PETITION

COMES NOW, the petitioner, to this Honorable Court, most respectfully

avers THAT:

1. She is a citizen of the Philippines, 47 years old and a resident of 134

Mabolo, Cebu City;

2. She has satisfactorily completed the required course in law at the

University of Southern Philippines Foundation (USPF) Lahug, Cebu City, in
2020;

3. She is a member of the Philippine Bar having been admitted on June

15, 2020 and holds Roll No. 123456. Machine copy of the Certificate of
Membership of the Philippine Bar is hereto attached, marked Annex “A”,
forming part hereof;

4. She is presently holding office at 12345 Brgy Mabolo, Cebu City with
Telephone Number (032) 12345. Should there be any change of his
address applicant will respectfully inform the Honorable Executive Judge
of such fact within ten days;
 5. She now desires to be appointed as Notary Public for the City of Cebu
under the jurisdiction of this Honorable Court, for the year 2022 and 2023

6. She possesses all the qualifications and none of the disqualifications

for the said office;

7. She is a member in good standing of the Integrated Bar of the

Philippines, Cebu City Chapter as evidenced by the Joint Certification of
Good Standing issued by the Integrated Bar of the Philippines, Cebu City
Chapter, machine copy of which is hereto attached, marked Annex “B”,
forming part hereof;

8. She holds the Official Receipt issued by the IBP, Cebu Chapter,
evidencing payment of his annual membership dues which receipt bears O.
R. No. 12345, issued on Dec. 26, 2021, and that he is a holder of
Professional Tax Receipt bearing number 12345 issued on Dec. 26, 2022 by
the Office of the Treasurer of Cebu City, Cebu;

Machine copies of the said receipts are hereto attached, respectively

marked Annexes “C” and “D”, forming parts hereof;

9. She has not been charged nor convicted of any crime involving moral
turpitude. A copy of the Certification from Office of the Clerk of Court Cebu,
dated DEec. 31, 2021 hereto attached as Annex “E” and per National
Bureau of Investigation (NBI) Clearance with number 12345 valid until
May 23, 2021, hereto attached as Annex “F”;
10. Further, She has no pending administrative case as evidenced by a
Certification from the Office of the Bar Confidant a copy of which is hereto
attached as Annex “G”;

1. She is a member of the Philippine Bar per Certification of Membership

in the Philippine Bar, issued by the same Office hereto attached as
Annex “H”;
 She has not initiated any petition or similar action with any other
Regional Trial Courts in the Philippines, with the Court of Appeals or
with the Supreme Court for the issuance of the Notarial Commission
except the petition stated above.

2. In compliance to the requirements of this Honorable Court for

Notarial Commission, hereto attached are the following additional
annexes and made an integral part of this petition:

a. Three (3) Specimen Signatures as Annex “I”; and

b. Three (3) pieces of Passport sized ID picture as Annex “J”.

WHEREFORE, it is most respectfully prayed of this Honorable Court

that after compliance with the prescribed requirements and hearing,
petitioner be granted a new Commission in the City of Cebu under the
jurisdiction of this Honorable Court, for the year 2022 and 2023.

All other relief and remedies are likewise prayed for.

I, JESERY V. VILLACANAS, of legal age, Filipino, married, resident of

12345 Brgy. Mabolo, after having been duly sworn to in accordance with law,
depose and say, THAT:

I am the petitioner in the above-captioned Petition;

I have caused the preparation and filing of this petition, read and
understood its contents and hereby affirmed them to be true and correct to
the best of my own personal knowledge and understanding and based on
authentic records;

I have not commenced any other actions involving the same issues dealt
within this petition before the Supreme Court, Court of Appeals or in any
other, tribunal or agencies and to the best of my knowledge and
understanding, no such other actions is pending before the Supreme Court,
Court of Appeals, or in any other courts, tribunals or agencies;

Should it come to my knowledge that there is a pending action involving

the same issues dealt within this petition, I undertake to inform this
Honorable Court within five days from knowledge thereof.
