authorised Person has sanctioned their removal. A Permit to Work/Test
shall be issued by the Authorised Person responsible for testing and with the
System Control Operator who shall record this in his logbook.

14.11.3 Safety
High Voltage tests shall be carried out under the direct supervision of an
Authorised Person who shall be responsible for guarding the apparatus to be
tested, the test set and temporary high voltage conductors used for testing by
properly placed “Danger Notices”, screens and barriers to prevent danger.
“Danger Notices” shall be placed in prominent positions. The “Danger
Notices”, screens and barriers shall not be removed until all tests have been
completed and the apparatus which was tested has been discharged and
earthed. All apparatus shall be earthed before and after the application of
high voltage for testing.
When High Voltage leads are used for phasing out, the whole of the work of
connecting the leads, phasing out procedure and removal of the leads shall
be carried out under the direct supervision of an Authorised Person who
shall be solely responsible for the safety of personnel and equipment
throughout the entire operation.

SECTION 15
EQUIPMENT
Whenever the cause of the controlling switch of an overhead line tripping or locking out is
known and it is safe to re-energise, a reclosure must be effected. If the cause is unknown the
following procedure shall be adopted.
15.1 Where a line trip has occurred, the System Control Operator shall attempt to verify
whether there was lighting activity in the vicinity of the line by contacting the security
guards at either end of the line or, in the case of lines terminating at a power station.
The Shift Superintendents or other employees working or residing in the area
concerned should also be contacted if necessary.
15.2 The System Control Operator shall then contact the relevant official in charge, who
may be the 66kV Standby Person (after hours) or, the H.O.D. Transmission and
Distribution, or his nominee (normal hours), informing him of the line trip and
whether lightning activity has been confirmed in the vicinity of the line or not. The
System Control Operator shall also confirm that the fault recordings from both ends
of the circuit have been uploaded to SCADA.
15.3 Where lightning activity has been confirmed and the relevant official has satisfied
himself that lightning is the most likely cause of the trip, he shall instruct the System
Control Operator to re-energise the circuit. In the case of a double circuit trip where
lightning is the most likely cause of outage. The second circuit shall be re-energised
following the successful re-energising of the first circuit.
15.4 In the event of a double circuit trip where it has been determined that lightning is the
most likely cause, and that re-energising of the first circuit has been unsuccessful, an
attempt to re-energise the second circuit is permissible.
15.5 In the event of either a single or double circuit trip where there has been no apparent
lightning activity, the relevant official shall arrange for the line to be inspected prior
to the re-energising of the circuit, except where the conditions described under clause
15.6 prevail.
Note: Reasons for the protection trip such as fires beneath the lines, monkeys, birds,
mechanical failure or other possible causes should be strongly suspected and all
reasonable attempts to establish the cause(s) should be made. Should there be any
doubt as to whether the line can be safely re-energised no attempt shall be made to do
so.

15.6 In situations where single or double circuit trips occur in the absence of lightning
activity and this together with the prevailing network configuration presents an
unacceptable risk to the security of supply of the power system, the 66kV Standby
Person shall consult with the H.O.D. Transmission and Distribution or his nominee,
who shall then decide whether the line can be re-energised without prior inspection in
order to mitigate further risk to the security of supply.
15.7 Following each occasion where a circuit(s) of an OHTL has tripped and has been
successfully re-energised without prior inspection, Transmission and Distribution
staff shall inspect the line for damage at the earliest possible opportunity.
15.8.3 The onus is on the man in the field to ensure that all of the required overhead
line is patrolled and checked.
15.8.4 If there is any doubt as to what should be patrolled and checked, the complete
overhead line plus any terminating equipment must be patrolled and checked.
15.9 GENERAL
15.9.1 Whenever one line of a double circuit transmission tower system is isolated
and earthed to allow work to be performed on it, should the live line trip out
automatically no attempt shall be made to re-energise it until all personnel are
clear of the tower.

SECTION 16
16.1 A Permit to Work for MV/HV mains and/or apparatus (Annexure 1) may only be
issued by an Authorised Person who shall:
16.1.1 Familiarise himself with the switching requirements of the job, prior to doing
any switching.
16.1.2 Ensure that there is a Competent Person present to witness switching when
earthing of mains and/or apparatus is required.
16.1.3 Liaise with the System Control Operators keeping them informed of switching
movements and Permit to Work details.
16.1.5 Apply personal padlocks and appropriate labels to all Control Point Earths
(CPEs).
16.1.6 Prepare the Permit to Work once all isolating, testing and earthing has been
completed.
16.1.7 Give explicit instructions to the Person-in-Charge of the job and ensure that
these instructions are understood.
16.1.8 Issue the Permit to Work once these instructions have been understood,
ensuring that all parties fully and correctly complete the Permit to Work.
16.1.9 Remove the original page of the Permit to Work and hand it, together with the
keys of the CPE’s to the Person-in-Charge of the job.
16.2 The Person-in-Charge at the workplace, shall before accepting the Permit to Work/Test
ensure that the work to be undertaken and the safe limits of the workplace are fully
understood, and shall:
16.2.1 Satisfy, himself that all isolating, testing and earthing has been carried out and
that the mains and/or apparatus is safe to work on.
16.2.2 Ensure that the underground mains have been identified and proved dead at the
workplace subject to the provisions of Clause 11.5.2 of section 11 if applicable.

16.2.4 The keys to padlocks applied to control points shall be handed to a Person-in-
Charge or retained in a safe place until all Permits to Work have been signed off
and the mains and/or apparatus are handed back to the Authorised Person
16.3 A Permit to Work shall not be issued to more than three Persons-in-Charge and their
crews at a work place.
16.4 When the Authorised Person is also the Person-in-Charge at a work place, he shall
make out a Permit to Work for himself, obtain the signature of the Competent Person
where necessary, and sign as a Person-in-Charge. He must also obtain the signatures
of other Persons-in-Charge at the workplace as appropriate.
16.5 On completion of the work the Persons-in-Charge of each crew shall satisfy
themselves that all the men of their crews are clear of the mains and/or apparatus, and
have removed any personal working earths applied by them before removing any
personal working earths they themselves have applied to the conductors. After
removal of their own personal working earths, the Person-in-Charge shall sign and
return the completed Permit to Work to the Authorised Person, and return any keys
that they may have held.
16.6 On return of the completed Permit to Work the Authorised Person who issued the
Permit to Work/Test is responsible for obtaining confirmation from the Persons-in-
Charge that all persons under their control are clear of the mains and/or apparatus and
that all personal working earths applied by crews working on overhead conductors
have also been removed. Thereafter the Authorised Person shall remove all earthing
equipment that he himself has applied at any work place, and in addition any “Danger
Do Not Make Alive” and/or “Danger Do Not Remove Earth” notices and personal
padlocks that were applied by him.
16.7 The Authorised Person shall, if requested, in liaison with the System Control Operator
re-commission the mains and/or apparatus and return all switching to normal.
This Permit to Work on/Test is to be used for the purpose of cable jointing in order to
facilitate the phasing out of MV/HV cables before the jointing procedure is to be
completed.
16.8.1 The Person-in-Charge and his assistants will cease work and leave the work
area. The Person-in-Charge shall then sign the sanction for test and the
Authorised Person shall then commence the testing procedure.

16.8.2 After the testing has been completed and any earths removed during testing
have been re-applied, the Person-in-Charge shall sign in agreement that it is
safe to complete the work on the cable.
16.8.3 On completion of the work, the Permit to Work on/Test MV/HV cables shall
be cleared by the Person-in-Charge and thereafter cancelled by the Authorised
Person.
16.9 Unusual circumstances, which may arise in the completion of Permits to Work.
16.9.2 Where the Authorised Person who issued the Permit to Work is not
available to receive back the Permit to Work and keys from the Person-
in-Charge, the Engineer in Charge or H.O.D. Transmission and
Distribution shall designate another Authorised Person to accept the
Permit to Work and keys and complete the procedure for handing back
the mains and/or apparatus to the System Control Operator.

SECTION 17
Whenever a Cable fault occurs between the Department of Electrical Services (DES)
primary distribution substation and the downstream main 11kV Berakas Power
System (BPS) switchboard, the following procedure shall be adhered to:
17.1.1 The BPMC System Control Operator on duty shall inform the DES that a
system fault affecting the DES distribution network has occurred.
17.1.2 BPMC Authorized staff shall isolate, lock-off and label the affected circuit
breaker at the BPS substation.
17.1.3 When the DES is ready to perform the cable fault location, they shall contact
BPMC and both parties will nominate an Authorised Person as the Person-in-
Charge of the test. The respective Persons-in-Charge shall be responsible for
ensuring that the cable fault location is conducted in a safe and proper manner
and in accordance with the respective DES and BPMC System Operating
Regulations.
17.1.4 Access to BPS substations by DES staff and DES Contractors shall only be
permitted if both the DES and BPMC Persons-in-Charge are in attendance and
that the required Non-Employees Permit (Annexure 3), allowing access for the
purposes of conducting a cable fault location has been issued to the DES
Person-in-Charge by the BPMC Person-in-Charge.
17.1.5 Prior to conducting any testing, the DES and BPMC Persons-in-Charge shall
jointly confirm that isolation, locking off and labeling of the circuit has been
carried out at both ends of the cable.
17.1.6 The DES Person-in-Charge shall then witness the testing, earthing and locking
off (with personal padlock) of the circuit at the BPS end of the cable by the
BPMC Person-in-Charge. Appropriate labels shall be affixed to the panel upon
completion of the earthing. The keys to the personal padlocks for the Control
Point Earth (CPE) shall be handed to the DES Person-in-Charge. The BPMC
Person-in-Charge shall then issue a Notice of Isolation of Electricity Supply
Mains (Annexure 2) to the DES Person-in-Charge.

17.1.7 The DES Person-in-Charge shall then assume responsibility for the circuit and
shall carry out the fault location from the DES control point using DES
equipment.
17.1.8 During testing, the BPMC Person-in-Charge shall remove and re-apply the
CPE at the BPS end of the cable at the request of the DES Person-in-Charge as
required. All operations at the BPS end of the cable are to be witnessed by the
DES Person-in-Charge or his designated representative, providing that such a
person has been included as assisting the DES Person-in-Charge on the access
Permit.
17.1.9 In the event that it becomes necessary to conduct the test from the BPS end of
the cable, the following procedure shall be adhered to:
Note: In addition to the BPS Control Point, the DES Control Point
must be included on the Permit and as such it shall be treated both as
a normal Control Point as defined in these Operating Regulations,
and in terms of the Control Point requirements for the isolation,
testing, earthing and the issuing of a Permit to Test.
17.1.9.3 The BPMC Person-in-Charge shall remove the CPE at the BPS end
of the cable to allow the DES to conduct the fault location test. The
DES shall then proceed to locate the cable fault from the BPS end of
the cable with the BPMC Person-in-Charge in attendance.
17.1.9.5 The BPMC Person-in-Charge shall then cancel the Permit to Test as
well as the Non-Employee’s Permit.

17.1.10 After completion of the cable repair the following procedure shall be
adhered to:
17.2.1 The DES shall submit a copy of all signed test reports/setting reports for the
new substation to BPMC Engineering and Planning for verification. The
report shall be retained and filed in the BPMC library.
17.2.2 The control sheet for commissioning of the new DES substation shall be
completed by BPMC Engineering and Planning and handed over to BPMC
Operations and Maintenance prior to commissioning.
17.2.3 The DES shall inform BPMC of the proposed switch on date of the new DES
substation by submitting form BPMC/DES/TS/*** to BPMC Engineering and
Planning, who shall then inform BPMC Operations and Maintenance section.
17.2.4 BPMC Operations and Maintenance shall arrange for the necessary Switching
Program and shall co-ordinate the shutdown for the commissioning of the new
DES substation with the responsible DES staff.

17.2.5 All switching operations shall be done in accordance with the approved
BPMC Switching Program. The DES shall be responsible for all switching,
testing and phasing on the DES equipment and similarly, BPMC shall be
responsible for the equivalent operations on the BPS equipment.
17.2.6 After witnessing the isolation, locking off and labeling of the cable at the DES
Control Point, the BPMC Person-in-Charge shall isolate the circuit breaker at
the relevant BPS substation and with the DES Person-in-Charge in attendance,
shall test and apply the Control Point Earth (CPE).

SECTION 18
18.1 The following HV/MV Mains and/or apparatus may only be energised by the
following, or more senior, Authorised persons:
18.1.1 Distribution overhead and underground Mains rated up to 11 000 volts:
Engineer in Charge.
18.1.2 Transmission Mains and apparatus rated at 66 000 volts: H.O.D. Transmission
and Distribution.
18.1.3 Power Station Plant and equipment rated at 11 000 volts and above: H.O.D.
Transmission and Distribution.
18.2 Officials energising mains and/or apparatus shall ensure that:
18.2.2 The System Control Operator has been notified and that all necessary details
have been furnished to enable the Control Room diagram to be updated in
advance.
18.2.3 Protection devices are correctly set or have been set by appropriate officials
of Engineering Division.
18.2.5 All spare circuit breakers and switches are isolated and have been labelled
“spare”, associated shutters and operating handles are locked off and that
danger notices are applied.
18.2.7 All covers are in place, and any exposed MV/HV conductors have been
earthed in accordance with Section 13.
18.2.8 No items are missing that are necessary for the safe and proper functioning
of energised equipment.
18.2.9 Where checklists are available that these have been properly completed prior
to commissioning.
18.2.10 Persons in the vicinity are safe and clear of the mains and/or apparatus.
18.2.11 Legal clearances of overhead lines have been checked and found correct.
18.3 In addition the Authorised Person must inspect the mains and/or apparatus and
surrounds to ensure that there are no obvious hazards, and that all operating spaces
and passages are clear.

SECTION 19
In the event of a fire within the Power Station, the specified officials must follow the
following course of action:
19.1.1 Ensure that all staff evacuates the area and that everyone is accounted for.
19.1.2 Initiate the fire alarm if it hasn’t already been initiated through the automatic
fire detection system.
19.1.3 Inform Control Centre Immediately.
19.1.4 Inform the Guard House to allow for entry of the Fire Engine, Ambulance etc.
19.1.5 In the event that the fire is on a gas supply line, change-over to diesel fuel, if
possible, before tripping Emergency Shutdown Valve (ESV)
19.1.6 If possible, extinguish the fire with portable extinguishers or the hose reel.
19.1.7 After the fire has been extinguished, inspect and replace fire-fighting
equipment.
19.1.8 Submit a report to the Deputy Director.
Action by SCADA Control
19.1.9 After receiving the information from the Shift-in-Charge, the System Control
Operator shall call the Fire Brigade (Bomba) by dialing 995. When calling the
Fire Brigade:

19.1.10 Carry out electrical isolation of any affected equipment and field staff to
be deployed if and when necessary.
In the event of a substation fire, the field staff on site must follow the following
course of action:
19.2.1 Ensure that all staff evacuates the substation building and that everyone is
accounted for.
19.2.3 For fires involving electrical equipment and in liaison with Control Centre,
carry out the electrical isolation of affected equipment. If access to equipment
is dangerous due to fire and smoke, isolation must be carried out on the
upstream circuit breakers.
If the fire is small and can be safely contained using portable firefighting
equipment, then attempts to do so should be made. If not await the arrival of
the Fire Brigade and confirm with the Officer in charge that the affected
substation had been electrically isolated.
19.2.5 After receiving the information from the field staff the System Control
Operator shall call the Fire Brigade (Bomba) by dialing 995. When calling the
Fire Brigade:
19.2.5.1 Identify yourself.
19.2.5.2 State the address, location and type of fire.
19.2.5.3 Provide details on any possible hazards.
19.2.5.4 Ensure that site staff is available to direct the Fire Brigade to
the fire site upon their arrival.
19.2.5.5 Carry out any necessary electrical isolation in conjunction with
field staff.

SECTION 20
Work on Gas Turbine Generator Packages and the associated auxiliary plant, which
encompasses both electrical and mechanical plant, shall be carried out in accordance with the
following provisions:
Prior to any work being carried out inside a Gas Turbine Package, either within the
gas turbine compartment or the generator compartment, a Permit to Work on / in
Generation Equipment (Annexure 9) shall be issued to the Person-in-Charge by the
relevant Authorised Person.
Prior to the issue of the Permit to Work, the following mandatory actions shall be
performed and shall be recorded on the Permit to Work:
20.1.1 Close and apply labels to the gas fuel valves supplying the Gas Turbine
Package.
20.1.2 Close and apply labels to the CO2 block valve and inhibit all fire suppression
systems.
20.1.3 Isolate, lock off and apply labels to all electrical sources of supply to the
turbine enclosure and generator enclosure ventilation fans at the motor control
centre (MCC).
In the case of inspection related work, which necessitates the operation of the
Gas Turbine or Generator, a Permit to Work on / in Generation Equipment is
not required however the procedural requirements for such work, which are
documented in S.O.P. PM007 - Work On/In Generation Equipment, are to be
strictly adhered to.
Where work is to be carried out on any fuel system, either internal or external to the
Gas Turbine Package, the requirements of clause 20.1 above shall be complied with.
In addition to this, the gas or diesel line shall be isolated, labelled and purged as
applicable. In the case of gas lines, gas tests shall be carried out to verify that the line
is free from any residual gas and is safe to be worked upon. A Permit to Work shall
then be issued irrespective of whether the work to be performed on the fuel lines is
external or internal to the Gas Turbine Package.

Where work is to be performed on the gas generator or power turbine, clause 20.1 and
clause 20.2, where applicable, shall be complied with. In addition to this, where the
nature of such work involves exposure to any hazards, which may be associated with
the inadvertent rotation of the gas generator, the supply to the gas generator hydraulic
starter shall be isolated, locked off and labelled at the relevant MCC. The isolation
shall also be recorded on the Permit to Work.
Where work is to be performed inside the Gas Turbine Package Exhaust Stack, clause
20.1 shall be complied with. In addition to this, the Vent Line valve shall be opened,
labelled and recorded on the permit to work. Gas detection within the confines of the
workspace shall also be carried out prior to the commencement of any work.
Note: Where the Hot Work is to be carried out either in or on the Gas Turbine
Package, a Permit to Work on/in Generation Equipment shall also be issued to the
Person-in-Charge.
Prior to any work being carried out inside a generator terminal box, a Permit to Work
on/in Generation Equipment as well as a Permit to Work on/Test MV/HV mains
and/or apparatus (Annexure 1) shall be issued to the Person-in-Charge by the relevant
Authorised Persons concerned.
Notwithstanding the requirements stipulated in clause 20.1 for the issue of the Permit
to Work on/in Generation Equipment, the Gas Generator Hydraulic Starter shall also
be isolated in accordance with the requirements of clause 20.3.

In the case of the Permit to Work on/test MV/HV mains and/or apparatus, the
following Electrical Control Points (CP) and Control Point Earths (CPE) are
mandatory requirements for the issue of the permit and as such, shall be recorded on
the permit.
*Note: Since there is no fixed earthing switch installed on the 11kV Generator
Terminal Box, approved portable earths shall be applied after testing.
Prior to any work being carried out inside the MV or HV cable terminal box of a
generator transformer, the provisions of clause 20.6 above shall be complied with.
Prior to any work being carried out inside the MV terminal box of a generator
auxiliary transformer or, on the LV terminals where both the MV and LV terminals
are contained within a common terminal box, the provisions of clause 20.6 above
shall be complied with.
Prior to any work being carried out on the neutral earthing resistor of a generator, the
provisions of clause 20.6 above shall be complied with.
